Background ICP pregnant women have a unique profile of serum bile acid metabolism, thus the early and accurate identification of ICP patients is beneficial to early appropriate treatment and improvement of pregnancy outcomes. In this study, ultra-high performance liquid chromatography-mass spectrometry/mass spectrometry (UPLC-MS/MS) was used to analyze the 15 types of serum bile acid profiles among patients with ICP in third trimester, patients with cholelithiasis, and patients with hepatitis B virus. The ICP diagnostic model established by partial least squares-discriminant analysis (PLS-DA) was used to screen the differential bile acids for clinical subtypes of ICP. 144 cases of ICP patients were involved in this study, and divided into four subgroups according to serum level of TBA, DBIL, and ALT.Results (1) The differential serum bile acid profiles of ICP group and normal pregnant women were DCA, TDCA, TCA, GDCA and GLCA. (2) The differential serum bile acid profiles of the ICP1 group (ICP with jaundice) and normal pregnant women were TCDCA, TCA, GCA, GCDCA, TUDCA and GUDCA. (3) The differential serum bile acid profiles of the ICP3 group (Hyperchoicemia of pregnancy) and normal pregnant group was GUDCA, LCA, GLCA, UDCA, TUDCA, CDCA, and TLCA (P < 0.05). (4) The differential serum bile acid profiles of ICP4 group (idiopathic aminotransferase abnormality during pregnancy) and normal pregnant group was UDCA, GUDCA, TUDCA, GCA and GLCA (P < 0.05). (5) The occurrence of meconium-stained amniotic fluid, premature delivery and cesarean section in ICP1 group was significantly higher than normal group, ICP2 group, ICP3 group, and ICP4 group (P < 0.05); The occurrence of meconium-stained amniotic fluid, premature delivery and cesarean section in ICP2 group, ICP3 group, and ICP4 group was significantly higher than normal group (P < 0.05), but no difference was found among ICP2 group, ICP3 group, and ICP4 group (P > 0.05).Conclusion Maternal serum bile acid profiles are useful to differentiate the four subtypes of ICP. ICP with jaundice could be an important predictor of adverse pregnancy outcomes of ICP.

Organ-on-a-chip (OOC) is now becoming a potential alternative to the classical preclinical animal models, which reconstitutes in vitro the basic function of specific human tissues/organs and dynamically simulates physiological or pathological activities in tissue and organ level. Despite of the much progress achieved so far, there is still an urgent need to explore new biomaterials to construct a reliable and efficient tissue–tissue interface and a general fabrication strategy to expand from single-organ OOC to multi-organ OOC in an easy manner. In this paper, we propose a novel strategy to prepare double-compartment organ-on-a-chip (DC-OOC) using electrospun poly( l -lactic acid)/collagen I (PLLA/Col I) nanofiber membrane as tissue–tissue interface. The unique features of PLLA/Col I nanofiber membrane like excellent biocompatibility, strong affinity to multiple cells, adjustable orientation, controllable thickness and porosity endow the tissue–tissue interface with excellent semi-permeability, appropriate mechanical support, inducible cell orientation, good cell adhesion and proliferation. The integration of 3D printing technology during the fabrication process enables precise size control of the tissue–tissue interface and stable bonding with microfluidic channels. More importantly, our fabrication strategy and OOC configuration makes it easy to extend from DC-OOC to multi-compartment organ-on-a-chip (MC-OOC). To show its possible application, in vitro jaundice disease model is established by constructing blood vessel/skin/liver/lung organ-on-a-chip via MC-OOC. The downward trends of the cell viability after perfusion of bilirubin, the variation in cell sensitivity to bilirubin for different type of cells and recovery of cell viability after blue light therapy prove the feasibility of this jaundice disease model. We believe this general strategy of constructing tissue–tissue interface and multi-organ OOC can be used for many other in vitro physiological and pathological models. The concentration of bilirubin in serum is a key indicator for liver function, because excess bilirubin accumulation may lead to serious health problems like Jaundice. It is known that in comparison with total bilirubin, the concentration of hydrophobic free bilirubin is more closely associated with Jaundice because of its capability to cross the capillary walls and even blood-brain barrier. However, it is not easy to detect the low concentration of free bilirubin (∼ nM) in the presence of large amount of protein-bound bilirubin (> 150 μM), and no point-of-care detection tool is available. In the present work, direct detection of free bilirubin was realized by using surface enhanced Raman spectroscopy (SERS). By mimicking the in vivo chemical interaction between glucuronic acid and bilirubin, glucose functionalized Au nanoparticles were synthesized for selective capture and detection of free bilirubin. By developing a convenient detection procedure of using robust Au array chip and internal standard calibration method, the sensitive detection of free bilirubin with detection limit down to 0.1 nM was achieved on a handhold Raman spectrometer by monitoring its fingerprint spectra. The clinical applicability was demonstrated with analyzing the spiked and real blood samples. As a proof of concept, the binding constant between bilirubin and human serum albumin (HSA) was determined with the present method. The influence of the binding capacity of the HSA for bilirubin in the presence of competing molecule salicylate sodium was also quantitatively measured, which demonstrated the importance of detection of free bilirubin instead of total bilirubin for warning the risk of bilirubin encephalopathy.

Point-of-care testing (POCT) devices represent a growing field that aims to develop low-cost, rapid, sensitive diagnostic testing platforms that are portable and self-contained. Surface-enhanced Raman spectroscopy (SERS) is an approach has shown high potential in POCT technology. However, the specificity or ability to uniquely detect a desired biomarker in complex biological samples is a key factor for translating SERS technologies to POCT. Herein, we fabricated cellulose SERS strips (CS) decorated with novel plasmonic nanoparticles, termed graphene-isolated-Au-nanocrystals (GIANs), for the portable detection of complex biological samples. This CS@GIANs SERS strip was used to detect free bilirubin (BR) in the blood of newborns, a biomarker of jaundice, without sample labeling or prepreparation. CS@GIANs showed superior affinity to hydrophobic BR molecules compared to typical SERS substrate, which reduced the steric hindrance effect from the nonspecific binding of BR with serum albumin in blood and improved sensitivity. Meanwhile, with the separation property of cellulose chromatography papers, CS@GIANs showed superior anti-interference to other biomolecules that had been previously adsorbed on the SERS strip. Moreover, the SERS signal from the graphitic shell of GIANs could be used as a stable internal calibration standard, which improved the reproducibility and accuracy of Raman analysis. Such a cellulose SERS strip holds high potential for enhancing current efforts in the development of rapid and low-cost point-of-care diagnostic testing.

Neonatal hyperbilirubinemia (jaundice) is a common disease with high incidence. Currently, hemoperfusion (blood exchange transfusion) is always used for the treatment of severe hyperbilirubinemia. However, there are several limitations of blood exchange transfusion, such as the lack of a safe blood supply and blood transfusion complications, while the main problem of hemoperfusions is the inefficient performance of current bilirubin absorbent, which makes it impossible to administer to the neonatal hyperbilirubinemia cases. Identification of a bilirubin hemoperfusion adsorbent with overall excellent performance is the most crucial key to solving the above clinical problems. In this work, a novel bilirubin adsorbent was customized based on molecular design strategy, namely, functional polymeric microspheres were first fabricated by the copolymerization of glycidyl methacrylate with other monomers, and then modified with an amine component, followed by linking with a functional spacer molecule, and finally immobilized with a functional ligand (albumin). Its chemical and physical properties were characterized; its adsorption performances for bilirubin were investigated under clinical conditions; its medical safety were evaluated by hemolysis test, thrombotest, and platelet adhesion test. The results showed that the adsorbent had good adsorption performance for bilirubin compared to a popular commercial clinical bilirubin hemoperfusion column; and it also had excellent medical safety evaluations. This study sheds a new light on how to develop hemoperfusion bilirubin adsorbents with overall good clinical performance as well as a reference for later studies on this topic.

We report a paper-based surface-enhanced Raman spectroscopy (SERS) biosensor integrating the enrichment capability, namely enPSERS biosensor, for the sensitive, label-free detection of free bilirubin in blood serum for the accurate diagnosis of jaundice and its related diseases. This biosensor comprises multifunctional graphene oxide-plasmonic gold nanostar (GO-GNS) hybrids decorated on the filter paper, which integrates the high sensitivity of SERS detection, enrichment for serum bilirubin and fluorescence superquenching capability of GO-GNS hybrids for sensitive detection of serum bilirubin. The study of adsorption kinetics reveals that both electrostatic and π-π interactions between the GO-GNS hybrids and targets are responsible for the enrichment of bilirubin, and the adsorption process follows the pseudo-second-order kinetic model . The results of SERS detection of bilirubin in blood serum show two differential linear response ranges from 5.0 to 150 μM and 150–500 μM with the detection limit as low as 0.436 μM. The comparison of the results obtained from our present enPSERS biosensor with the commercial diazo reaction method for determination of free bilirubin in blood serum reveals the clinical effectiveness and suitability of the developed paper-based SERS biosensor. We believe that this sensitive and label-free SERS biosensor holds considerable promise for clinical translation in accurate diagnosis of jaundice.

Ethnopharmacological relevance As one of the most commonly used herbs, Artemisia capillaris Thunb. (ACT) display favorable effect in the treatment of jaundice. However, mechanism of ACT in the treatment of jaundice remains unclear at present, which limits its development and application. Aim of the study To investigate effect and mechanism of Artemisia capillaris Thunb. (ACT) in the treatment of jaundice using pharmacodynamics , network pharmacology and metabolomics. Methods Effect of ACT in treating jaundice was evaluated by biochemical assays and pathological observation using the α-naphthyl isothiocyanate (ANIT)-induced mice. Jaundice-relieving mechanism of ACT was investigated by integration of network pharmacology and metabolomics. Results After the mice with jaundice were administrated ACT extract for 9 days, compared to that of the model group, serum D-BIL, T-BIL and ALP levels of the mice in the low, medium, high dose of ACT group decreased by 39.81%, 15.30% and 16.92%; 48.06%, 42.54% and 36.91%; 26.90%, 12.34% and 16.90%, respectively. The pathologic study indicated that ACT improved the symptoms of liver injury of the mice with jaundice. The network of herb (i.e., ACT)-components-targets-disease (i.e., jaundice) was established, which consisted of 17 components classified in flavonoids , chromones , organic acids, terpenoids , and 234 targets related to treatment of jaundice. Metabolomics analysis showed that, compared to that in the model group, level of 8 differential metabolites were upregulated and level of 29 differential metabolites were downregulated in the mice liver in the ACT group, respectively. The main metabolic pathways involved in treatment of jaundice by ACT were pantothenate and CoA biosynthesis, glutathione metabolism, biosynthesis of unsaturated fatty acids , primary bile acid biosynthesis in the liver, respectively. The integrated analysis of network pharmacology and metabolomics showed that 3α,7α,12α a-Trihydroxy-5β-cholanate, glycocholate , taurocholate , pantetheine 4′-phosphate, and d-4′-phosphopantothenate were the potential biomarkers for treatment of jaundice, and AKR1C4, ALDH2 and HSD11B were the potential drug targets in the treatment of jaundice by ACT. Conclusion The study based on metabolomics and network pharmacology indicated that ACT can display favorable jaundice-relieving effect by its multiple components regulating multiple biomarkers, multiple targets and multiple pathways, and may be a rational therapy for the treatment of jaundice.
