3D-printed polyetheretherketone (PEEK) scaffolds are developed as novel bone substitutes, however, their bioinert surface hinders osteogenesis and modifying PEEK scaffolds without blocking their inter-connected porous structure is challenged. In this study, 45S5 bioactive glasses (BG) were homogeneously coated on PEEK scaffolds inspired by traditional sand-casting. The structure of PEEK scaffolds was preserved under shaping effect of BG fillers. Modulus, coating yield and hydrophilicity of scaffolds after different thermal treatment time were comparatively investigated. Excellent hydroxyapatite-forming ability of BG-coated scaffolds was confirmed by mineralization study. In-vitro assessments, BG-coated scaffolds cultured with MC3T3-E1 cells presented potential as bone implants with excellent cytocompatibility and osteogenic properties. Following similar coating strategy, conductive particles, multi-component particles and template particles were also coated on PEEK surface. The proposed methodology highlights a useful approach towards producing tunable biomedical coatings and microstructure on porous PEEK scaffolds for bone tissue engineering.

Once bone is damaged, a rapid surge of reactive oxygen species (ROS) and anaerobic glycolysis will create an oxidative stress and acidic microenvironment, thus inhibiting osteogenic factor expression and disrupting bone regeneration. To address this, we innovatively developed an alkaline hydrogel-coated poly(ether ether ketone) (PEEK) porous scaffold with VitaFlux functionality in this study. This decorated PEEK scaffold distinct from those hydrogel-perfused scaffolds with obstructed pores still kept its initial 3D-printed pore structure to allow the ingrowth of bone tissues. The results showed that the modified scaffold could not only exhibit significant efficacy in lowering oxidative stress, neutralizing acidic microenvironment, and activating TGF-β1 expression in serum through its inherent alkalinity but also recruit stem cells to the bone defect site and upregulate osteogenic factor expression via the TGF-β1/Smad signaling pathway. This study proved the obvious advantage of the modified scaffold in enhancing bone regeneration by the synergy of immune microenvironment remodeling and osteogenic factor expression enhancement, thus providing a facile method to modify bone scaffold which can facilitate a desirable bone repair process through self-activating the transfer of growth factors between the material and tissues.

Treatments for malignant bone tumors are urgently needed to be developed due to the dilemma of precise resection of tumor tissue and subsequent bone defects. Although polyether-ether-ketone (PEEK) has widely attracted attention in the orthopedic field, its bioinertness and poor osteogenic properties significantly restrict its applications in bone tumor treatment. To tackle the daunting issue, we use a hydrothermal technique to fabricate novel PEEK scaffolds modified with molybdenum disulfide (MoS 2 ) nanosheets and hydroxyapatite (HA) nanoparticles . Our dual-effect synergistic PEEK scaffolds exhibit perfect photothermal therapeutic (PTT) property dependent on molybdous ion (Mo 2+ ) concentration and laser power density, superior to conventional PEEK scaffolds. Under near-infrared (NIR) irradiation, the viability of MG63 osteosarcoma cells is significantly reduced by modified PEEK scaffolds, indicating a tumor-killing potential in vitro . Furthermore, the incorporation of HA nanoparticles on the surface of PEEK bolsters proliferation and adherence of MC3T3-E1 cells, boosting mineralization for further bone defect repair. The results of micro-computed tomography (micro-CT) and histological analysis of 4-week treated rat femora demonstrate the preeminent photothermal and osteogenesis capacity of 3D-printed modified scaffolds in vivo . In conclusion, the dual-effect synergistic orthopedic implant with photothermal anticancer property and osteogenic induction activity strikes a balance between tumor treatment and bone development promotion, offering a promising future therapeutic option.

As an attractive biomaterial for orthopedic implants , polyetheretherketone (PEEK) has many prominent characteristics such as good biocompatibility and biomechanical properties , as well as natural radiolucency. However, the potential clinical application is limited due to its biologically inert surface and poor osseointegration. To solve this problem, we fabricate a macroporous PEEK scaffold modified with methacrylated chitosan/polyhedral oligomeric silsesquioxane (CSMA/POSS) nanocomposite through 3D printing , sulfonation and UV-induced graft polymerization. The water contact angle, protein adsorption ability and biomineralization study of the sulfonation-treated PEEK (SPEEK)-CSMA/POSS scaffolds were measured in order to evaluate the surface properties. The results indicated that CSMA/POSS as a bioactive microporous surface integrated with nanoparticles promoted the protein adsorption and apatite formation. For the in vitro and in vivo evaluation of the osteogenicity, rat bone marrow mesenchymal stem cells (rBMSCs) and rat calvaria defect repair model were used. Biological results showed that the 3D porous structure and bioactive surface of modified PEEK scaffolds provided a suitable condition for cell adhesion and proliferation, enhanced the osteogenic differentiation of rBMSCs and promoted osteogenesis in vivo compared to the untreated PEEK scaffold. Therefore, 3D-printed macroporous PEEK scaffold modified with CSMA/POSS bioactive microporous surface represents a promising modification strategy for enhancing osseointegration of PEEK implants.
