Initiation of endogenous repair mechanisms, including key steps of stem cell recruitment and cartilage intermediate formation in endochondral ossification , is vital to regeneration of large bone defects. To biomimetically promote a rapid initiation and ensuing osteogenic stimulation, exogenous chemokine IL-8 and growth factor BMP-2 were orchestrated in a mesoporous bioactive glass (MBG)-based spatiotemporal delivery system, to achieve a rapid release of IL-8 followed by a long-term sustained release of BMP-2. The synergistic effect of IL-8 and BMP-2 on initiation stage of bone healing and underlying mechanism were thoroughly investigated in vitro and in vivo . Intriguingly, apart from its superiority in stem cell recruitment to BMP-2, IL-8 not only endowed a histological “prep-state” of endochondral ossification by up-regulating chondrogenic genes and inducing the formation of extensive cartilage tissues, facilitating rapid bone transformation by BMP-2, but also triggered a cellular “prep-state” with high expression of BMP receptors, enhancing the osteoinductivity of BMP-2. With the spatiotemporal delivery system, orchestrated signal stimuli of IL-8 and BMP-2 induced a rapid initiation including efficient stem cell recruitment and a “chondrogenic/osteogenic balance” at the first stage of endochondral ossification, and the scaffold facilitated sufficient osteoconductivity , together resulting in early extensive bone mineralization and an advanced regeneration throughout the repair of large bone defect. We believe this new idea could provide insights toward designing bone-repairing biomaterials with higher regenerative efficiency.

With its excellent bioactivity and mesoporosity that endows controlled release of chemical or biological cues, mesoporous bioactive glass (MBG) is widely used in bone regeneration, yet less recognized in dentistry. This study investigates the enamel remineralization effect of MBG in commercial fluoride varnishes (FVs), and developes an MBG/fluoride chitosan composite hydrogel. MBG powders are added into commercial FVs (3M™ Clinpro™, Duraphat™) at different ratios to screen out the optimized proportion for mineralization. Further, an MBG/fluoride chitosan composite hydrogel (MBG/NaF/CS) has been developed at the optimized ratio, and its in vitro remineralization effect on tooth enamel after artificial saliva soaking for 1/3/5 days are evaluated via SEM, EDS and Vickers hardness test. Incorporation of 5 wt% MBG in FVs results in the most rapid and extensive hydroxyapatite (HAp) deposition within 24 h. MBG/NaF/CS hydrogel exhibits homogeneity instead of fluoride precipitation in NaF/CS hydrogel. The addition of MBG contributes to rapid, larger, and continuous fluorapatite (FAp) remineralization on enamel surface over time, and restores the hardness of enamel over normal level after 24 h. It is indicated that F - adsorption in MBG mesopores endowed the MBG/NaF/CS hydrogel an unreported continuous enamel remineralization effect up to 5 days under daily brushing. MBG enhances enamel remineralizing capacity of commercial FVs, and the novel MBG/fluoride chitosan composite hydrogel with continuous enamel remineralization effect represented a promising approach for caries management. This study may shed light on further application of MBG in dentistry.

Osteoporotic bone regeneration poses significant challenges due to the complexity of the condition. Osteoporosis, a degenerative disorder, results from an imbalance in bone homeostasis driven by dysregulation of osteoblast and osteoclast activity. This complicates the treatment of osteoporosis and its related bone injuries in clinical practice. Despite the development of various polymer scaffolds for bone defect repair, achieving effective regeneration in osteoporotic bones—especially when combined with osteoporosis medications—remains difficult. In this study, we designed a drug delivery system composed of mesoporous bioactive glass (MBG) and photo-crosslinked hyaluronic acid methacrylate (HAMA). This system, loaded with the osteogenesis-promoting peptide DWIVA (D5) and the osteoclastogenesis-inhibiting drug alendronate (ALN), is gelled using a light initiator and 405 nm wavelength light. The MBG@D5-Gel complex enables the controlled spatiotemporal release of these agents, markedly enhancing bone regeneration in osteoporotic conditions within ovariectomized rats by inhibiting osteoclastogenesis and bone resorption while promoting osteogenic differentiation and mineralization. This dual-action system synergistically regulates osteoblast and osteoclast activity, optimizing the pathological microenvironment of osteoporosis and facilitating the repair of osteoporotic bone defects. MBG@D5-Gel holds great potential as an effective organic-inorganic hybrid biomimetic implant material for the treatment of osteoporotic bone defects.

In the process of biomaterials mediated bone regeneration, rhBMP-2 delivery at efficient dose in sustained kinetics is crucial for promoting cell osteogenic differentiation. Meanwhile, surface morphology of the biomaterials could regulate cellular responses as well as strengthen the rhBMP-2 interaction with cells for better bone induction. Herein, TiO 2 nanorod films with varied mesoporous bioactive glass (MBG) incorporation amount were designed to strengthen the efficacy of rhBMP-2, basing on optimized loading/release behaviors and surface nanostructure cooperatively. The MBG incorporation improved rhBMP-2 loading amount and regulated its release behavior. Consequently, the osteogenic differentiation of bone marrow-derived mesenchymal stem cells (BMSCs) on the incorporated films was extremely enhanced, and the incorporated nanorod film with 200 nm MBG thickness exhibited the best osteoinduction effect. However, MBG film and the incorporated nanorod film had the same loading amount of rhBMP-2, the latter showed a much higher expression of 7-day osteogenic differentiation index than the former, which could be attributed to the synergistic effect of optimized rhBMP-2 release behavior and surface morphology. The MBG incorporated TiO 2 nanorod films here presents a promising strategy for enhancing osteoinduction through optimized rhBMP-2 release behavior.

It remains a challenge to achieve satisfactory balance between biodegradability and osteogenic capacity in biosynthetic bone grafts. In this study, we aimed to address this challenge by incorporating mesoporous bioactive glass (MBG) into poly(caprolactone-co-glycolide) (PGA-PCL) at gradient ratios. MBG/PGA-PCL (PGC/M) scaffolds with MBG incorporation ratio at 0, 10%, 25% and 40% (PGC/M0-40) were synthesized using a modified solvent casting-particulate leaching method, and their physiochemical and biological properties were comprehensively evaluated. PGC/M scaffolds exhibited highly perforated porous structure with a large-pore size of 300–450 μm, with ordered MBGs of around 6.0 nm mesopores size uniformly dispersed. The increase in MBG incorporation ratio significantly improved the scaffold surface hydrophilicity, apatite-formation ability and pH stability, increased the weight loss rate while insignificantly influenced the molecular chains degradation of PGA-PCL component, and facilitated the attachment, spreading, viability and proliferation of rat bone marrow stromal cells (rBMSCs) on scaffolds. Moreover, rBMSCs cultured on PGC/M10-40 scaffolds demonstrated enhanced ALP activity and osteogenesis-related gene expression in a MBG dose-dependent manner as compared with those cultured on PGC/M0 scaffolds. When implanted to the rat cranial bone defect, PGC/M25 and PGC/M40 scaffolds induced significantly better bone repair as compared to PGC/M0 and PGC/M10 scaffolds. Besides, the biodegradability of PGC/M scaffolds correlated with the MBG incorporation ratio. These data suggested this novel PGC/M scaffolds as promising bone repair biomaterial with highly tunable hydrophilicity, bioactivity, cytocompatibility, osteogenic activity as well as biodegradability.

With the worldwide rising need of severe bone defect treatment, the development of available bone substitutes with optimal mechanical strength, sustained drug release, cell affinity and osteoinductivity remains a great challenge. In this study, an rhBMP-2 loaded polymer-coated mesoporous bioactive glass (MBG) composite scaffold was developed. The uncrosslinked poly(glycerol sebacate) (PGS) or PEGylated PGS (PEGS) coating modification had enhanced the mechanical strength of the composite scaffold, solved the brittleness problem of the MBG matrix, increased the cell affinity of the material surface, and diminished the initial burst release of rhBMP-2 from mesopores of MBG. The results indicated that the PGS coating promoted the proliferation of rat bone marrow stem cells (rBMSCs), while the PEGS coating exhibited an enhancement in the osteogenic differentiation of rBMSCs. The in vivo ectopic bone formation results provide strong evidence that the rhBMP-2-loaded MBG/PEGS composite scaffolds exhibited a rapid bone forming capacity and might yield extraordinary achievements in the field of bone tissue engineering. The design considerations can be extended to other artificial scaffolds and are expected to provide new thoughts on the development of future tissue engineering materials.

Osteoarthritis (OA) involves synergistic pathological changes in both cartilage degradation and subchondral bone remodeling due to oxidative stress, yet current therapies typically target these processes separately. To address this limitation, we developed a dual-functional nanogel (MBG@Pae@HA) by conjugating hyaluronic acid (HA) and paeoniflorin (Pae) to mesoporous bioactive glass nanoparticles via amide bonding and non-covalent interactions, respectively. Leveraging the inherent hydrophilicity of HA, MBG@Pae@HA spontaneously forms nanogels and shows enhanced adhesion force which ensure extended Pae and bioactive ions release in the OA microenvironment. MBG@Pae@HA exhibits potent reactive oxygen species (ROS)-scavenging capacity, contributing to the homeostatic balance between cartilage matrix anabolism and catabolism. Transcriptomic analysis revealed that MBG@Pae@HA treatment reactivates chondrocyte function through activating the cAMP/PKA/CREB antioxidant pathway. Furthermore, with controlled release of Pae and bioactive components (including Ca ions, Si ions, and PO43-) it significantly promotes osteogenesis of bone stem cells. Medial meniscotibial ligament (DMM) model demonstrated the dual therapeutic efficacy of MBG@Pae@HA nanogel with histological and micro-CT analyses confirming concurrent protection against cartilage degradation and enhancement of subchondral bone regeneration. By simultaneously addressing both major pathological features of OA through microenvironment remodeling and prolonged drug delivery, this HA-functionalized and Pae-loaded nanogel represents a significant advance in OA treatment strategies, offering a comprehensive approach to halt disease progression and promote joint repair.

Chronic diabetic wounds require dressings that combine high liquid absorption, antibacterial activity, and bioactivity to support tissue repair. Here, copper-containing mesoporous bioactive glass (Cu-MBG) was incorporated into polyurethane foam to create a bioactive composite dressing (PU-Cu-MBG). The resulting foam exhibited strong liquid absorption, suitable water vapor transmission, and robust mechanical properties. The Cu-MBG endowed the dressing with antibacterial activity (>99% against Staphylococcus aureus and Escherichia coli ) and promoted the proliferation of human endothelial cells. In vivo , the PU-Cu-MBG dressing enhanced angiogenesis, collagen deposition, and accelerated wound closure compared with commercial dressings in a diabetic skin wound model. These findings demonstrate that bioactive glass-loaded foam dressings can effectively combine physical and biological functions, offering a promising approach for the management of chronic diabetic wounds.

Corneal stromal injury is a common surgical disease. With the development of tissue engineering materials, many artificial corneal scaffolds have been developed to replace allograft corneal transplantation and solve the problem of corneal donor shortage. However, few researchers have paid attention to corneal stromal wound healing. Herein, a nanocomposite of amino modified mesoporous bioactive glass (MBG-NH2) and microRNA-133b (miR-133b) was introduced into the patterned collagen films to achieve corneal stromal injury repair. MBG-NH2 nanoparticles as a nano delivery carrier could efficiently load miR-133b and achieve the slow release of miR-133b. The physicochemical properties of collagen films were characterized and found the microgrooved collagen films loaded with miR-133b@MBG-NH2 nanoparticles possessed similar swelling properties, optical clarity, and biodegradability to the natural cornea. In vitro cell experiments were also conducted and proved that the patterned collagen films with miR-133b@MBG-NH2 possessed good biocompatibility, and miR-133b@MBG-NH2 nanoparticles could be significantly uptake by rabbit corneal stromal cells (RCSCs) and have a significant impact on the orientation, proliferation, migration, and gene expression of RCSCs. More importantly, the patterned collagen films with miR-133b@MBG-NH2 could effectively promote the migration of RCSCs and accelerate wound healing process, and down-regulate the expression levels of α-SMA, COL-I, and CTGF genes associated with myofibroblast differentiation of corneal stromal cells, which has a potential application prospect in the repair of corneal stromal injury.

Peripheral nerve injury frequently occurs alongside vascular injury due to the incorporation of vascular tissue within the nerve bundle. During the repair process of peripheral nerve injuries, the concurrent regeneration of capillaries within the nerve bundles is crucial for facilitating nerve regeneration. However, current studies often overlook the regeneration of blood vessels in nerve bundles when designing nerve repair scaffolds, leading to suboptimal nerve regeneration outcomes. To address this limitation, amino-modified Cu-doped mesoporous bioactive glass (Cu-MBG) was developed as a nanodelivery carrier for miR-29a to construct miR-29a@Cu-MBG nanoparticles, which exhibit dual functionality in promoting nerve regeneration and inducing vascular regeneration. Subsequently, miR-29a@Cu-MBG was incorporated into a CS/GelMA hydrogel to create an engineered multifunctional nerve conduit through the ionic cross-linking of chitosan and the optical cross-linking of GelMA, which demonstrated excellent mechanical properties and biocompatibility. In vitro cell experiments indicated that the sustained release of miR-29a@Cu-MBG from the conduit effectively delivered miR-29a into PC12 cells, leading to the upregulation of nerve regeneration-associated genes (GAP-43 and NF-200) and the outgrowth of neurite. Additionally, Cu-MBG was found to enhance the migration of HUVECs and upregulate genes related to angiogenesis (HIF-α, ANG, eNOS, and KDR), promoting the tube formation of HUVECs. These findings offer a perspective for the design of nerve conduits capable of promoting both nerve and vascular regeneration.

Skull defect repair is a complex and critical medical challenge, and there is an urgent need to develop multifunctional tissue engineering scaffolds for skull regeneration. The success of bone tissue engineering depends on the construction of scaffolds that can regulate the immune microenvironment of bone regeneration and mimic the liquid crystal and viscoelastic properties of natural bone extracellular matrix. Hence, a smart hydrogel (PEGDA5/AM15/CLC-BMP-4@MBG) with good biocompatibility and the ability to modulate the wound immune microenvironment has been developed for the repair of skull defects. The hydrogel consists of chitin liquid crystal hydrogel (PEGDA5/AM15/CLC) and mesoporous bioactive glasses (MBGs) loaded with bone morphogenetic protein-4 (BMP-4). The liquid crystal hydrogel not only offers the necessary biological support and mechanical properties but also maintains the stability of the liquid crystal state, facilitating adhesion and regeneration of surrounding bone tissue. In addition, BMP-4@MBG intelligently regulates the release rate of BMP-4 in response to changes in wound microenvironment, thus effectively promoting the transformation of macrophages from M1 to M2 macrophages. At the same time, Ca2+ and Si4+ released by MBG degradation and BMP-4 synergically promote bone repair process. The PEGDA5/AM15/CLC-BMP-4@MBG hydrogel shows excellent immunomodulatory and osteogenic properties of bone microenvironment and is a promising scaffold material for bone tissue engineering.

Mesoporous bioactive glass (MBG) is a potential biomedical material in bone defect repairment because of its bioactivity, biocompatibility, and osteoinduction properties. Here we report that Mg-doped MBG scaffold with 3:1 Ca/Mg ratio (MBG-Ca/Mg-3) is good for MC3T3-E1 osteoblast differentiation and mineralization. Mimicking bone extracellular matrix structure by electrospinning, we used MBG-Ca/Mg-3 adsorbed with Osteocalcin-Osteopontin-Biglycan (OOB), a new unique matrix fusion protein, to form [email protected] /Mg-3 scaffold, which has multifunctional ability in calvarial bone defect repairment in vivo . Intriguingly, we found that [email protected] /Mg-3 scaffold increases the expression of osteoblastic marker genes, including bone morphogenetic protein ( Bmp2 ), osteopontin ( Opn ), Osterix , Runx2 through activation of ERK1/2. We concluded that [email protected] /Mg-3 scaffold promotes osteoblast differentiation and mineralization through ERK1/2 pathway and it can also enhance bone formation in vivo , which provides a new biomaterial in bone tissue engineering.
