Extracorporeal Membrane Oxygenation (ECMO) is the most important supporting therapy to rescue patients with respiratory distress syndrome (ARDS), for which the membrane oxygenator offers a place for CO 2 and O 2 exchange. Although Poly (4-methyl-1-pentene) (PMP) has been extensively used as ECMO material, the problems of protein adsorption fouling and the need for anticoagulation remain challenging for clinical applications. Additionally, the challenges associated with the functional modification of PMP material underscore the importance of finding a new material with excellent gas permeability and blood compatibility. On the other hand, Polymers of Intrinsic Microporosity (PIM) has been extensively studied in the field of gas separation due to their unique twisted helical structure and high free volume. However, its hydrophobic nature also raises concerns about its anti-fouling ability and hemocompatibility, which restricts the exploration of PIM in ECMO applications. Herein, we modify the PIM-1 membrane and study the hemocompatibility of PIM-1, carboxylated-PIM-1, and amidoxime-functionalized-PIM-1, and explore their gas permeability by molecular dynamics simulation and experimental verification. The modified PIM membranes exhibit remarkable comprehensive performance, including effective anti-protein adhesion and hemocompatibility, high oxygen permeability, and CO 2 removal rate. Blood oxygenation tests also reveal that the modified membranes prepared in this work fully meet practical requirements, exhibiting great application potential.

Extra-corporeal membrane oxygenation (ECMO) systems can perform the roles of the human heart and lungs to realize extra-corporeal oxygenation of blood. This system mainly depends on the gas-blood exchange membrane, the quality of which impacts the oxygenation performance. Currently, the most widely used gas-blood exchange membrane is made of poly(4-methyl-1-pentene) (PMP) hollow fibers. However, plasma leakage often occurs during clinical applications, which decreases the oxygenation performance and the service life and may endanger the patient's life in serious cases. In this work, Hyflon AD/PMP hollow fiber composite membranes were prepared by coating Hyflon AD on the surfaces of PMP hollow fibers to form ultra-thin, dense layers. Compared the plasma leakage time of the composite membrane with that of the pristine PMP membrane, the Hyflon AD60 layer showed great improvement in anti-leakage performance. The Hyflon AD60/PMP hollow fiber composite membrane possessed lower platelet adhesion and protein adhesion than that of the PMP membrane, indicating better blood compatibility of the Hyflon AD60 membrane. Cytotoxicity experiments were conducted to further confirm the biosafety of Hyflon AD60 as a blood contact medical membrane material. Gas permeance and oxygenation performance of the Hyflon AD60/PMP hollow fiber composite membrane were tested to ensure gas exchange efficiency during the gas separation process. Therefore, the optimized Hyflon AD60/PMP hollow fiber composite membrane has potential for clinical use.

ABSTRACT During the application of extracorporeal membrane oxygenation (ECMO), protein adsorption on the membrane not only leads to a decline in the efficiency of gas exchange but also triggers a series of adverse effects and even blood coagulation. Herein, we provide the hydrophilic and low-surface-energy strategy to prepare a fouling-resistance and fouling-release membrane for ECMO. Polyvinylimide (PEI) was grafted with tannic acid (TA) on the PES membrane to form a hydrophilic coating, on which fluorinated small molecules with low surface energy were subsequently grafted by ring-opening reaction to form low-surface-energy domains. The hydrophilic coating resists fouling adhesion by forming hydration layers, while the low surface energy of the hydrophobic domains facilitates the release of pollutants. Moreover, the introduction of the hydrophobic domains facilitates gas-liquid exchange in the ECMO-simulated environment. After PBS rinsing, BSA and BFG removal improved by 97.81% and 227.83%, respectively. Finally, in our ECMO-simulated gas-liquid circulation detection device, the modified membrane exhibits excellent gas exchange properties (The O 2 and CO 2 exchange rates reach 123.37 and 168.52 mL·min -1 ·m -2 , respectively) and is resistant to plasma leakage. This study provides a new surface modification anti-fouling strategy with simultaneous enhancement of hemocompatibility and gas exchange efficiency.

Extracorporeal membrane oxygenation (ECMO) system is a life-supporting technique that plays an important role in cardiopulmonary failure or heart transplantation. Blood passes through the oxygenator to expel carbon dioxide (CO 2 ) produced during the metabolic processes while supplementing the blood with oxygen (O 2 ) to maintain the patient's life. As the key part of the oxygenator, the membranes come into direct contact with the blood for gases exchange. Therefore, membranes should possess excellent hemocompatibility to inhibit thrombosis and hemolysis. In this work, an antithrombotic coating was constructed on the surface of poly-4-methyl-1-pentene (PMP) hollow fiber membranes based on the co-deposition of zwitterionic polymer and dopamine to reduce the protein adsorption and improve hemocompatibility of the pristine PMP membranes. The experimental results were confirmed that the protein adsorption and the clotting time of the modification coating on the surface of PMP hollow fiber membranes were 16.3 μg/cm 2 and 643 s, which were 70.58 % lower and 36.8 % higher than that of the commercial PMP hollow fiber membranes (55.4 μg/cm 2 and 470 s), respectively. Test devices of in vitro blood circulation and in vivo circulation in animals were established to evaluate blood compatibility and the blood-gas exchange performance of as-prepared membrane modules, respectively, the evaluation conditions of which are similar to those of the real ECMO system. Meanwhile, the corresponding results exhibited that the poly (sulfobetaine methacrylate) (PSBMA) modified coating had good oxygenation performance and biosafety, implying that the zwitterionic polymer could provide excellent hemocompatibility for gas-blood exchange membranes in ECMO systems.

Commercial poly(4-methyl-1-pentene) (PMP) membranes for extracorporeal membrane oxygenation (ECMO) are fabricated via the thermally induced phase separation (TIPS) method, which requires high-temperature conditions and involves complicated processes. Moreover, the gas exchange capacity of current PMP membranes remains relatively low. Herein, mixed matrix membranes (MMMs) incorporating zeolitic imidazolate framework-8 (ZIF-8) nanoparticles (PMP/ZIF-8 MMMs) were developed to enhance the gas permeability in ECMO oxygenators. The MMMs exhibited good hemocompatibility with significantly enhanced gas permeability (P O2 =165 Barrer, P CO2 =450 Barrer) compared to pure PMP membrane (P O2 =32 Barrer, P CO2 =98 Barrer). And defect-free PAN/MMMs composite membranes with high ZIF-8 loading (40 wt%) were successfully constructed for ECMO oxygenators. They exhibited higher gas exchange capacity while maintaining stable CO 2 /O 2 selectivity. ECMO-simulated circulation tests showed that the PAN/MMMs had high blood oxygenation efficiency and ZIF-8 was stable in blood. This work demonstrates that the composite membranes possess great application potential in the future.

In clinical extracorporeal membrane oxygenation (ECMO), prolonged blood residence time in oxygenators can lead to thrombus formation and complications. Heparin-based bioactive coatings are commonly used for anticoagulation, but prolonged shear stress during blood flow can cause heparin desorption, degradation, and inactivation, reducing durability and antithrombotic efficacy. In this study, we developed a silk fibroin (SF)-based multilayer coating to enhance both mechanical stability and anticoagulant performance. A SF foundation layer was first fabricated on hollow-fiber membranes using layer-by-layer (LbL) self-assembly. Then SF acted as a bridge to assemble heparin and albumin through electrostatic interactions, followed by crosslinking to stabilize the structure, resulting in a uniform SF-heparin-albumin composite coating with controllable thickness. 10-day shear oscillation and nanoscratch tests showed that the SF bridging layer significantly improved mechanical robustness and interfacial adhesion. In vitro and in vivo blood circulation assays demonstrated hemocompatibility, with reduced platelet adhesion, prolonged coagulation time, and enhanced antithrombotic performance. Blood-gas exchange tests using a miniaturized oxygenator confirmed that the coating did not impair gas transfer efficiency. In summary, the SF bridging layer effectively links the substrate to bioactive components, improving long-term mechanical integrity and anticoagulant properties without compromising oxygenator function. Statement of significance During clinical ECMO use, shear stress often compromises the integrity of anticoagulant coatings, leading to detachment and reduced functionality. Here, we present a silk fibroin (SF)-based multilayer coating engineered to enhance the long-term stability and anticoagulant performance of ECMO hollow-fiber membranes. Utilizing a layer-by-layer self-assembly strategy, heparin and albumin were stably incorporated into the SF matrix, resulting in a coating with improved shear resistance and preserved gas exchange. As the first silk-based coating specifically designed for ECMO applications, this work bridges biomolecular design with device-level functionality. Moreover, it demonstrates biocompatibility with no detectable cytotoxicity, offering a promising approach for safe and durable blood-contacting applications.

Extracorporeal membrane oxygenation (ECMO) is a life-saving technology for patients with severe cardiopulmonary failure. However, bleeding complications remain a major clinical challenge, largely due to high shear stress-induced Von Willebrand factor (VWF) dysfunction. Existing methods often lack timeliness and sensitivity for VWF detection in early bleeding risk assessment. Here, we proposed an electrochemical biosensor that combined Two-Step Synthesized graphene oxide-gold nanoparticles (rGO@AuNPs) nanocomposites modified screen-printed electrodes and a miniaturized wireless communication circuit for point-of-care testing (POCT) of VWF levels. The introduction of rGO@AuNPs significantly enhanced the sensing performance, achieving a low limit of detection of 0.39 pg/mL for VWF within 15 min. Moreover, there are Pearson correlation coefficients of 0.926 and 0.974 between the VWF biosensor and ELISA for porcine models and clinical samples. Notably, VWF degradation was associated with high shear stress, and VWF depletion was observed at the same time of APTT prolongation, suggesting its utility as a biomarker for bleeding risk. Its rapid, portable, and cost-effective design supports POCT, offering a promising tool for early monitoring and intervention in ECMO-related bleeding.

Membrane oxygenator performs lung function in the Extra-corporeal Membrane Oxygenation (ECMO) system. In the clinical use, to ensure the balance of gas exchange of patient’s body, the gas-blood exchange membrane is required to possess CO 2 /O 2 selectivity, which is suggested to be in the range of 10-12. In our work, the molecular dynamics simulation results displayed that the amino-functionalized Zr-MOFs provides fast transport channels for CO 2 molecules because of the enhancement of the adsorption and diffusion ability for CO 2 . Therefore, we chose organic ligands with abundant CO 2 -philic groups to synthesize target amino-functionalized Zr-MOFs, and then dispersed them into medical-grade PDMS solution to study the separation performance of CO 2 /O 2 . On the basis of constructing anti-leakage coating layer, biomimetic gas channels were designed to increase the clearance rate of CO 2 . Experimental results exhibited that the CO 2 /O 2 selectivity of (NH 2 ) 2 -UiO-66/PDMS MMM was as high as 11.19, which was basically consistent with the simulation results. The obtained amino-functionalized Zr-MOF/PDMS coating layer displayed favorable biocompatibility and biosafety. Meanwhile, the construction of biomimetic channel solves the difficult problem of CO 2 removal in PMP membrane, which verifies the feasibility of applying porous materials to gas-blood exchange membranes in the ECMO system.

Summary Extracorporeal membrane oxygenation (ECMO) is a life support system that provides extracorporeal respiration and circulation. Current oxygenation membranes still face the challenges of plasma leakage, low gas exchange efficiency of O 2 and CO 2 , and poor biocompatibility. Here, we describe the design and realization of a biomimetic Janus membrane to simulate the structural characteristics of the alveolar blood-gas barrier. The asymmetric structure is constructed by sequential assembly of a hydrophobic polydimethylsiloxane layer, hydrophilic amino acid layer, and sulfonate grafting layer, which provide the functions of preventing plasma leakage, improving gas exchange performance, and maintaining biocompatibility. Importantly, the membrane can offer efficient blood oxygenation, achieving O 2 and CO 2 transfer rates of ∼54 and ∼131 mL m −2 min −1 , respectively. This work demonstrates that the smartly designed Janus structure provides the desired comprehensive properties of an oxygenation membrane and offers valuable guidance for the development of more advanced oxygenation membranes.

Extracorporeal membrane oxygenation (ECMO) is one of the most important supporting therapies for patients with severe cardiopulmonary failure, and the core component of ECMO is the membrane oxygenator . However, in clinical application, the membrane oxygenator faces a dilemma in balancing anticoagulation and hemorrhage risks, which restrains the service life of membrane oxygenator and affects patients’ safety seriously. In this work, we introduce poly (1-vinyl-2-pyrrolidone) (PVP) and poly (acrylic acid) (PAA) into polyethersulfone (PES) membranes simply by in-situ crosslinking polymerization and nonsolvent induced phase separation (NIPS) method. All the modified membranes exhibit favorable hemocompatibility with prolonged activated partial thromboplastin time (APTT) (>10 s). Besides, the introduction of PAA can improve CO 2 and O 2 permeabilities simultaneously (increased by 36.57% and 30.86% at maximum, respectively, compared with pristine PES membrane). Finally, an ECMO-simulated gas-liquid contactor circulation device is designed, and the modified membranes show better oxygenation performance and CO 2 removal performance. The membrane modification and fabrication methods in this work are simple, low-cost, and easily industrialized. The work has practical guiding significance for the subsequent membrane research and application toward oxygenators.

Poly (4-methyl-1-pentene) (PMP) is widely used between blood and gas in extracorporeal membrane oxygenation (ECMO). However, it is more suitable to miniaturize the oxygenator from an economic and biosafety standpoint, that necessitates the PMP oxygenation membrane with enhanced gas permeance and hemocompatibility. This study introduced fluorinated covalent organic framework (FCOF) nanofiber into PMP matrix to construct PMP-FCOF hybrid flat sheet membranes and hollow fiber membranes (HFMs) via thermally induced phase separation (TIPS). FCOF exhibits structural and chemical similarity with PMP, ensuring the good compatibility with PMP matrix. The abundant C–F bonds rendered membrane low-surface-energy of ∼30.2 mN m −1 for PMP–FCOF–3 flat sheet membrane, exhibiting better hemocompatibility compared with that of ∼34.4 mN m −1 for the PMP membrane. The fabricated PMP–FCOF–3 HFMs for blood gas exchange with O 2 exchange rate of ∼309.8 mL min −1 m −2 , which was 77.3 % higher than the commercial PMP HFM, and comparable CO 2 exchange rate of ∼251.7 mL min −1 m −2 with commercial PMP HFM. The blood-gas exchange and hemocompatibility were synergistically enhanced by a simple hybrid strategy, exhibiting great application potentials.

Extracorporeal membrane oxygenation is a technique that provides short-term supports to the heart and lungs. It removes CO 2 from the blood and provides enough oxygen, which is a huge help in the fight against COVID-19. As the key component, the artificial lung membranes have evolved in three generations including silicon, polypropylene and poly (4-methyl-1-pentene). Herein, we for the first time design and fabricate a novel poly (4-methyl-1-pentene)/polypropylene (PMP/PP) thin film composite (TFC) membrane with the anticoagulant coating composed of poly (sodium 4-styrenesulfonate) and cross-linked poly (vinyl alcohol). Poly (sodium 4-styrenesulfonate) provides sulfonic acid groups to inhibit the coagulant factors (FVIII and FXII), and cross-linked poly (vinyl alcohol) increase the stability of the anticoagulant coating and further improve the hydrophilicity via abundant hydroxyl groups to depress the protein adsorption. Long-term anticoagulant property was demonstrated by whole human blood for 28 days. Blood compatibility was evaluated by hemolysis rate, anticoagulation activity (APTT, TT and PT), complement activation, platelet activation and contact activation. Pure CO 2 , O 2 and N 2 permeation rates were determined to evaluate the mass transfer properties of PMP/PP TFC membranes. Gas permeation results revealed that gas permeation flux increased in the TFC membranes because of the decrease of crystallinity. Overall, the so prepared PMP/PP membrane shows good CO 2 /O 2 selectivity and blood compatibility as novel TFC artificial lung membrane.
