Bone substitutes are widely used in maxillofacial and oral surgeries. However, in clinical practice, bone substitutes with various forms, including separated particulates, powders, and blocks, have exhibited poor handling properties and space maintenance characteristics, resulting in long surgery procedures and unstable volume of the newly formed bone. Movable separated particulates with high stiffness have induced local inflammatory responses that hinder bone regeneration. The present study aimed to develop a new method to enhance the stability and operability of bone substitutes commonly used in dentistry by premixing with photocurable hydrogel GelMA. The GelMA-encapsulated particulate had a strong capacity to aggregate separated particulates and firmly attach to the host bone defect after photocuring compared to particulates alone. Additionally, macrophages at the surface of the GelMA-stabilized particulates tended to present a more M2-like phenotype than those at the surface of Bio-Oss®, leading to more MMR+ multinucleated giant cell formation and the induction of blood vessel invasion and new bone formation. In conclusion, this hydrogel-coated bone substitute strategy facilitates bone regeneration with increased operability, a stable volume of osteogenic space, and a favorable osteogenic microenvironment, indicating its potential value in the field of maxillofacial and oral surgeries when bone substitutes are needed.

This study evaluated biological compatibility and antibacterial activity of a bone substitute material consisting of silver-doped hydroxyapatite (AgHA) and Bio-Oss ® with different mixture ratios in vitro and investigated its antibacterial mechanism. AgHA was synthesized by a chemical precipitation method. After characterization, AgHA was mixed with Bio-Oss ® at three ratios: 1:1, 1:2, and 1:4 by weight. Then, Porphyromonas gingivalis (Pg) and Fusobacterium nucleatum (Fn) were used to test the antibacterial activity of the mixture. Human periodontal ligament fibroblasts and rat bone marrow stromal cells were selected for cytocompatibility experiments. According to results, the peak value of the size of the AgHA was concentrated in the 100–200 nm range, and AgHA particles consisted of short rods. It was confirmed that the structure of AgHA was similar to that of standard hydroxyapatite. All three mixture ratios exhibited obvious antimicrobial properties, which increased with increasing AgHA. According to the effects on the expression of bacterial virulence genes, groups 1:1 and 1:2 both negatively affected Pg and Fn more significantly than group 1:4. Cytotoxicity experiments showed that 1:1 caused little cytotoxicity, while groups 1:2 and 1:4 exerted no significant cytotoxicity. Considering its biological compatibility and antibacterial activity, group 1:2 is the most recommended. © 2017 Wiley Periodicals, Inc. J Biomed Mater Res Part B: Appl Biomater, 106B: 410–420, 2018.

Objective The aim of this study was to investigate the effect of IL-10 on the phenotype polarization of macrophages and osteogenesis in diabetes mellitus type 2 (T2DM) rat jaw defects. Methods Lipopolysaccharide (LPS) and interleukin-10 (IL-10) were chosen to induce the polarization of macrophages. In vitro assessment included wound-healing assay, western blotting, and alizarin red staining after co-culture of the bone marrow-derived mesenchymal stem cells (BMSCs) and induced macrophages. For in vivo study, IL-10 was loaded on GelMA-Heparin and applied to bone defects of the alveolar ridge in diabetic rats, while Bio-Oss Collagen, simple GelMA-Heparin, and blank control groups were set for contrast experiment. The mandibles of rats were processed for micro-computed tomography, histology, and immunohistochemistry 1 week and 4 weeks after the operation. Results IL-10 induced expression of arginase 1, TGF-β1, EGR2, and Mannose Receptor (CD206), whereas LPS induced expression of iNOS, TNF-α, IL-6, CD80. The BMSCs co-cultured with macrophages induced by IL-10 showed increased migration, osteogenic differentiation, and mineralization in vitro. Notably, the IL-10-laden GelMA-Heparin group showed quicker new bone formation and a higher M2/M1 ratio of macrophages in the jawbone defect area compared with the control groups. Conclusions IL-10 can stably induce macrophages to M2 type, thereby influencing BMSCs and improving the osteogenesis of jaw bone defects.
