ABSTRACT The digestive behavior and absorption properties of lipids are affected by differences in the interface structures of fat globules in human milk (HM) and infant formula. However, it is not clear how the interface changes of fat globules further regulate lipid uptake by improving lipid digestion. Based on the milk fat globule membrane (MFGM) and milk protein, two infant formula emulsions with different interface structures (F1: MFGM-stabilized emulsion, F2: whey protein and casein-stabilized emulsion) were prepared to simulate HM fat globules. The interface structure of lipid droplets, digestion behavior and absorption characteristic of lipids in infant formula emulsions were evaluated and compared with those of HM and commercial infant formula (IFM: supplied with MFGM). The results showed that MFGM-stabilized fat globules in F1 had a similar interface structure to HM. F1 could alleviate the coalescence and aggregation of lipid and protein in comparison with F2 and IFM. The order of triglyceride secretion from high to low was: HM (51.63 ± 2.0 μmol/L), F1 (45.19 ± 1.7 μmol/L), F2 (35.28 ± 1.3 μmol/L) and IFM (32.23 ± 1.3 μmol/L), indicating that HM and F1 exhibited faster lipid digestion and more chylomicron secretion. The expression of SLC27A4, FABP2, SCARB1, DGAT2, PLIN2, and MOGAT2 in HM and F1 was significantly higher than in F2 and IFM. Altogether, the simulation of HM fat globule at the interface structure based on MFGM could better promote cellular lipid uptake by influencing the digestion behavior of fat globules.

In this study, we analyzed and compared the fat globule interfacial compositions and structures in human milk and three types of infant formulas (IF1: supplied with MFGM , IF2: without MFGM and soy lecithin , IF3: supplied with soy lecithin). The results suggested that the interfacial protein composition of IF1 was comparatively closer to human milk, but still lacked certain bioactive MFGM proteins including XO, ADPH and PAS6/7. Considering the interfacial phospholipid , we observed 23, 31, and 29 phospholipid species that could be used to distinguish human milk and infant formulas (IF1, IF2 and IF3). We also found that phosphatidylinositol (PI) and phosphatidylserine (PS), which can absorb lipase, phosphatidylethanolamine (PE), and phosphatidylcholine (PC) and connect functional fatty acids, were lacking in infant formulas. Moreover, the infant formulas had a smaller average particle size of 0.38 μm and a thicker interfacial layer that interacted with the casein micelles. Even if IF1 included MFGM, fat globules structures like those found in human milk did not form, and the majority of the MFGM was still in the aqueous phase in free form. Overall, the comprehensive analysis in this study could be used to simulate or mimic human milk lipids at the supramolecular level.

Current infant formulas lack the native multilayer structure of breast milk fat globule membrane (MFGM), impacting lipid digestion. In this study, the inner layer material and concentration of the biomimetic fat globule membrane were optimized by comparing particle size, Zeta-potential and interface protein load. It was found that compared with sodium caseinate (CN) and whey protein (WP), when the lactoferrin (LF) concentration was 2 %, the particle size was lower (277.85 ± 6.15 nm) and Zeta-potential value was higher (19.67 ± 1.27 mv). Using milk phospholipid (MPL) as the outer layer material, when the MPL concentration was 2 %, the emulsion had a smaller particle size (291.33 ± 1.15 nm) and a better stability (10.22 ± 0.62 %). Therefore, the biomimetic multilayer membrane was constructed by electrostatic layer-by-layer deposition of 2 % LF and 2 % MPL. Combining Fluorescence and Fourier transform infrared spectroscopy (FTIR), the interaction between LF and MPL molecules in the LF-MPL multilayer structure is primarily a spontaneous, endothermic process driven by hydrophobic forces, exhibited superior stability (except thermal stability) than LF monolayer membrane. The results of in vitro digestion showed that compared with LF, WP and WP-MPL emulsions, LF-MPL emulsions had the highest free fatty acid (FFA) release rate of 69.97 %. LF-MPL enhanced gastric stability and promoted intestinal lipolysis and improved the degree of lipid digestion. In addition, LF-MPL promoted the absorption and utilization of triglyceride (TAG) in cells and animals, and secretion and upregulated lipid absorption genes (FATP4, DGAT1, APOB, APOA4, MTTP). These findings demonstrate that biomimetic LF-MPL multilayers improve lipid digestion, absorption, and bioavailability, providing a theoretical basis for designing more breast milk-like infant formulas.

Dairy technologists has attempted to produce “improved” infant formulas mimicking human milk by supplementation with bovine MFGM and/or phospholipids-enriched materials. The present study investigated and compared the lipid profile and particle sizes of mature human milk and infant formula fat globules (IF 1, IF 2, IF 3, and IF 4) and elucidated the relationship between physicochemical properties and in vitro simulated gastrointestinal lipolysis rate of the different milk samples. Despite having larger micron-sized fat globules, mature human milk demonstrated the highest gastrointestinal lipolysis rate with higher release of medium- and long-chain saturated fatty acids. In comparison, IF 3, which contained the lowest phospholipids content, demonstrated the lowest gastrointestinal lipolysis rate. Higher gastrointestinal lipolysis rate of mature human milk fat as compared to infant formula fats might be due to the presence of MFGM interfacial layer (phospholipids) surrounding the fat droplets which govern lipase activity on lipid droplets.

The milk fat globule membrane (MFGM) is rich in bioactive lipids essential for infants. Using Hebei as a reference region, this study investigated regional differences in MFGM phospholipid composition in human milk from Northeast China, where dietary patterns vary, including differences in fat sources and seafood intake. The results showed that although significant regional differences were observed in milk fat globule size and zeta potential, the overall microstructure remained largely consistent. Oleic acid (C18:1) was the predominant fatty acid across all regions. Hebei exhibited the highest saturated fatty acid content (48.85 ± 0.07%), whereas Liaoning showed the lowest saturated fatty acids (37.97 ± 0.11%). Phosphatidylcholine (PC) was dominant in Hebei and Liaoning, whereas sphingomyelin predominated in Heilongjiang. Orthogonal projections to latent structures-discriminant analysis identified 69-84 differential phospholipids among regions, with PC contributing most to regional discrimination. Network analysis revealed strong correlations between differential lipids and key hub molecules. These findings also provide a scientific basis for optimizing infant formula.

This study provides insights into the composition and in-situ structures of the milk fat globule membrane (MFGM) in buffalo milk with different fat globule sizes (0.55 μm and 8.04 μm). Small fat globules contained a higher amount of MFGM proteins and polar lipids, while the content of MFGM components (except for ADPH) was greater per unit membrane area in large fat globules. A total of 386 differentially expressed proteins were identified, with many of the most differentially expressed proteins being associated with human diseases, such as cancer and elderly diseases. The coverage of sphingomyelin and certain MFGM proteins (BTN and PAS 6/7) on large fat globules was higher, potentially linked to the presence of larger, irregular lipid rafts and patch-shaped glycocalyxes, respectively. These findings suggest that small fat globules are suitable for nutritional fortification of buffalo milk, while large fat globules are appropriate for the preparation of functional dairy-based ingredients.
