Unilateral thermosensitive Janus hemostatic dressing for matching coagulation physiological processes

Chuanjuan Chen,Yicheng Zhang,Siqi Chen,Qinhui Chen

Hemostatic dressings are a simple and effective method for managing bleeding emergencies. Based on the blood coagulation process, hemostatic dressings work optimally by aggregating hemocytes, releasing coagulation factors, and sealing blood in a sequential manner. Janus materials, which have different components compartmentalized, have the ability to independently perform multiple functions. In this study, a Janus nonwoven fabric (BG-PNIPAM) with thermo-sensitivity on one side, designed to consistent with the physiological process of coagulation, was created. When utilized for hemostasis, the poly(N-isopropylacrylamide) (PNIPAM) side of Janus BG-PNIPAM undergoes a phase transition within 1 minute. Prior to this, it adsorbs blood in injured site and aggregate hemocytes to form primary thrombus; Afterwards, it plays a sealing role to decrease blood loss. During this process, the bioactive glass (BG) side continuously releases SiO44- and Ca2+ to activate coagulation factors and accelerate the cascade reaction. The phase transition and hydrophilic/hydrophobic transition are triggered by body temperature and rapidly completed just like a click on the switch. Animal studies have demonstrated that Janus BG-PNIPAM shows superior hemostatic performance compared to nonwoven fabric and commercial QuikClot® gauze. Furthermore, its biosafety profile provides assurance for potential clinical applications in the future.

Glucose-Activated Janus Wound Dressing for Enhanced Management of Infected and Exudative Diabetic Wounds

Liqin Tang,Yingjie Zhang,Huiru Zhang,Xiao Yang,Jun Wang,Jifu Mao,Lu Wang,Yan Li

Diabetic wounds, often multifactorial and affecting multiple organs, pose substantial challenges to patient well-being, drawing significant interest in biomedical engineering. The demanding wound microenvironment, marked by heightened glucose levels, local exudate, and bacterial infections, emphasizes the pressing demand for advanced wound dressings to meet escalating clinical needs. Herein, a Janus wound dressing with an integration of an antimicrobial hydrophobic nanofiber layer and a 3D hydrophilic sponge was designed and prepared to manage and utilize wound exudate. The hydrophobic layer skillfully combined electrospun poly(ε-caprolactone) (PCL) nanofiber membranes (ENMs) and metal–organic frameworks (MOFs) with peroxidase-like properties by solvent etching, and glucose oxidase (GOx) was grafted through ligand interaction. GOx acts to consume glucose while modulating pH, thus suitable pH and self-supplied H2O2 were able to activate the catalytic activity of MOFs to generate •OH. Additionally, hydrophilic 3D sponges are constructed using gas foaming technology, which are tactfully combined with hydrophobic ENMs to form a Janus structure, which can transport exudate through the antimicrobial layer to the sponge layer, while sufficient glucose contact with GOx enhances the antimicrobial properties of the designed Janus wound dressing. Experimental results demonstrate the effectiveness of the cascade effect of GOx@PCL/MOF ENMs, ultimately releasing reactive oxygen species and exhibiting robust antibacterial properties. In vivo animal experiments reveal the ability of the Janus wound dressing to mitigate methicillin-resistant Staphylococcus aureus (MRSA) infections in the early stages, thereby expediting the wound healing process. In vivo animal study, the Janus wound dressing achieved a healing rate of 54% on day 3. Our findings underscore the substantial potential of the Janus wound dressings in promoting the healing of chronic diabetic wounds.

A pH/glucose dual-responsive Janus dressing with optimized gradient modulus/tissue adhesion promotes diabetic wound healing by dynamic drug-payload release

Huabiao Ye,Ting Dong,Ruoxuan Li,Lihua Fang,Kaifang Xie,Guangting Han,Que Chen,Shan Chi,Jia-Horng Lin,Ching-Wen Lou

A dynamic drug-payload release dressing (H@GmJ-P) was logically engineered for promoting diabetic wound healing using two purpose-specific units: a pH/glucose dual-responsive hydrogel and a Janus membrane with centrally graded-thickness. ‌The underlying hydrogels were created by a sequential functionalization strategy with recipes of dopamine-grafted oxidized sodium alginate, methacrylate-phenylboronic acid functionalized carboxymethyl chitosan, tannic acid and borax, which functionalized the H@GmJ-P with excellent bio-adhesive (∼74.5 kPa), antibacterial (>98 %), antioxidant, hemostatic, and self-healing properties. The upper Janus membranes composed of polycaprolactone (PCL) hydrophobic layer and PCL/F127 hydrophilic layer were engineered by thickness-varied design, functionalizing the H@GmJ-P with gradient decreasing modulus from ∼1.5 MPa (middle) to ∼0.6 MPa (periphery) for superior wound tear-resistance (∼197 kPa). Functional collaboration between the two units established unidirectional drug delivery platform following drug-sustained release behavior by pathophysiological wound pH/glucose stimuli. In vivo chronic diabetes skin wound model confirmed that H@GmJ-P promoted wound closure with only 8.2 ± 2.2 % unhealed area by day 21. This work proposes rational design of soft hydrogel dressing with stiff membrane can create matched modulus/tissue adhesion against wound tear. In addition, an innovative regenerative strategy emphasizing precise drug-sustained release offers a novel paradigm for managing challenging wounds.
