Effects of strontium and cobalt codoped nanotube implants on osseointegration in osteoporotic rats

Yaoyao Yang,Xin Huang,Qianqian Zhang,Dehua Zheng,Wen Gong,Shuqiang Niu,Yanling Yu,Yu Song

Osteoporosis patients are prone to surgical failure after oral implantation due to the low bone density and poor bone healing ability. Improving the osseointegration effect by processing the surface topography of the implant is a promising method. Using the TiO 2 nanotubes as a new type of carrier to load metal elements is a good strategy for preparing multifunctional implant coatings that have both the required osteogenic and angiogenic activities. The present study aimed to investigate the effect of new-type implants on osseointegration by constructing strontium and cobalt codoped nanotubes in osteoporotic rats. Our results demonstrated that it induced more new bone formation and tighter osseointegration which may provide a new strategy for osteoporosis patients who need implant surgery.

Immobilizing bacitracin on titanium for prophylaxis of infections and for improving osteoinductivity: An in vivo study

Bin’en Nie,Haiyong Ao,Teng Long,Jianliang Zhou,Tingting Tang,Bing Yue

Bacitracin immobilized on the titanium (Ti) surface significantly improves anti-bacterial activity and biocompatibility in vitro . In the current study, we investigated the biologic performance (bactericidal effect and bone-implant integration) of bacitracin-modified Ti in vivo . A rat osteomyelitis model with femoral medullary cavity placement of Ti rods was employed to analyze the prophylactic effect of bacitracin-modified Ti (Ti-BC). Thirty-six female Sprague Dawley (SD) rats were used to establish the Ti implant-associated infection. The Ti and Ti-BC rods were incubated with and without Staphylococcus aureus to mimic the contaminated Ti rod and were implanted into the medullary cavity of the left femur, and sterile Ti rods were used as the blank control. After 3 weeks, the bone pathology was evaluated using X-ray and micro-computed tomography (micro-CT) analysis. For the investigation of the Ti-BC implant osseointegration in vivo , fifteen SD rats were divided into three groups (N   =   5), namely Ti, Ti-dopamine immobilized (Ti-DOPA), and Ti-BC. Ti rods were implanted into the left femoral cavity and micro-CT and histological evaluation was conducted after 12 weeks. The in vivo study indicated that Ti-immobilized bacitracin owned the prophylaxis potential for the infection associated with the Ti implants and allowed for the osseointegration. Thus, the multiple biofunctionalized Ti implants could be realized via immobilization of bacitracin, making them promising candidates for preventing the Ti implant-associated infections while retaining the osseointegration effects.

Preparation of Gelatin-Quaternary Ammonium Salt Coating on Titanium Surface for Antibacterial/Osteogenic Properties

Hongyang Song,Lei Xing,Jinjian Wei,Xue Wang,Yaozhen Yang,Pengbo Zhe,Mingming Luan,Jing Xu

Titanium (Ti) and its alloys are widely used in medical treatment, engineering, and other fields because of their excellent properties including biological activity, an elastic modulus similar to that of human bones, and corrosion resistance. However, there are still many defects in the surface properties of Ti in practical applications. For example, the biocompatibility of Ti with bone tissue can be greatly reduced in implants due to a lack of osseointegration as well as antibacterial properties, which may lead to osseointegration failure. To address these problems and to take advantage of the amphoteric polyelectrolyte properties of gelatin, a thin layer of gelatin was prepared by electrostatic self-assembly technology. Diepoxide quaternary ammonium salt (DEQAS) and maleopimaric acid quaternary ammonium salt (MPA−N+) were then synthesized and grafted onto the thin layer. The cell adhesion and migration experiments demonstrated that the coating has excellent biocompatibility, and those grafted with MPA−N+promoted cell migration. The bacteriostatic experiment showed that the mixed grafting with two ammonium salts had excellent bacteriostatic performance againstEscherichia coliandStaphylococcus aureus, with bacteriostasis rates of 98.1 ± 1.0% and 99.2 ± 0.5%, respectively. Keywords:Ti;osseointegration;antibacterial;electrostatic self-assembly technology

Enhanced Osseointegration of Porous Titanium Modified with Zeolitic Imidazolate Framework-8

Xin Zhang,Junyu Chen,Xiang Pei,Jian Wang,Qianbing Wan,Shaokang Jiang,Chao Huang,Xibo Pei

Nanoscale zeolitic imidazolate framework-8 (ZIF-8)-modified titanium (ZIF-8@AHT) can enhance osteogenesis in vitro. In this study, we systematically and quantitatively examined the effects of ZIF-8@AHT on osteogenesis, and investigated its ability to form bone in vivo. First, we coated various quantities of nanoscale ZIF-8 crystals on alkali- and heat-treated titanium (AHT) by controlling the concentration of the synthesis solution. We then characterized the ZIF-8@AHT materials using scanning electron microscopy (SEM), powder X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FT-IR), and determination of the sessile drop contact angle. To illustrate the combined effects of micro/nanotopography and ZIF-8@AHT composition on bone regeneration, we cultured MC3T3-E1 preosteoblast cells on various titanium substrates in vitro by setting pure titanium (Ti) and AHT as control groups. The ZIF-8@AHTs enhanced cell bioactivity compared with AHT and Ti, as evidenced by increased extracellular matrix (ECM) mineralization, collagen secretion and the upregulated expression of osteogenic genes (Alp, Col1, Opg, and Runx2) and osteogenesis-related proteins (ALP and OPG). ZIF-8@AHT-1/8 exhibited better osteogenic activity compared with the other ZIF-8@AHT groups investigated. We subsequently inserted Ti, AHT, and ZIF-8@AHT-1/8 implants into the healed first molars (M1s) of mice, and found that ZIF-8@AHT-1/8 also promoted osseointegration at the bone–implant interface. These results suggest that ZIF-8@AHT-1/8 has great potential for practical application in implant modification.

Nitric Oxide-Mediated Dual-Functional Smart Titanium Implant Coating for Antibacterial and Osseointegration Promotion in Implant-Associated Infections

Zishuo Hou,Kun Wang,Guming Liu,Zhang Yuan,Haowei Peng,Yue Yuan,Hongbo Wei,Tengjiao Wang,Peng Li

The balance of antibacterial and osseointegration-promoting properties on titanium (Ti) implants in a simple and efficient manner is crucial for the management of implant-associated infections, a condition that has become a significant global health concern. Herein, a nitric oxide (NO)-mediated dual-function smart coating with antibacterial and osseointegration-promoting properties is developed for Ti implants. The coating leverages the distinct properties of NO at high and low concentrations to enable an on-demand functional switch. Specifically, antibacterial function is achieved through a rapid release of high-dose NO in response to the infection microenvironment and near-infrared stimulation. Once the infection is resolved and normal physiological conditions are restored, the coating gradually releases low-dose NO to promote osseointegration. In vitro tests confirm that the coating exhibits antibacterial ratio of 97.84% and 97.18% against methicillin-resistant Staphylococcus aureus and its biofilms, respectively, and demonstrates the ability to activate osteoblasts. The rat femoral implant-associated infection model further certifies that the responsive NO release mechanism of the coating efficiently facilitates the on-demand functional switch between antibacterial and osseointegration-promoting properties. Notably, the use of the dual-functional nonantibiotic agent, NO, significantly mitigates the risk of bacterial resistance.

3D Printed Pedicle Screws with Microarc Oxidation Ceramic Interfaces Enhance Osteointegration and Orthopedic Fixation Feasibility

Zehao Chen,Binghao Wang,Chengliang Yang,Zhendong Lv,Yu Wei,Tianming Pan,Fuqing Xuan,Xingdie Zhou,Hao Chen,Hongxing Shen,Liqiang Wang,Yuhui Zhang

Effective osteointegration is of great importance for pedicle screws in spinal fusion surgeries. However, the lack of osteoinductive activity of current screws diminishes their feasibility for osteointegration and fixation, making screw loosening a common complication worldwide. In this study, Ti-6Al-4V pedicle screws with full through-hole design were fabricated via selective laser melting (SLM) 3D printing and then deposited with porous oxide coatings by microarc oxidation (MAO). The porous surface morphology of the oxide coating and the release of bioactive ions could effectively support cell adhesion, migration, vascularization, and osteogenesis in vitro. Furthermore, an in vivo goat model demonstrated the efficacy of modified screws in improving bone maturation and osseointegration, thus providing a promising method for feasible orthopedic internal fixation.

Genistein loaded into microporous surface of nano tantalum/PEEK composite with antibacterial effect regulating cellular response in vitro, and promoting osseointegration in vivo

Shulin Luo,Pei Wang,Min Ma,Zhangyi Pan,Liangyu Lu,Feng Yin,Junfeng Cai

Poly(ether-ether-ketone) (PEEK) with good biocompatibility exhibits high mechanical strengths but bioinert. In addition, tantalum (Ta) possesses outstanding osteogenesis but high density and elastic modulus , and cost. In this study, by blending Ta nanoparticles with PEEK, Ta/PEEK composite (TP) was prepared, which was then treated by concentrated sulfuric acid to form a microporous surface containing Ta particles on TP (TPS). Moreover, genistein (GS) with antibacterial property was loaded into the microporous surface of TPS (TPSG). Compared with TP, the surface properties ( e.g., surface roughness and hydrophilicity) of TPS was obviously improved because of the microporous surface including Ta nanoparticles . Moreover, TPS showed low antibacterial properties because of presence of sulfonic group while TPSG exhibited excellent antibacterial properties due to GS loaded into the microporous surface. Furthermore, compared with TP, TPS obviously promoted attachment and proliferation of MG63 cells, while TPSG with GS remarkably inducing osteogenic differentiation of the cells compared with TPS in vitro . Moreover, in comparison with TP, TPS with optimized surface properties promoted new bone regeneration and osseointegration, while TPSG loading GS further enhanced bone regeneration as well as osseointegration in vi vo. In summary, the GS loaded into microporous surface including Ta nanoparticles of TPSG exhibited antibacterial and osteogenic activity, which would have great potential for bone tissue repair.

Augmenting osteoporotic osseointegration through a temporal release nanocoating-based reversing dysregulated osteogenic microenvironment

Jiaxin Zhang,Haotian Bai,He Liu,Xiaonan Wang,Mingwei Xu,Guokun Zhang,Zexin Di,Xin Zhao,Jincheng Wang,Luquan Ren

Rationale Imbalances in the osteogenic–osteoclastic microenvironment hinder effective osseointegration between the prosthesis and host bone, which is a key factor contributing to the high incidence of prosthesis loosening and periprosthetic fractures in osteoporosis patients. Developing an implant interface that can reverse the dysregulated osteogenic microenvironment is an effective strategy to address this challenge. Methods A novel bioinspired interface with a spatial gradient structure was engineered by 3D-printed porous titanium alloy implants and hollow mesoporous silica nanoparticles coating. Apart from the bioinspired bone microstructure, the amine-functionalized nanocoating of the bioactive interface could also load and temporally sustain the zoledronic acid (ZOL) release. A series of in vitro experiments were carried out to characterize the bioactive interface and to clarify the regulation of osteogenic–osteoclastic balance by ZOL temporal release from the nanocoatings. Afterward, an osteoporotic model was utilized to validate the in vivo bioactive interface osseointegration effect. Results Based on the structural features and chemical properties of the bioactive interface, efficient ZOL loading was achieved. Additionally, the temporal release mechanism of ZOL through free diffusion and ionic bond ionization enabled long-term sustained release. Meanwhile, the early high-concentration local release of ZOL effectively inhibited osteoclast formation and activation in the microenvironment. As the ZOL release decreased over time, the focus shifted to promoting the process of bone formation, thereby reversing the dysregulated osteogenic microenvironment. According to this, we identified the optimal gradient concentration (10 -5 M) of the ZOL temporal release system that efficiently exerts bidirectional regulatory effects. Most notably, the spatial gradient structured interface loaded with this optimal concentration of ZOL (10 -5 M) significantly augmented osseointegration under in vivo osteoporotic conditions. Conclusion Overall, our study provides a novel nanocoating capable of temporally releasing ZOL to reverse the dysregulated osteogenic microenvironment, aiming to maintain the long-term stability of artificial prostheses for osteoporosis patients. The translational potential of this article This study validates the structure–performance-effect relationship of ZOL-releasing nanocoatings with a temporal release mechanism, providing a novel approach to addressing the high incidence of postoperative complications in osteoporotic patients undergoing prosthetic replacement.

Construction of zinc-incorporated nano-network structures on a biomedical titanium surface to enhance bioactivity

Conghui Deng,Xinkun Shen,Weihu Yang,Zhong Luo,Pingping Ma,Tingting Shen,Ju Liu,Kaiyong Cai

Surface topography had been identified as a crucial property that affects osseointegration; thus, topographical modification was the most frequently adopted technique in titanium-based implant research. In this study, ethyl cellulose was employed as an additive to construct a zinc-incorporated nano-network layer onto a titanium surface by the sequential treatments of spin-coating, high-temperature calcination , and alkali heat corrosion. SEM results showed that 20 mg/mL of ethyl cellulose was optimal to fabricate a relatively flat porous coating, and the ideal nano-network structures formed by only 4 h of corrosion. Other results of XPS and ICP further proved that zinc ions were successfully incorporated into the final samples (Ti-Zn0.1, Ti-Zn0.3, and Ti-Zn0.4). Moreover, the in vitro cellular ( e.g. , CCK-8, ALP , mineralization) and bacterial assays presented that Ti-Zn0.3 substrates not only had the greatest proliferation and differentiation capacities for osteoblasts but also possessed relatively strong antibacterial abilities for both Escherichia coli and Staphylococcus aureus . This study provided a new way to rapidly construct the pro-osteogenesis and antibacterial nano-network structures on titanium surfaces for orthopedic application.

Fungal Component Coating Enhances Titanium Implant-Bone Integration

Yuchen Shi,Lintao Wang,Yiming Niu,Na Yu,Panfei Xing,Lei Dong,Chunming Wang

Bioactive coating on bone and dental implants is widely used to increase their integration with the host tissue, with millions of implantations performed globally each year. Numerous coating strategies focus on creating a cell-adhesive surface by employing bone matrix components, but insufficient integration and its associated complications remain a substantial clinical challenge. Designing an implant coating to more comprehensively promote healing while appropriately modulating host immune response is still an unfulfilled promise. This study reports the development of a polysaccharide coating that can improve osseointegration by targeting and regulating inflammatory activity at the implant surface. Zymosan—a fungal component—is screened and covalently grafted onto titanium (Ti) substrates. The zymosan coating specifically stimulates macrophages through toll-like receptors into a proregenerative phenotype, which produces abundant osteogenic/angiogenic cytokines to enhance osteogenic activities locally. Further assessment in a rat femur condyle defect indicates that zymosan coated at a medium dose (0.1 µg mm −2 ) remarkably increases the integration of Ti column with bone tissue, causing no abnormality to the host. This is the first demonstration that modulating inflammation with an immunoactive coating material can promote bone-implant integration, which may inspire new strategies for the design of bone and dental implants.

Biomimetic Self-Assembling Metal–Organic Architectures with Non-Iridescent Structural Coloration for Synergetic Antibacterial and Osteogenic Activity of Implants

Chunlei Zhang,Guangyu Chu,Zesong Ruan,Ning Tang,Cunfeng Song,Qichao Li,Wenjie Zhou,Jiale Jin,Hossam Haick,Yunfeng Chen,Daxiang Cui

Materials in nature feature versatile and programmable interactions to render macroscopic architectures with multiscale structural arrangements. By rationally combining metal–carboxylate and metal–organophosphate coordination interactions, Au25(MHA)18 (MHA, 6-mercaptohexanoic acid) nanocluster self-assembled structural color coating films and phytic acid (PA)–metal coordination complexes are sequentially constructed on the surface of titanium implants. The Lewis acid–base coordination principle applies for these metal–organic coordination networks. The isotropic arrangement of nanoclusters with a short-range order is investigated via grazing incidence wide-angle X-ray scattering. The integration of robust M–O (M = Ti, Zr, Hf) and labile Cu–O coordination bonds with high connectivity of Au25(MHA)18 nanoclusters enables these artificial photonic structures to achieve a combination of mechanical stability and bacteriostatic activity. Moreover, the colorless and transparent PA–metal complex layer allows the viewing of the structural color and surface wettability switching to hydrophilic and makes feasible the interfacial biomineralization of hydroxyapatite. Collectively, these modular metal–organic coordination-driven assemblies are predictive and rational material design strategies with tunable hierarchy and diversity. The complete metal–organic architectures will not only help improve the physicochemical properties of the bone–implant interface with synergistic antibacterial and osseointegration activities but also can boost surface engineering of medical metal implants

In situ fabrication of NIR-II responsive TiO2 bio-metasurface for photothermal antibacterial and enhanced osseointegration

Fan Xiao,Chen-xiao Huang,Jun-hao Dai,Jun-hui Ye,Kang-jie Cheng,Xu Xu,Li-quan Deng,Jia You,Wei Wang,Wan-bing Jin

Titanium implants, widely used in orthopedic and dental applications, often face challenges due to inadequate osseointegration and the risk of postoperative infections , which can potentially lead to implant failure. To address this issue, we developed a titanium implant with an in-situ fabricated anatase TiO 2 metasurface , characterized by a subwavelength thickness and a preferential orientation of the (001) facets. This metasurface, which consists of uniformly distributed nano-crystallites of approximately 30 nm in size, aims to enhance both osseointegration and antibacterial properties. The addition of isopropanol (IPA) to the precursor solution used in the fabrication process played a crucial role in controlling the grain size and coating thickness of the metasurface. This process led to a redshift of the light absorption peak into the near-infrared (NIR)-II region, thereby increasing the penetration depth of photothermal therapy . The implants with TiO 2 metasurface exhibited increased hydrophilicity, rapid apatite formation in simulated body fluid, and an effective photothermal response under NIR-II light, showing significant antibacterial activity against E. coli and S. aureus . In vivo experiments in rabbits indicated superior bone regeneration and osseointegration around the implants with TiO 2 metasurface compared to the control titanium implants. At 4 weeks postoperatively, the bone volume to tissue volume (BV/TV) ratio within the implant was 53.94 %, and by 8 weeks, the implants were nearly entirely filled with new bone, with a BV/TV ratio of approximately 70 %. The findings suggest that the TiO 2 metasurface, which combines antibacterial and bioactive features, has the potential to reduce the risk of postoperative infections and improve clinical outcomes.
