Zinc (Zn) and its biocompatible and biodegradable alloys have substantial potential for use in orthopedic implants. Nevertheless, pure Zn with a hexagonal close-packed crystal structure has only two independent slip systems, therefore exhibiting extremely low elongation and yield strength in its as-cast condition, which restricts its clinical applications. In this study, as-cast Zn–xTi (titanium) (x = 0.05, 0.10, 0.20, and 0.30 wt.%) binary alloys were hot-rolled and their microstructures, mechanical properties , wear resistance, and cytocompatibility were comprehensively investigated for orthopedic implant applications. The microstructures of both as-cast and hot-rolled Zn–xTi alloys consisted of an α-Zn matrix phase and a TiZn 16 phase, while Zn–0.2Ti and Zn–0.3Ti exhibited a finer α-Zn phase due to the grain-refining effect of Ti. The hot-rolled Zn–0.2Ti alloy exhibited the highest yield strength (144.5 MPa), ultimate strength (218.7 MPa), and elongation (54.2%) among all the Zn–xTi alloys. The corrosion resistance of Zn–xTi alloys in Hanks’ solution decreased with increasing addition of Ti, and the hot-rolled Zn–0.3Ti alloy exhibited the highest corrosion rates of 432 μm/y as measured by electrochemical testing and 57.9 μm/y as measured by immersion testing. The as-cast Zn–xTi alloys showed lower wear losses than their hot-rolled counterparts. The extracts of hot-rolled Zn–xTi alloys at concentrations of ≤ 25% showed no cytotoxicity to MG-63 osteosarcoma cells and the extracts of Zn–xTi alloys exhibited enhanced cytocompatibility with increasing Ti content.

Bacteria-associated late infection of the orthopedic devices would further lead to the failure of the implantation. However, present ordinary antimicrobial strategies usually deal with early infection but fail to combat the late infection of the implants due to the burst release of the antibiotics. Thus, to fabricate long-term antimicrobial (early antibacterial, late antibacterial) orthopedic implants is essential to address this issue. Herein, we developed a sophisticated MAO-I2-PCLx coating system incorporating an underlying iodine layer and an upper layer of polycaprolactone (PCL)-controlled coating, which could effectively eradicate the late bacterial infection throughout the implantation. Firstly, micro-arc oxidation was used to form a microarray tubular structure on the surface of the implants, laying the foundation for iodine loading and PCL bonding. Secondly, electrophoresis was applied to load iodine in the tubular structure as an efficient bactericidal agent. Finally, the surface-bonded PCL coating acts as a controller to regulate the release of iodine. The hybrid coatings displayed great stability and control release capacity. Excellent antibacterial ability was validated at 30 days post-implantation via in vitro experiments and in vivo rat osteomyelitis model. Expectedly, it can become a promising bench-to-bedside strategy for current infection challenges in the orthopedic field. Keywords:antibacterial coating;micro-arc oxidation;orthopedic implants;polycaprolactone;povidone-iodine

Here, we developed a new synthetic method for the production of a new class of polymeric inorganic hybrid biomaterial that has potential for dental implant applications and, in general, other orthopedic applications owing to its excellent mechanical properties and biomechanical compatibility. The new hybrid biomaterial is a composite consisting of polyetherketoneketone (PEKK) and hydroxyapatite (HA). This hybrid material boasts several unique features, including its high HA loading (up to 50 wt%), which is close to that of natural human bone; the homogeneous HA distribution in the PEKK matrix without phase separation; and the fact that the addition of HA has no effect on the molecular weight of PEKK. Nanoindentation analysis was used to investigate the mechanical properties of the composite, and its nano/microstructure variations were investigated through a structural model developed here. Through nanoindentation technology, the newly developed PEKK/HA hybrid biomaterial has an indentation modulus of 12.1 ± 2.5 GPa and a hardness of 0.42 ± 0.09 GPa, which are comparable with those of human bone. Overall, the new PEKK/HA biomaterial exhibits excellent biomechanical compatibility and shows great promise for application to dental and orthopedic devices. Keywords:polyetherketoneketone;hydroxyapatite;composite;hybrid;dental;orthopedic;orthodontic;high performance;mechanical compatibility

Poly-ether-ether-ketone (PEEK) has attracted increasing attention as a promising orthopaedic implant material owing to its excellent mechanical properties and biocompatibility. However, its antibacterial properties must be improved as an implant material. In this study, PEEK was sulfonated to obtain a porous surface, and graphene oxide (GO) was deposited to form a coating with antibacterial activity and biocompatibility. After PEEK was sulfonated for different durations, GO was deposited on the surface to prepare the coating (SPEEK-GO), which was then characterised using scanning electron microscopy (SEM), Raman spectroscopy, and contact angle measurements. The in vitro study included antimicrobial and cellular tests. The results showed that the PEEK sulfonated using a 10-min treatment exhibited a uniform porous structure and provided a better basal surface for the deposition of GO. The SPEEK-GO coating displayed strong antibacterial activity against two common dental pathogens. It exhibited good adhesion and proliferation of MC3T3-E1. Moreover, it showed osteogenic differentiation as bone implant material.

Biofilm-associated infections and the lack of successful tissue integration of biomaterial surfaces are the two main barriers to the long-term service of implanted biomaterials. Development of novel biocompatible antimicrobial materials has provided insights into their potential biomedical applications. Many clinical studies have successfully proved that hydroxyapatite coating has excellent osteogenic activity but lacks antibacterial infection in the early stages after implantation. Rare earth (Re) elements have become promising antibacterial biocides and bone-forming effects. Antibacterial capacity of 14 rare earth elements (Eu, Gd, Ce, Nd, Y, La, Pr, Er, Sm, Ho, Tb, Yb, Lu, Dy) was assayed. The gadolinium (Gd) showed outstanding broad-spectrum antibacterial activity against both Gram-positive and Gram-positive bacteria. Here, we report Gd-HA coatings deposited on titanium (Ti) substrate by liquid thermal spraying. The grain size of Gd-HA decreased slightly after Gd 3+ incorporation. The antibacterial properties of Gd-HA composite coatings were determined against Gram-negative pathogens Escherichia coli and Gram-positive pathogens Staphylococcus epidermidis . The anti-infection performances were assessed by examining bacteria adhesion and biofilm formation on the coatings. The in vitro cytotoxicity of the Gd-doped HA coatings was further measured on human osteoblast cell line by CCK-8 method. The thermal sprayed HA-Re composite coatings show improved antimicrobial and biocompatible properties and great applicable potential in orthopedics.

Bacterial infections have been identified as the main cause of orthopaedic implant failure. Owing to their high antibiotic delivery efficiency, titania nanotubes loaded with antibiotics constitute one of the most promising strategies for suppressing bacterial infections. However, it is difficult to control the drug-release behaviour of such nanotubes. Although sealing the nanotubes with a polymer solution provides sustained release effects to a certain extent, it inevitably influences their initial antibacterial activity. This study reports on the controlled release of gentamicin sulphate (GS) from titania nanotube surfaces whereby their initial antibacterial activity remains unaffected. Titania nanotubes were fabricated via electrochemical anodization and loaded with GS through physical adsorption. Experimental results showed that this loading method is feasible and efficient. The GS-loaded titania nanotubes were further covered by a thin film comprising a mixture of GS and chitosan (GSCH). The release kinetics confirmed that the drug release could be controlled by this thin film. Moreover, such a film was shown to not only inhibit initial bacterial adherence owing to its strong antibacterial properties but also enhance cell viability. Thus, GS-loaded titania nanotubes coated with GSCH have considerable potential as biomaterials for preventing initial release and peri-implant infection in the field of orthopaedics.

Hydrogels are crosslinked hydrophilic polymer networks of high-water content. Although they have been widely investigated, preparing hydrogels with excellent mechanical properties and biocompatibility remains a challenge. In the present work, we developed a novel GelMA/κ-carrageenan (GelMA/KC) double network (DN) hydrogel through a dual crosslinking strategy. The three-dimensional (3D) microstructure of KC is the first network, and covalently crosslinked on the κ-carrageenan backbone is the second network. The GelMA/KC hydrogel shows advantages in physical properties, including higher compression strength (10% GelMA/1% KC group, 130 kPa) and Young's modulus (10% GelMA/1% KC group, 300), suggesting its excellent elasticity and compressive capability. When using a higher concentration of GelMA, the hybrid hydrogel has even higher mechanical properties. In addition, the GelMA/KC hydrogel is favorable for cell spreading and proliferation, demonstrating its excellent biocompatibility. This study provides a new possibility for a biodegradable and high-strength hydrogel as a new generation material of orthopedic implants.

Implant-related infections (IRIs) pose a major challenge in orthopedic applications due to the persistence of biofilms, which are highly resistant to conventional antibiotics. This study introduces oxygen vacancy-engineered Zn–Fe spinel nanoparticles as microwave-responsive antibacterial agents. The oxygen vacancies in the spinel structure enhance reactive oxygen species (ROS) generation under microwave irradiation, providing a dual-mode antibacterial mechanism of thermal and oxidative stress. Zn–Fe spinel nanoparticles were synthesized using a sol–gel method and evaluated for their antibacterial efficacy against Staphylococcus aureus biofilms. Under microwave irradiation, the Zn–Fe spinel demonstrated significant biofilm disruption and bacterial eradication. Mechanistic studies revealed that oxygen vacancies promoted ROS generation, leading to bacterial membrane damage. In vivo experiments using a mouse infection model confirmed the material's antibacterial efficacy and biocompatibility, with no observed toxicity. This study highlights the potential of oxygen vacancy-enhanced Zn–Fe spinel as a microwave-assisted, antibiotic-free strategy for treating deep-seated infections associated with orthopedic implants.

Bacteria-associated infection represents one of the major threats for orthopedic implants failure during their life cycles. However, ordinary antimicrobial treatments usually failed to combat multiple waves of infections during arthroplasty and prosthesis revisions etc. As these incidents could easily introduce new microbial pathogens in/onto the implants. Herein, we demonstrate that an antimicrobial trilogy strategy incorporating a sophisticated multilayered coating system leveraging multiple ion exchange mechanisms and fine nanotopography tuning, could effectively eradicate bacterial infection at various stages of implantation. Early stage bacteriostatic effect was realized via nano-topological structure of top mineral coating. Antibacterial effect at intermediate stage was mediated by sustained release of zinc ions from doped CaP coating. Strong antibacterial potency was validated at 4 weeks post implantation via an implanted model in vivo . Finally, the underlying zinc titanate fiber network enabled a long-term contact and release effect of residual zinc, which maintained a strong antibacterial ability against both Staphylococcus aureus and Escherichia coli even after the removal of top layer coating. Moreover, sustained release of Sr 2+ and Zn 2+ during CaP coating degradation substantially promoted implant osseointegration even under an infectious environment by showing more peri-implant new bone formation and substantially improved bone-implant bonding strength.

Iodine has been known as an effective disinfectant with broad-spectrum antimicrobial potency yet without drug resistance risk when used in clinic. However, the exploration of iodine for antibacterial therapy in orthopedics remains sparse due to its volatile nature and poor solubility. Herein, leveraging the superior absorption capability of metal–organic frameworks (MOFs) and their inherent photocatalytic properties, iodine-loaded MOF surface is presented to realize responsive iodine release along with intracellular reactive oxygen species(ROS) oxidation under near-infrared (NIR) exposure to achieve synergistic antibacterial effect. Iodine is successfully loaded using vapor deposition process onto zeolitic imidazolate framework-8(ZIF-8), which is immobilized onto micro arc oxidized titanium via a hydrothermal approach. The combination of NIR-triggered iodine release and ZIF-8 mediated ROS oxidative stress substantially augments the antibacterial efficacy of this approach both in vitro and in vivo. Furthermore, this composite coating also supported osteogenic differentiation of bone marrow stromal cells, as well as improved osseointegration of coated implants using an intramedullary rat model, suggesting improvement of antibacterial efficacy does not impair osteogenic potential of the implants. Altogether, immobilization of iodine via MOF on orthopedic implants with synergistic antibacterial effect can be a promising strategy to combat bacterial infections.

Zn-Li alloys are considered promising candidate materials for biodegradable orthopedic implant applications due to their high mechanical performance and good biocompatibility. However, the presence of a large number of second-phase particles in this class of Zn alloys can lead to severe localized degradation due to micro-galvanic corrosion, which is detrimental to the mechanical integrity of the alloys during tissue healing in the human body. In this study, we report ultrahigh strength, uniform corrosion, good cytocompatibility, and effective antibacterial ability in Zn- x Li ( x = 0.3, 0.5, 0.7; wt%) alloys achieved through supersaturated solid solution treatment (SSST) at 375 °C under ultrahigh pressure of 3 GPa. A high concentration of β-LiZn 4 phases coexisted with the -Zn matrix in the as-cast Zn- x Li alloys, whereas almost all the Li dissolved into the -Zn matrix of SSST counterparts. The yield strength was 437 MPa for SSST Zn-0.3Li, 592 MPa for SSST Zn-0.5Li, and 686 MPa for SSST Zn-0.7Li. The SSST Zn-Li alloys showed uniform degradation with remarkably reduced degradation rates compared to their as-cast counterparts. The 25% concentration extracts of the Zn- x Li alloys demonstrated no cytotoxicity toward MC3T3-E1 cells, and the alloys exhibited effective antibacterial ability against methicillin-resistant staphylococcus aureus.

This study presents a novel approach to mitigating bacterial infections and antibiotic resistance in medical implants through the integration of iodine-doping and 3D printing techniques. Iodine, with its potent antibacterial properties, and titanium alloy (Ti), a popular metal for implants due to its mechanical and biological properties, were combined via electrodeposition on 3D-printed titanium alloy (3D-Ti) implants. Scanning electron microscopy, energy dispersive spectroscopy, and X-ray photoelectron spectroscopy confirmed the successful creation of iodine-doped titanium implants with improved iodine content due to the rough surface of the 3D-printed material. In vitro studies revealed that these implants significantly inhibited bacterial adhesion and biofilm formation and showed favorable release kinetics for iodine ions. Biocompatibility tests demonstrated no cytotoxic effects and good hemocompatibility. The implants demonstrated enhanced antimicrobial efficacy against Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus) bacteria strains. The findings imply that the integration of iodine-doping and 3D printing technologies is a promising strategy for treating postoperative infections associated with medical implants, consequently bettering the prognosis for patients. Future investigations are encouraged to delve into the long-standing impacts and prospective clinical utility of this groundbreaking methodology.
