Enhanced mechanical properties and anti-washout of calcium phosphate cement/montmorillonite composite bone-cement for bone-repair applications

Min Wei,Yufei Tang,Lei Chen,Bo Zhang,Shitong Zhang,Kang Zhao,Zixiang Wu

Calcium phosphate cement (CPC) has received extensive attention as a filler for heteromorphic bone defects owing to its excellent biocompatibility, self-curing property, and degradability. However, the low strength and poor anti-washout properties still restrict the use of CPC in the clinical application. In this study, montmorillonite (MMT) was introduced into CPC to prepare CPC/MMT composite bone-cement. The compressive strength of the composite cement was synergistically enhanced by increasing intermolecular bonding and the impeding crack propagation . Owing to its unique lamellar structure , viscosity, and hydrophilicity, CPC bound to MMT via the bridging effect of water molecules and the electrostatic interaction between MMT and Ca 2+ . When subjected to an applied load, crack propagation was further hindered by MMT pull-out, load transfer, crack deflection, crack branching, and crack bridging, which consuming more fracture energy, significantly improving the compressive strength of the cement strength. At 50 wt% MMT addition, the compressive strength of the composite bone cement (CPC+50%MMT) was 48.5 MPa, which is 227.04% higher than that of CPC alone (14.83 MPa). Surprisingly, the anti-washout ability of CPC was significantly improved, as indicated by the decrease in the mass loss of the composite bone cement from 71.25 to 6.48%, overcoming the trade-off between strength and anti-washout properties. The final setting time was shortened from 22 to 11.37 min, and the injectability exceeded 90%. In addition, the composite bone cement demonstrated outstanding cell adhesion and proliferation abilities, making it a promising material for bone-repair therapy.

Facile preparation of novel type polymethyl methacrylate/CoFe2O4/BNNS composite cements and their caloric performance in alternating magnetic fields

Jingxiang Wang,Min Li,Xunwei Liu,Jiangmin Yu,Dicheng Yang,Chunping Yuan,Xiaoying Yin,Yinan Yan

Magnetic hyperthermia generated by functionalized bone cement is a promising approach to treat postosteocarcinoma cancer regeneration. However, it is crucial to minimize the magnetic field intensity using localized high-caloric-generating nanoparticles to avoid the potential hazards of electromagnetic field exposure to living organisms. Herein, in situ hydrothermally synthesized CoFe 2 O 4 (CoFe) nanoparticles and boron nitride nanosheet (BNNS)–supported CoFe complexes (BNCoFe) are incorporated into polymethyl methacrylate (PMMA) cement to prepare a composite implant with high-intensity magnetic–thermal performance and safety. The results show that the CoFe nanoparticles (size = ~70 nm) anchored on BNNS surfaces exhibit a maximum magnetic–thermal ablation temperature of ~42°C within 50 s under an alternating magnetic field of 400 KHz (field frequency) and 30 Oe (field intensity) in vitro. Furthermore, the incorporation of BNNSs into the PMMA matrix notably increases the thermal conductivity of PMMA from 0.07 to 0.25 W mK −1 and improves its mechanical properties (compressive strength increases from 74.0 ± 1.6 to 81.0 ± 1.0 MPa), which is attributed to the crystalline structure of BNNSs in the PMMA matrix. Inductively coupled plasma analysis shows a considerably reduced Co 2+ ion release from PMMA/BNCoFe, indicating its potential as a safe implantation material for hyperthermia treatment.

Enhanced compressive strength and in vitro degradation of porous pectin/ calcium phosphate cement scaffolds by freeze casting without sintered

Hao Zhang,Yufei Tang,Xuan Zhou,Qian Liang,Yani Sun,Bo Zhang,Kang Zhao,Zixiang Wu

The development of biodegradable bone scaffolds that align with the growth cycle of new bone has become a prominent focus in the field of bone defect repair. The sintered porous hydroxyapatite scaffolds exhibit a high degree of crystallization and demonstrate a slow degradation rate following implantation. The apatite phase with low crystallinity can be obtained following the hydration reaction of the non-sintered calcium phosphate bone cement (CPC) scaffolds. However, this leads to the formation of a weak alkaline environment during degradation, ultimately resulting in unsustainable degradation. The present study focuses on the fabrication of porous pectin/CPC composite scaffolds through two approaches during the preparation process of non-sintered porous CPC scaffolds: uniform composite and surface modification. The degradation performance of porous composite scaffolds exhibited an increase with the rise in pectin content. At a pectin content of 70 mg/mL, the scaffolds demonstrated a degradation rate of 13.79% within a span of 30 days. The mechanical properties of the porous scaffolds were enhanced with an increase in pectin concentration through surface coating. When the pectin concentration was 5 wt%, the scaffold exhibited a compressive strength of 8.72 MPa, an elastic modulus of 0.82 GPa, and experienced a degradation rate of 12.57% after a period of 30 days. The local acidic environment not only facilitates the dissolution of bone-like apatite but also enhances cell proliferation and adhesion. Pectin/apatite porous scaffolds exhibit excellent biocompatibility, offering a novel approach for the development of biodegradable bone replacement scaffolds.

Injectable double-crosslinked bone cement with enhanced bone adhesion and improved osteoporotic pathophysiological microenvironment for osteoregeneration in osteoporosis

Lingfei Zhao,Chenyu Liu,Xing Chen,Zirui He,Shuiquan Zhang,Anan Zhang,Shuaimin Tang,Zihan Wu,Changsheng Liu,Yuan Yuan

The osteoporotic bone defect caused by excessive activity of osteoclasts has posed a challenge for public healthcare. However, most existing bioinert bone cement fails to effectively regulate the pathological bone microenvironment and reconstruct bone homeostasis in the presence of osteoclast overactivity and osteoblast suppression. Herein, inspired by natural bone tissue, an in-situ modulation system for osteoporotic bone regeneration is developed by fabricating an injectable double-crosslinked PEGylated poly(glycerol sebacate) (PEGS)/calcium phosphate cement (CPC) loaded with sodium alendronate (ALN) (PEGS/CPC@ALN) adhesive bone cement. By incorporating ALN, the organic-inorganic interconnection within PEGS/CPC@ALN results in a 100 % increase in compression modulus and energy dissipation efficiency. Additionally, PEGS/CPC@ALN effectively adheres to the bone by bonding with amine and calcium ions present on the bone surface. Moreover, this in-situ regulation system comprehensively mitigates excessive bone resorption through the buffering effect of CPC to improve the acidic microenvironment of osteoporotic bone and the release of ALN to inhibit hyperactive osteoclasts, and facilitates stem cell proliferation and differentiation into osteoblasts through calcium ion release. Overall, the PEGS/CPC@ALN effectively regulates the pathological microenvironment of osteoporosis while promoting bone regeneration through synergistic effects of drugs and materials, thereby improving bone homeostasis and enabling minimally invasive treatment for osteoporotic defects.

Physicochemical properties and cytocompatibility of radiation-resistant and anti-washout calcium phosphate cement by introducing artemisia sphaerocephala krasch gum

Chenyu Tang,Teliang Lu,Haishan Shi,Jiandong Ye

The anti-washout ability of calcium phosphate cement (CPC) determines the effectiveness of CPC in clinical application. The γ-ray irradiation method often used in the sterilization process of CPC products is easy to degrade some commonly polymer anti-washout agent, which greatly reduces its anti-washout performance. Artemisia sphaerocephala Krasch gum (ASKG) has the potential of radiation resistance and anti-washout, but no one has considered its performance as anti-washout agent of CPC and mechanism of radiation resistance and anti-washout so far. In this study, we report the effect of γ-ray on ASKG and the effectiveness of ASKG for enhancing of radiation resistance and anti-washout ability of CPC, the physical, chemical properties and in vitro cell behaviors of ASKG-CPCs were also investigated. The results showed that addition of ASKG before and after irradiation could significantly enhanced the anti-washout performance of CPC, which is differ from conventional anti-washout agents. Meanwhile, ASKG-CPCs had an excellent injectable property and biocompatibility, and low content of irradiated ASKG could promote bone differentiation well. We anticipate that the radiation-resistant and anti-washout ASKG-CPCs have potential application prospect in orthopaedic surgery.

Effect of substitutional Sr ion on mechanical properties of calcium phosphate bone cement

Tao Yan,Li Dongxu,Li Yanbao

A novel calcium phosphate cement was developed by adding different amount of SrO to tetracalcium phosphate during the fabrication process. The experimental results show that compressive strength of cements based on tetracalcium phosphate doped with SrO significantly increases with the increase of SrO content, approximately 60 MPa, whilst the mechanical behavior of CPC slightly decreases if 0.7wt% SrO is added. X-ray diffraction measurement confirms the setting reaction of doped cements is similar to that of pure calcium phosphate cement (CPC). Low crystalline hydroxyapatite (HA) is found to be the main constituent of set cement. A mechanical reinforcement effect is resulted from the substitution of Sr ion to Ca 2+ in tetracalcium phosphate (TTCP), accelerating HA crystal formation and a more cross-linked cement structure. In vitro bioactivity tests showed that CPC added with 0.5wt% SrO had a more rapid degradation compared with pure CPC.

Fabrication of the antibiotic-releasing gelatin/PMMA bone cement

Lei Chen,Yufei Tang,Kang Zhao,Xiang Zha,Jiaxin Liu,Hao Bai,Zixiang Wu

High mechanical property especially the exorbitant elastic modulus is the common complication of the clinical polymethylmethacrylate (PMMA) bone cement which will generate the fracture of the adjacent bone and even aseptic loosening, other side effects including excess thermal temperature and low efficiency of the drug release bother the development of the PMMA bone cement. The present study aims to investigate the optimum dosage of gelatin as the porogen, which reduced the elastic modulus of the bone cement to the relatively close level of the cancellous bone. Meanwhile, better antibiotic release profile was introduced by enhancing the specific surface area and interconnectivity than the neat PMMA bone cement. Compared to the PMMA bone cement, the mechanical and thermal property was successfully reduced by the porous structure, the component with 200–400 μm gelatin has the better porosity which resulted in the increasing drug release amount and rate than that of the PMMA bone cement. Furthermore, data analysis and fitting curve could guide experiments, in turn, to obtain the PMMA bone cement with specific requirements of the mechanical properties by the addition of gelatin as the pore-forming agent, and in some cases for predictive purposes, to estimate how a change of gelatin may affect the porosity, mechanical properties, and drug release profiles.

Development of a phase change microcapsule to reduce the setting temperature of PMMA bone cement

Xue Xia,Rui Shi,Jinhui Huang,Yubao Li,Yi Zuo,Jidong Li

The aim of the current study is to alleviate the adverse effect of the strongly exothermic polymerization of polymethyl methacrylate (PMMA) bone cement in clinical applications. In this study, paraffin/poly(methyl methacrylate–methylene bisacrylamide) (paraffin/P(MMA-MBA)) phase change microcapsules (MPn; n = 1, 2) were developed via the emulsion polymerization method. The reduction of the maximum temperature of polymerization (Tmax) and physicochemical properties were evaluated after doping commercial PMMA cement with MPn in specific proportions (10%, 20%, and 30%). The results reveal that the MPn-doped PMMA exhibited an effective reduction in Tmax, which can help alleviate the adverse effect of the strong exothermic reactions during PMMA setting. After doping with the MPn, the mechanical properties of the PMMA cement decrease and the values are close to that of body cancellous bone. The Tmax of the cement doped with 20 wt% MP1 is 37.6°C, which is close to body temperature. Significantly, the setting and compressive properties of the optimized group can still adhere to clinical requirements. The MPn doping PMMA technique holds much promise in clinical practice.

Sequential release of double drug (graded distribution) loaded gelatin microspheres/PMMA bone cement

Lei Chen,Yufei Tang,Kang Zhao,Xiang Zha,Min Wei,Quanchang Tan,Zixiang Wu

Drugs are loaded into PMMA bone cement to reduce the risk of infection in freshly implanted prostheses or to promote the differentiation and growth of osteoblasts. However, the same method of loading of drugs in the bone cement cannot simultaneously achieve an effective antibacterial response and long-term treatment outcomes for osteoporosis based on a patient's clinical needs. In the present study, gentamicin sulfate (GS)/alendronate (ALN)-dual-loaded gelatin modified PMMA bone cement (GAPBC) was fabricated to provide rapid and continuous antibiotic release and long-term anti-osteoporotic therapy. Specifically, the gelatin microspheres were loaded with the drugs using separate methodologies, namely, ALN was loaded during fabrication of the gelatin microspheres after which GS was absorbed onto the gelatin from solution. The results demonstrate that sequential release of the GS and ALN was achieved, GS release playing a major role over the first 24 hours and ALN release dominant after 3 weeks of immersion in PBS, resulting from the graded distribution within the gelatin microspheres, and the final drug release ratio of GS (73.6%) and ALN (68.5%) from the modified bone cement was significantly higher than from PMMA bone cement. Therefore, GAPBC represents a potential drug carrier for future clinical applications.

Fast shaping, biodegradation and microenvironment modulation in bioceramic calcium phosphate cement enhances osteointegration and bone regeneration

Yukai Yin,Xiao Lu,YuMeng Xie,Shiyong Lin,Jin He,Yuanming Ouyang

Critical bone defects require repair materials for optimal treatment. The current range of available materials has limitations, including donor availability, rejection, disease transmission, and inadequate filling. Calcium phosphate bone cement (CPC) is a bone repair material, but most CPCs on the mar-ket have two drawbacks: difficulty degrading and prolonged solidification time. The purpose of this study was to develop a CPC that is rapidly moldable and biodegradable, improves the acid‒base mi-croenvironment, and is more suitable for clinical use. This CPC was prepared from β-tricalcium phosphate bioceramics (TCP) and calcium phosphate monohydrate and is designated a bioceramic calcium phosphate bone cement (BCPC). TCP was used as a control to determine the biocompatibil-ity of BCPC and its impact on osteogenesis-related protein activity. The BCPC and TCP implants were placed in the femurs of rabbits, and X-ray/micro-CT images were obtained at weeks 4, 8, and 12 postoperatively. Additionally, samples from the three time points were stained and analyzed for their osteogenic and degradation properties. BCPC submerged in phosphate buffer reached a neutral pH of 6.98 ± 0.02 on Day 3. In vitro tests revealed that BCPC increased alkaline phosphatase and osteo-pontin activities in MC3T3-E1 cells. The X-ray and micro-CT results revealed that BCPC degraded while the TCP volume remained stable. Micro-CT revealed that BCPC degraded by 26.93% and formed 12.89% new bone by week 12. The histological results showed that BCPC had good bio-compatibility and osteointegration ability. BCPC is characterized by rapid solidification and molding and good biocompatibility, and its degradation rate matches the rate of bone regeneration. BCPC could rapidly improve the surrounding pH, providing the foundation for its clinical application.

Surface functionalized barium sulfate nanoparticles: controlled in situ synthesis and application in bone cement

Chao Fang,Ruixia Hou,Kefeng Zhou,Feibin Hua,Yang Cong,Jianfeng Zhang,Jun Fu,Ya-Jun Cheng

The controlled synthesis of surface functionalized BaSO4 nanoparticles with a diameter below 50 nm is achieved by combining the in situ generation of SO42− and the use of a difunctional surface modification agent, (2-(methacryloyloxy)ethyldimethyl-(3-sulfopropyl)ammoniumhydroxide (MSAH). Particularly, a pre-decomposition process of the SO42− precursor (S2O82−) is applied to decouple the in situ generation of SO42− and precipitation of BaSO4. As a result, the nucleation and growth of the BaSO4 particles is manipulated, which significantly reduces the particle size. Different combinations of the pre-decomposition time and MSAH/BaCl2 molar ratios showed a remarkable influence on the morphology and surface functionality of the BaSO4 particles. Furthermore, the morphology control by using other difunctional surface modification agents e.g. 3-sulfopropylmethacrylate potassium salt (SMPS) and 2-acrylamido-2-methylpropane sulfonic acid (AMSA) is examined. The MSAH-functionalized BaSO4 nanoparticles, used as s filler in bone cements, effectively improved the bending modulus and compressive strength of the poly (methyl methacrylate), PMMA, bone cements. The bone cement with MSAH-functionalized BaSO4 nanoparticles exhibited good radiopacity. Moreover, in vitro cell culture experiments demonstrated an improved biocompatibility compared to the bare BaSO4 particles. The MSAH-functionalized BaSO4 nanoparticles may be an ideal candidate material as a bone cement filler.

Biological activity of hydroxyapatite/poly(methylmethacrylate) bone cement with different surface morphologies and modifications for induced osteogenesis

Youlu Che,Shan Min,Mohong Wang,Minyu Rao,Changyun Quan

Although having advantages such as good mechanical property and rapid curing, the poor absorption and biological activity of hydroxyapatite (HA)/poly(methylmethacrylate) (PMMA) bone cement limits its clinical application. Therefore, it is of vital importance to improve the integration and the biological activity of HA/PMMA bone cement. In this study, spherical and rod-like HA/PMMA bone cement with different content of P(MMA- co -MPS) were chosen to enhance its interface bioactivity. The content of P(MMA-co-MPS) on the surface of HA was 12, 30, 15, and 28%, respectively, corresponding to 0.3 r-HA, 1.5 r-HA, 0.3 s-HA, and 1.5 s-HA, whose calcium-to-phosphorus (Ca/P) ratios were 1.62, 1.5, 1.65, and 1.53, respectively, and were confirmed by energy dispersive spectroscopy. From scanning electron microscope, we found that the spherical HA (s-HA) promotes the biomineralization on the surface of PMMA than rod-like HA (r-HA). In addition, cell experiments in vitro showed that s-HA/PMMA with lower modification degree (0.3) of P(MMA-co-MPS) results in more cell adhesion and more evenly distributed on surface spreading while observed under laser confocal microscope. Meanwhile, in vitro , cell proliferation and alkaline phosphatase activity indicated that s-HA/PMMA bone cement promotes early osteogenic differentiation of rBMSCs in a deeper extent than r-HA/PMMA, demonstrating that 0.3 s-HA/PMMA provides a favorable theoretical basis for the further clinical application of HA/PMMA bone cement as an optimized bone repair material. © 2019 Wiley Periodicals, Inc.
