High mechanical property especially the exorbitant elastic modulus is the common complication of the clinical polymethylmethacrylate (PMMA) bone cement which will generate the fracture of the adjacent bone and even aseptic loosening, other side effects including excess thermal temperature and low efficiency of the drug release bother the development of the PMMA bone cement. The present study aims to investigate the optimum dosage of gelatin as the porogen, which reduced the elastic modulus of the bone cement to the relatively close level of the cancellous bone. Meanwhile, better antibiotic release profile was introduced by enhancing the specific surface area and interconnectivity than the neat PMMA bone cement. Compared to the PMMA bone cement, the mechanical and thermal property was successfully reduced by the porous structure, the component with 200–400 μm gelatin has the better porosity which resulted in the increasing drug release amount and rate than that of the PMMA bone cement. Furthermore, data analysis and fitting curve could guide experiments, in turn, to obtain the PMMA bone cement with specific requirements of the mechanical properties by the addition of gelatin as the pore-forming agent, and in some cases for predictive purposes, to estimate how a change of gelatin may affect the porosity, mechanical properties, and drug release profiles.

Drugs are loaded into PMMA bone cement to reduce the risk of infection in freshly implanted prostheses or to promote the differentiation and growth of osteoblasts. However, the same method of loading of drugs in the bone cement cannot simultaneously achieve an effective antibacterial response and long-term treatment outcomes for osteoporosis based on a patient's clinical needs. In the present study, gentamicin sulfate (GS)/alendronate (ALN)-dual-loaded gelatin modified PMMA bone cement (GAPBC) was fabricated to provide rapid and continuous antibiotic release and long-term anti-osteoporotic therapy. Specifically, the gelatin microspheres were loaded with the drugs using separate methodologies, namely, ALN was loaded during fabrication of the gelatin microspheres after which GS was absorbed onto the gelatin from solution. The results demonstrate that sequential release of the GS and ALN was achieved, GS release playing a major role over the first 24 hours and ALN release dominant after 3 weeks of immersion in PBS, resulting from the graded distribution within the gelatin microspheres, and the final drug release ratio of GS (73.6%) and ALN (68.5%) from the modified bone cement was significantly higher than from PMMA bone cement. Therefore, GAPBC represents a potential drug carrier for future clinical applications.

A sortase A inhibitor (SrtA I)-loaded bone cement (SLBC) containing 1-(3,4-dichlorophenyl)-3-(dimethylamino)propyl-1-one hydrochloride (AEEK1) was reported. Compared with gentamicin-loaded bone cement (GS cement), which is a widely used antibiotic-loaded bone cement in cemented total joint replacement (TJR), SLBC demonstrated a significantly prolonged effective antibacterial duration against both Staphylococcus aureus (S. aureus) and methicillin-resistant S. aureus (MRSA). The quasi-static compressive strength, maximum polymerization temperature, and setting time of SLBC were within the limits stated in the ISO 5833 standard and comparable to the corresponding values of GS cement. In vitro and in vivo experimental results confirmed the biocompatibility of SLBC and showed its easy injectability into porcine T12 vertebrae. Taken together, these findings suggest that SLBC may have potential for managing/treating prosthetic joint infections, which is a common complication in cemented TJR.

ABSTRACT The treatment of vertebral compression fractures of the spinal necessitates the use of injectable bone repair materials with the adaptive modulus of elasticity. Currently, newly developed injectable self-curing materials for minimally invasive surgery are primarily categorized into calcium phosphate and calcium silicate systems, both of which exhibit excellent biocompatibility. However, calcium phosphate bone cement lacks adequate mechanical properties and the stress does not match between bone and material. Calcium silicate cement has a long curing time, low mechanical strength, and high alkalinity. These deficiencies render them inadequate for meeting clinical requirements. Therefore, we incorporated inorganic disodium phosphate, organic polyvinyl alcohol, and solid component cerium oxide into the matrix phase of tricalcium silicate bone cement to establish a ternary self-curing hydration reinforcement system and produced an organic-inorganic composite injectable bone cement. Even though the composite bone cement has a longer setting time, it exhibited high compressive strength (61.9Mpa) and fracture strain (7.88%), demonstrating excellent mechanical properties. Furthermore, it also demonstrates good injectable properties (86%), anti-collapse properties, good biological activity, and outstanding antibacterial properties. The composite bone cement has great potential to develop a new injectable self-curing material to replace PMMA bone cement in clinical vertebral compression fractures.
