Improving Osseointegration Potential of 3D Printed PEEK Implants with Biomimetic Periodontal Ligament Fiber Hydrogel Surface Modifications

Xiaojie Xu,Jiahui Zuo,Huajing Zeng,Yuan Zhao,Zengjie Fan

The bioinertness, poor antibacterial properties, and weak osseointegration of polyether-etherketone (PEEK) limit its clinical application as oral implants. In this study, it is developed a personalized and customizable PEEK implant with multifunctional capabilities through sequential 3D printing, surface sulfonation (SPEEK), and modification using a drug-loaded photocured hydrogel composed of acrylic acid-acrylamide (AA-AM) containing simvastatin-cefepime (SIM-CFP). The modified implant, referred to as SCHSPEEK, mimics the structure of biomimetic periodontal ligament (PDL) fibers. The chemical composition, surface characteristics, and biological properties of SCHSPEEK both in vitro and in vivo are systematically investigated. The results demonstrate that hydrogel modification not only enhances hydrophilicity and reduces surface roughness and friction coefficient, but also improves biocompatibility, drug release, and antibacterial efficacy. Importantly, SCHSPEEK exhibits a significant improvement in osseointegration capacity in vivo. These findings highlight the potential of SCHSPEEK implants to serve as promising orthopedic and dental implants in clinical settings.

Endogenous Glucose/pH Dual-Responsive PEEK Implant With Cascade-Amplified Antibacterial and Osteogenic Activities for Infected Diabetic Bone Defects

Rui Ding,Jiaheng Liang,Shuo Wang,Pandi Peng,Jiabo Qi,Jiajun Zhang,Yuheng Zhou,Weihao Zhao,Yiqiu Zhou,Peng Li

Hyperglycemia in diabetics compromises bone defect repair by elevating infection risks and suppressing osteogenesis. Although polyetheretherketone (PEEK) possesses suitable mechanical properties and biocompatibility, the intrinsic bioinertness constrains its therapeutic efficacy in diabetics. Herein, we developed an antibiotic-free, glucose/pH dual-responsive PEEK implant that exclusively leverages endogenous conditions to trigger cascade-amplified therapeutic effects. This design features co-immobilization of synthesized copper-tannic acid nanoparticles (CuTANps) and glucose oxidase (GOx) onto a porous sulfonated PEEK surface. In this system, GOx catalyzes glucose to produce H 2 O 2 and gluconic acid. The resulting acidic environment (comprising both the inherent acidity of the bacterial infection and accumulated gluconic acid) induces the dissociation of CuTANps. The released Cu 2 ⁺ is then reduced to Cu⁺ by TA and intracellular glutathione, which subsequently reacts with H 2 O 2 via a Fenton-like reaction to generate highly reactive hydroxyl radicals, enabling amplified antibacterial effects. The optimized PEEK achieves killing rates exceeding 98% against drug-resistant Gram-positive/-negative bacteria in vitro. Correspondingly, this implant reduced bacterial colonization by over 97% in vivo, while the synergistic effect of Cu 2 ⁺ and the porous sulfonated surface further enhanced osseointegration, yielding approximately 1.5-fold increase in bone volume fraction compared to pristine PEEK. Together, this study developed an endogenous stimuli-responsive PEEK implant tailored for the infected diabetic bone defects microenvironment, which achieves cascade-amplified antibacterial and osteogenic therapies.

Enhanced osteogenesis and antibacterial activity of dual-functional PEEK implants via biomimetic polydopamine modification with chondroitin sulfate and levofloxacin

Mengjue Li,Junyan Liu,Yutong Li,Wenyu Chen,Zhou Yang,Yayu Zou,Yi Liu,Yue Lu,Jianfei Cao

Polyetheretherketone (PEEK) implants have emerged as a clinically favored alternative to titanium alloy implants for cranial bone substitutes due to their excellent mechanical properties and biocompatibility. However, the biological inertness of PEEK has hindered its clinical application. To address this issue, we developed a dual-functional surface modification method aimed at enhancing both osteogenesis and antibacterial activity, which was achieved through the sustained release of chondroitin sulfate (CS) and levofloxacin (LVFX) from a biomimetic polydopamine (PDA) coating on the PEEK surface. CS was introduced to promote cell adhesion and osteogenic differentiation. Meanwhile, incorporation of antibiotic LVFX was essential to prevent infections, which are a critical concern in bone defect repairing. To our delight, experiment results demonstrated that the SPKD/CS-LVFX specimen exhibited enhanced hydrophilicity and sustained drug release profiles. Furthermore, in vitro experiments showed that cell growth and adhesion, cell viability, and osteogenic differentiation of mouse calvaria-derived osteoblast precursor (MC3T3-E1) cells were significantly improved on the SPKD/CS-LVFX coating. Antibacterial assays also confirmed that the SPKD/CS-LVFX specimen effectively inhibited the growth of Escherichia coli and Staphylococcus aureus, attributable to the antibiotic LVFX released from the PDA coating. To sum up, this dual-functional PEEK implant showed a promising potential for clinical application in bone defects repairing, providing excellent osteogenic and antibacterial properties through a synergistic approach.
