Multi-omics signatures reveal genomic and functional heterogeneity of Cutibacterium acnes in normal and diseased skin

Tianze Yu,Xiaoqiang Xu,Yang Liu,Xiaokai Wang,Shi Wu,Zhuoqiong Qiu,Xiaochun Liu,Xiaoyu Pan,Chaoying Gu,Shangshang Wang,Lixin Dong,Wei Li,Xu Yao

Cutibacterium acnes is the most abundant bacterium of the human skin microbiome since adolescence, participating in both skin homeostasis and diseases. Here, we demonstrate individual and niche heterogeneity of C. acnes from 1,234 isolate genomes. Skin disease (atopic dermatitis and acne) and body site shape genomic differences of C. acnes , stemming from horizontal gene transfer and selection pressure. C. acnes harbors characteristic metabolic functions, fewer antibiotic resistance genes and virulence factors, and a more stable genome compared with Staphylococcus epidermidis . Integrated genome, transcriptome, and metabolome analysis at the strain level unveils the functional characteristics of C. acnes . Consistent with the transcriptome signature, C. acnes in a sebum-rich environment induces toxic and pro-inflammatory effects on keratinocytes. L-carnosine, an anti-oxidative stress metabolite, is up-regulated in the C. acnes metabolome from atopic dermatitis and attenuates skin inflammation. Collectively, our study reveals the joint impact of genes and the microenvironment on C. acnes function.

Antioxidant Activities of the Cell-Free Supernatant of a Potential Probiotic Cutibacterium acnes Strain CCSM0331, Isolated From a Healthy Skin

Li Shao,Jieyan Huang,Yan Li,Laiji Ma,Yujie Niu,Wen Jiang,Chunying Yuan,Tianming Bai,Suzhen Yang

Objective Oxidative stress activates the reactive oxygen species (ROS) and excessive ROS can damage skin cells, initiating oxidative stress responses that contribute to inflammation, aging, and other skin issues. As a resident skin bacterium, Cutibacterium acnes ( C. acnes ) plays an important role in maintaining skin homeostasis and provides antioxidant benefits. However, the metabolite components and mechanisms of C. acnes exerting antioxidant activity are not yet clear. This study aimed to analyze the potential antioxidant effects of C. acnes cell-free supernatant and the mechanisms. Methods The antioxidant effects were evaluated by measuring the scavenging activities of 2,2-diphenyl-1-picrylhydrazyl (DPPH), 2,2′-azinobis (3-ethylbenzothiazoline-6-sulfonic acid ammonium salt) (ABTS) radicals, and hydroxyl radicals, as well as the effects on ROS levels in menadione-induced primary human keratinocytes in vitro. Additionally, western blot analysis was performed to assess the antioxidant effects of the C. acnes CCSM0331 cell-free supernatant (CFS). Results C. acnes CCSM0331 was isolated from the facial skin of healthy individuals. This strain, classified as type II, is associated with healthy skin. The CFS of strain CCSM0331 contained various short-chain fatty acids (SCFAs), glutathione peroxidase (GSH-Px), and total superoxide dismutase(T-SOD), exhibiting strong DPPH and ABTS radical scavenging capabilities, thus demonstrating substantial antioxidant activity. In a reactive oxygen species model induced by menadione in primary human keratinocytes, the addition of 5% of the fermentation supernatant from this strain significantly reduced ROS levels, indicating a notable ROS-scavenging effect. Western blot analysis further confirmed that the CCSM0331 fermentation supernatant activated the expression of Nrf-2 and HO-1 proteins, thereby activating the Nrf-2 oxidative stress pathway and exerting antioxidant effects. Conclusion C. acnes CCSM0331 is a promising skin probiotic with notable antioxidant properties. The activity of this strain exhibited significant free radical scavenging activity, suggesting its potential application in the development of antiaging products. This study provides theoretical support for the screening of functional skin bacteria or skin probiotics.

ZnO colludes with C. acnes in healing delay and Scar hyperplasia by barrier destruction

Zhang Fenglan,Wang Tianyi,Wang Wenqiao,Lv Yaqian,Qu Yingshan,Liu Danping,Sun Xiaoyue,Kong Xiaoying,Wang Changyuan,Shi Jinsheng

As an important component of sunscreen products for sensitive skin, the potential damage mechanism of ZnO nanoparticles on skin surface with barrier structure or function defect caused by Cutibacterium acnes (C. acnes) has not been elucidated, which poses a serious challenge for reasonable selection of sunscreen products for acne-infected skin. In this work, we demonstrated for the first time that C. acnes induced significant changes in the membrane permeability and intracellular pH of fibroblasts through lipase up-regulation and lipid peroxidation, promoting endocytosis and ionization of ZnO NPs. High amounts of Zn2 + further delayed acne wound healing and aggravated scar hyperplasia by intervening matrix metalloproteinase-9 (MMP-9) and TGF-β1/Smad pathway. MMP9 was confirmed to be the key target of ZnO in delaying acne wound healing by the wound regulatory effects of MMP9 agonist and MMP9 inhibitor. In summary, this work clarified the interaction mechanism between ZnO NPs and acne skins, providing guideline for the application of physical sunscreens for special skins.Graphical abstract

Reversal Potential of Multidrug Antibiotic Resistance in Cutibacterium acnes by Ethanol Extracts of Rhubarb

Doudou Yang,Yu Cui,Sen Zhu,Ruoliang Wang,Haijun Xu,Guanjie Zhao,Dandan Zhang,Yinku Liang

Graphical 1. Ethanol extract of Rhubarb effectively reversed regular clinical antibiotic resistance, and was confirmed by growth curves and oxidative markers. 2. Reversal effects were observed with erythromycin (1000-fold), clindamycin (250-fold) and tetracycline (2–4-fold). 3. The extract has stronger reversal effects on antibiotics with higher resistance. Cutibacterium acnes ( C. acnes ) is a major contributor to acne inflammation and exhibits significant antibiotic resistance, but research focusing on reversing this resistance is limited. Rhubarb, a natural plant with known therapeutic effects, shows potential in combating antibiotic resistance, however, no studies have been explored before. The aim of this study was to investigate rhubarb's ability to reverse antibiotic resistance in C. acnes . Strains 11,827 and 6919 were cultured and passaged with 0.019 μg/mL ethanol extract, and antibiotic sensitivity was monitored from passage 0 to 12. The extract effectively reversed antibiotic resistance, and was also confirmed by growth curves and oxidative markers. The impact varied across antibiotics, with the most significant reversal being erythromycin (1000-fold), followed by clindamycin (250-fold), and a weaker effect for tetracycline (2–4-fold). This suggests that the extract has a stronger reversal effect on antibiotics with higher resistance. LC-MS analysis identified flavonoids and heterocyclic compounds were may be key active components, with (-)-epicatechin being the most abundant and crucial for antibacterial and reversal activities. The study suggests a new strategy of using rhubarb ethanol extract as a promising acne treatment with much lower resistance, with vital advantages over conventional antibiotics. These also provide new insights into using herbal plants to combat antibiotic resistance.

Commensal Cutibacterium acnes-derived indolelactic acid safeguards skin barrier function through the aryl hydrocarbon receptor

Shan Zhang,Fang Li,Xiaoqiang Xu,Tianze Yu,Yang Liu,Xiaokai Fang,Yuan Zhou,Beilei Xu,Yu Zhang,Yang Luo,Qianjin Lu,Xiaochun Liu,Wei Li,Xu Yao

Skin microbial dysbiosis is a key characteristic of atopic dermatitis (AD). Although Cutibacterium acnes ( C. acnes ) is the most abundant bacterium in the skin microbiota, its precise functions in AD remain unclear. Here, whole-genome sequencing of C. acnes isolates reveals strain-level genomic and functional heterogeneity between AD patients and healthy individuals. Metabolomic profiling identifies reduced indolelactic acid (ILA), a tryptophan metabolite, in AD skin and in cultures of AD-associated C. acnes strains . In murine models of epidermal-barrier-damaged and AD-like dermatitis, topical ILA application activates the aryl hydrocarbon receptor (AhR) pathway in keratinocytes, upregulating tight junction proteins and suppressing interleukin (IL)-33 to restore barrier integrity and mitigate dermatitis. A proof-of-concept clinical trial confirms ILA’s efficacy in reducing transepidermal water loss and alleviating AD symptoms. Our study establishes C. acnes -derived ILA as a crucial regulator of skin barrier repair via AhR signaling, offering therapeutic potential for AD. Clinical trial registration: ChiCTR2400090988.

Innate lymphoid cell-based immunomodulatory hydrogel microspheres containing Cutibacterium acnes extracellular vesicles for the treatment of psoriasis

Yujie Xu,Yuyang Gan,Fangfang Qi,Xinyu Lu,Xiaofei Zhang,Jiarui Zhang,Hailin Wang,Yue Li,Zhiyang Zhou,Xusheng Wang,Dongqiang Zeng,Feng Lu,Chunhua Zhang,Biao Cheng,Zhiqi Hu,Gaofeng Wang

Psoriasis is a chronic skin inflammation influenced by dysregulated skin microbiota, with the role of microbiota in psoriasis gaining increasing prominence. Bacterial extracellular vesicles (bEVs) serve as crucial regulators in the interaction between hosts and microbiota. However, the mechanism underlying the therapeutic potential of bEVs from commensal bacteria in psoriasis remains unclear. Here, we investigated the therapeutic role of Cutibacterium acnes ( C. acnes )-derived extracellular vesicles (CA-EVs) in psoriasis treatment. To prolong the active duration of CA-EVs, we encapsulated them in gelatin methacrylate (GelMA) to fabricate hydrogel microspheres (CA-EVs@GHM) with sustained release properties. As GelMA degraded, CA-EVs were gradually released, maintaining a high concentration in mouse skin even 96 h post-treatment. In human keratinocyte cells (HaCaT), CA-EVs@GHM enhanced resistance to Staphylococcus aureus (S. aureus ), promoted proliferation and migration of HaCaT cells exposed to S. aureus , and significantly reduced the expression of inflammatory genes such as interleukin ( IL )-6 and C-X-C motif chemokine ligand 8 ( CXCL8 ). In vivo , CA-EVs@GHM, more potent than CA-EVs alone, markedly attenuated proinflammatory gene expression, including tumor necrosis factor ( TNF ), Il6, Il17a, Il22 and Il23a in imiquimod (IMQ)-induced psoriasis-like mice, and restored skin barrier function. 16S rRNA sequencing revealed that CA-EVs@GHM might provide therapeutic effects against psoriasis by restoring microbiota diversity on the back skin of mice, reducing Staphylococcus colonization, and augmenting lipid metabolism. Furthermore, flow cytometry analysis showed that CA-EVs@GHM prevented the conversion of type 2 innate lymphoid cells (ILC2) to type 3 innate lymphoid cells (ILC3) in psoriasis-like mouse skin, reducing the pathogenic ILC3 population and suppressing the secretion of IL-17 and IL-22 . In summary, our findings demonstrate that the long-term sustained release of CA-EVs alleviated psoriasis symptoms by controlling the transformation of innate lymphoid cells (ILCs) subgroups and restoring skin microbiota homeostasis, thus offering a promising therapy for psoriasis treatment. Statement of significance Cutibacterium acnes , which is reduced in psoriasis skin, has been reported to promote skin homeostasis by regulating immune balance. Compared to live bacteria, bacterial extracellular vesicles (bEVs) are less prone to toxicity and safety concerns. bEVs play a pivotal role in maintaining bacterial homeostasis and modulating the immune system. However,bEVs without sustained release materials are unable to function continuously in chronic diseases. Therefore, we utilized hydrogel microspheres to encapsulate Cutibacterium acnes ( C. acnes )-derived extracellular vesicles (CA-EVs), enabling long term sustained release. Our findings indicate that, CA-EVs loaded gelatin methacrylate hydrogel microspheres (CA-EVs@GHM) showed superior therapeutic effects in treating psoriasis compared to CA-EVs. CA-EVs@GHM exhibited a more significant regulation of pathological type 3 innate lymphoid cells (ILC3) and skin microbiota, providing a promising approach for microbiota-derived extracellular vesicle therapy in the treatment of skin inflammation.

Transcriptomic and metabolomic analyses of the antimicrobial activity of phenoxyethanol against phylotype IA1 and II Cutibacterium acnes

Shao Li,Wang Lizhuang,Li Yan,Ma Laiji,Jiang Hong,Liu Fei,Jiang Shanshan,Bai Tianming,Yang Suzhen

AimsPhenoxyethanol is a broad-spectrum antimicrobial agent widely used in cosmetic formulations. However, its antibacterial effects on different skin bacteria, particularly the predominant Cutibacterium acnes and its various phylotypes, remain unclear. The objective of this study was to examine the antimicrobial effects of phenoxyethanol on C. acnes and explore the mechanism. Methods and ResultsPhenoxyethanol exhibited strong antimicrobial effects against both C. acnes ATCC6919 (phylotype IA1) and CCSM0331 (phylotype II), achieving a minimum inhibitory concentration (MIC) of 0.5% (v/v). Sub-MIC concentrations showed a stronger inhibitory effect on CCSM0331. RNA-seq and metabolomic analyses revealed that phenoxyethanol disrupted cell membrane integrity and influenced essential metabolic pathways, such as energy metabolism, amino acid metabolism, and pyrimidine metabolism. Additionally, glycolysis and the Wood-Werkman cycle were inhibited in CCSM0331 but enhanced in ATCC6919. The expression of genes involved in porphyrin metabolism, associated with inflammation, was significantly reduced.ConclusionsPhenoxyethanol exhibits the antimicrobial activity against C. acnes, with differential effects on phylotypes, targeting critical metabolic pathways and cellular processes. These findings indicate its potential for acne treatment.

Utilizing the Photodynamic Properties of Curcumin to Disrupt Biofilms in Cutibacterium acnes: A Promising Approach for Treating Acne

Nana Zheng,Yuanyuan Xie,Meng zhou,Yuzhen Liu,Haoxiang Xu,Rong Zeng,Chunping Wan,Min Li

Background : The treatment of acne vulgaris is often challenging due to the antibiotic resistance frequently observed in Cutibacterium acnes ( C.acnes ), a prevalent bacterium linked to this condition. Objective : The objective of this research was to examine the impact of curcumin photodynamic therapy (PDT) on the survival of C.acnes and activity of biofilms produced by this microorganism. Methods : Following the Clinical and Laboratory Standards Institute (CLSI) guidelines, we assessed the drug sensitivity of 25 clinical C.acnes strains to five antibiotics (erythromycin, clindamycin, tetracycline, doxycycline, minocycline) and curcumin by implementing the broth microdilution technique. In addition, we established C.acnes biofilms in a laboratory setting and subjected them to curcumin-PDT(curcumin combined with blue light of 180 J/cm 2 ). Afterwards, we evaluated their viability using the XTT assay and observed them using confocal laser scanning microscopy. Results : The result revealed varying resistance rates among the tested antibiotics and curcumin, with erythromycin, clindamycin, tetracycline, doxycycline, minocycline, and curcumin exhibiting resistance rates of 72%, 44%, 36%, 28%, 0%, and 100%, respectively. In the curcumin-PDT inhibition tests against four representative antibiotic-resistant strains, it was found that the survival rate of all strains of planktonic C. acnes was reduced, and the higher the concentration of curcumin, the lower the survival rate. Furthermore, in the biofilm inhibition tests, the vitality and three-dimensional structure of the biofilms were disrupted, and the inhibitory effect became more significant with higher concentrations of curcumin. Conclusion : The results emphasize the possibility of using curcumin PDT as an alternative approach for the treatment of C.acnes , especially in instances of antibiotic-resistant variations and infections related to biofilms.

Synergistic effects of glycerol and lactose on propionic acid production by Cutibacterium acnes ZW-1: Insights from untargeted metabolomics

Zeyu Wang,Hao Xu,Minyu Suo,Yu Mei,Dzmitry Hrynsphan,Savitskaya Tatsiana,Christian Kennes,Jun Chen

Biodiesel production from dairy wastewater yields crude glycerol containing residual lactose, forming co-substrates suitable for microbial fermentation. This work converts these residues into propionic acid (PA) using Cutibacterium acnes ZW-1 and clarifies how a small lactose fraction reshapes metabolism. ZW-1 was cultivated anaerobically on a 90 % glycerol-10 % lactose co-substrate and on a 100 % glycerol control. The co-substrate system reached 4.36 g/L PA after 14 days, 1.20 g/L higher than the control, and doubled the maximum production rate from 0.39 to 0.80 g/L/d. Lactose addition increased biomass (OD 600 from 0.62 to 0.74), lowered the final pH (5.14–4.68) and raised the intracellular NADH/NAD⁺ ratio from 2.9 to 4.0. Activities of pyruvate carboxylase and lactate dehydrogenase increased by 1.8- and 2.1-fold, indicating strengthened anaplerotic and redox-balancing routes. Untargeted metabolomics identified 454 differential metabolites; galactose metabolism and the pentose phosphate pathway were markedly upregulated, while biotin derivatives, pyrimidine nucleotides, pyridine compounds and fatty acyls showed positive correlations with PA yield. Overall, a modest lactose supplement unlocks more efficient glycerol fermentation and provides a mechanistically supported route for upgrading dairy-related byproducts to value-added PA.

Essential oil of Saposhnikovia divaricata mitigates Cutibacterium acnes-induced inflammatory acne via Nrf2 pathway activation and NF-κB pathway inhibition

Song-Xue Yang,Wei-Lie Xiao,Xing-Jie Zhang

Ethnopharmacological relevance The root of Saposhnikovia divaricata has been traditionally used in Chinese medicine to treat inflammation and immune related skin diseases, such as skin allergy and itchy rash. However, scientific research supporting its efficacy, particularly in curing inflammation from acne, is still scarce. Aim of the study To evaluate the therapeutic effects of essential oil of Saposhnikovia divaricata (EOSD) on Cutibacterium acnes ( C. acnes )-induced acne and investigate its mechanisms. Materials and Methods An acute acne model was established by injecting C. acnes into mouse ears, followed by topical EOSD treatment (0.25 %, 0.5 % and 1 %). Ear tissues were analyzed 24 hours post-treatment. In vitro, C. acnes -stimulated J774A.1 macrophages were treated with EOSD (3, 10 and 30 μg/ml) to assess mitochondrial damage, oxidative stress, and inflammatory responses. Key markers were analyzed using electron microscopy, real-time PCR, immunoblotting, ELISA, and immunofluorescence. Nrf2 knockdown was achieved via siRNA transfection. Results C. acnes suppressed Nrf2 activation, increased ROS levels, and triggered NLRP3 inflammasome activation, leading to inflammation. EOSD restored Nrf2 activity, reduced ROS levels, and inhibited NLRP3 activation. In vitro, EOSD disrupted Nrf2/Keap1 interaction, enhanced Nrf2 nuclear translocation, reduced mitochondrial damage, and suppressed pyroptosis by downregulating IL-1β, Caspase-1, GSDMD, and NF-κB signaling. These effects were reversed by Nrf2 knockdown. Conclusion EOSD alleviates oxidative stress and modulates NLRP3 inflammasome activation through upregulating Nrf2 pathway and inhibiting NF-κB pathway, as well as improving mitochondrial homeostasis and inflammatory responses in acne. These findings suggest that EOSD may be a promising therapeutic agent for the treatment of acne vulgaris.
