The extracorporeal carbon dioxide removal (ECCO 2 R) system is a life-support technique that has been shown to facilitate a protective ventilation strategy. When the lung disease cannot remove carbon dioxide (CO 2 ) in time, resulting in CO 2 retention, ECCO 2 R is used to remove the excess CO 2 . As the key component of the artificial lung, the gas transport performance of the membrane determines the CO 2 removal capacity of the membrane lung, which should present high CO 2 selectivity to increase the efficient CO 2 removal rate. In this study, a biomimetic metalloenzyme, ZIF-8-NH 2 , was prepared to mimic the function of carbonic anhydrase (CA). The pore in ZIF-8-NH 2 was constructed to act as gas channel, in order to enhance the CO 2 /O 2 selectivity. ZIF-8-NH 2 /Pebax, which has functions similar to those of CA and gas channel (AQP1), was coated on the surface of poly(4-methyl-1-pentene) (PMP) membranes to fabricate the composite membrane. The results confirmed that the CO 2 /O 2 selectivity of the ZIF-8-NH 2 /Pebax composite membrane was 7.4, which was 7.5 times higher than that of the pristine PMP membrane (1.0). The protein adsorption and thrombin time were 35.5 % lower and 34.4 % higher than those of PMP hollow fiber membrane (44.9 μg/cm 2 and 27.6 s), respectively. These results demonstrated that the prepared composite membrane possessed good blood compatibility. Additionally, an in-vitro blood circulation device was established to comparatively evaluate the hemocompatibility and CO 2 removal efficiency of PMP and the composite membrane modules under dynamic blood flow. Meanwhile, the corresponding results exhibited that the modified coating has good biosafety, implying that the mentioned materials can be safely applied in clinical trials.

A metal–phenolic network coating was constructed on PMP hollow fiber membranes. • The Engineered interface forms a stable hydration layer that resits protein adsorption and platelet adhesion. • The Coating synergistically suppressed the thrombosis-inflammation cascade. • The modified membrane exhibited potent reactive oxygen species scavenging activity. • Stable CO 2 removal was maintained during prolonged blood circulation. Polymer hollow fiber membranes used in blood-contacting gas exchange applications often suffer from insufficient hemocompatibility due to their intrinsically hydrophobic and biologically inert surfaces, which can induce nonspecific biofouling, thrombosis, and inflammatory responses during long-term operation. Herein, we present a facile bioinspired metal–phenolic interfacial engineering strategy to construct a stable, multifunctional coating on poly(4-methyl-1-pentene) (PMP) hollow fiber membranes through one-step coordination between tannic acid (TA) and Fe 3+ ions. The resulting TA–Fe 3+ metal–phenolic network forms a highly hydrated and mechanically stable surface layer, providing effective resistance against nonspecific adsorption and maintaining interfacial integrity under dynamic flow conditions. The engineered interface significantly enhanced hemocompatibility by suppressing protein adsorption (reduced from 53.8 μg/cm 2 to 10.4 μg/cm 2 ), minimizing blood cell adhesion, prolonging activated partial thromboplastin time, and decreasing thrombus formation by approximately 75%. Meanwhile, the phenolic groups within the metal–phenolic network provide efficient reactive oxygen species (ROS)-scavenging capability, thereby attenuating macrophage-mediated inflammatory responses and reducing the secretion of pro-inflammatory cytokines. Dynamic circulation tests confirmed the excellent interfacial stability of the coating under continuous blood flow without compromising the intrinsic gas transport characteristics or CO 2 removal performance of the membranes. This work establishes a robust metal–phenolic interfacial engineering strategy for multifunctional modification of polymer hollow fiber membranes, offering new opportunities for developing durable blood-contacting membranes for extracorporeal carbon dioxide removal (ECCO 2 R) and other membrane-based extracorporeal circulation systems. Extracorporeal CO 2 removal (ECCO 2 R) is a type of extracorporeal life support (ECLS) technology. It aims to remove carbon dioxide (CO 2 ) from small flow of the blood and correct hypercapnia and related acidosis. The ECCO 2 R system removes CO 2 retained in the patient’s body through the gas-blood exchange membranes, which in use, blood flows on the outside of the membrane, and the inside of the membrane is for gas flow. Existing ECCO 2 R system generally improves the emission rate of CO 2 by the increase of the gas flow rate. However, there is a problem that O 2 is simultaneously taken out, which will reduce the blood O 2 concentration. Therefore, the gas-blood exchange membranes should not only increase the CO 2 removal rate by the improvement of CO 2 /O 2 selectivity, but also retain more O 2 in the blood and then maintain the oxygen saturation of the blood. In present work, a method was proposed to fabricate COF-5/Pebax mixed matrix membranes (MMMs) on the PMP surface for enhancing CO 2 /O 2 selectivity, which was increased from 0.96 to 5.57. The CO 2 removal ability of COF-5/Pebax MMM was tested by blood circulation, and it was found that the CO 2 removal effect was increased 10.9% than that of the pristine membranes. The O 2 concentration was maintained at a higher level, about 1.69 times that of the pristine membranes. In contrast to the pristine PMP membrane, blood compatibility of COF-5/Pebax MMMs was improved because protein adhesion decreased by 44.75%. The results of cytotoxicity test and rabbit pyrogen test exhibited that the prepared COF-5/Pebax MMM presented great biosafety. Overall, the proposed COF-5/Pebax MMM has good performance in ECCO 2 R system and attractive potential for medical application.
