Enhanced computed tomography (CT) imaging with iodinated imaging probes is widely utilized for the diagnosis and evaluation of various liver diseases. However, these iodine-based imaging probes face intractable limitations including allergic reactions and contraindications. Herein, we propose the utilization of renal-clearable iodine-free bismuth chelate (Bi-DTPA dimeglumine) for the non-invasive fast assessment of hepatic ischemia–reperfusion injury (HIRI) via CT imaging for the first time. Bi-DTPA dimeglumine offers several advantages such as simple synthesis, no purification requirement, a yield approaching 100%, large-scale production capability (laboratory synthesis > 100 g), excellent biocompatibility and superior CT imaging performance. In a normal rat model, the administration of Bi-DTPA dimeglumine resulted in a significant 63.79% increase in liver CT value within a very short time period (30 s). Furthermore, in a HIRI rat model, Bi-DTPA dimeglumine enabled the rapid differentiation between healthy and injured areas based on the notable disparity in liver CT values as early as 15 min post-reperfusion, which showed a strong correlation with the histopathological analysis results. Additionally, Bi-DTPA dimeglumine can be almost eliminated from the body via the kidneys within 24 h. As an inherently advantageous alternative to iodinated imaging probes, Bi-DTPA dimeglumine exhibits promising prospects for application in liver disease diagnosis.

Triple-negative breast cancer (TNBC) has emerged as one of the dreadful metastatic tumors in women due to complexity, specificity, and high recurrence, resulting in poor therapeutic outcomes and requiring real-time monitoring for improved theranostics. Despite the success as efficient radiosensitizers and computed tomography (CT)-based contrast agents, bismuth (Bi)-based composites suffer from poor colloidal stability, dose-dependent toxicity, and pharmacokinetic shortcomings, leading to poor therapeutic monitoring. In addition, several small molecule-based therapeutics, including nanoparticle-based delivery systems, suffer from several limitations of poor therapeutic delivery and acquired multidrug resistance by cancer cells, depriving the therapeutic needs. To overcome this aspect, this study demonstrates the fabrication of drug-like/drugless nanoarchitectures based on copper sulfide-nanoplated bismuth oxide (Bi2O3@CuS, shortly BC) composites for improved theranostic efficacy against TNBC. These systematically characterized BC nanocomposites exhibited pH-/near-infrared (NIR, 808 nm) light-responsive degradability towards dual modal therapies. Due to the band transition of Cu species, the designed BC composites displayed exceptional photothermal (PTT) conversion efficiency towards localized PTT effects. In addition to pH-/NIR-responsiveness, the internally overexpressed glutathione (GSH)-responsiveness facilitated the release of Cu2+ species for chemodynamic therapy (CDT) effects. To this end, the Bi3+ species in the core could be fully hydrated in the acidic tumor microenvironment (TME), resulting in GSH depletion and reducing CDT-induced reactive oxygen species (ROS) clearance, thereby ablating tumors. The acid-responsive degradability of CuS resulted in the intratumoral enrichment of BC, demonstrating remarkable CT imaging efficacy in vivo. Together, these pH-/NIR-/GSH-responsive biodegradable BC composites could realize the integrated PTT/CDT/CT theranostics against breast carcinoma.

Owing to the unpredictable size of wounds and irregular edges formed by trauma, nanofibers' highly customizable and adherent in situ deposition can contribute to intervention in the healing process. However, electrospinning is limited by the constraints of conventional polymeric materials despite its potential for anti-inflammatory and antimicrobial properties. Here, inspired by the Janus structure and biochemistry of nanometal ions, we developed an in situ sprayed electrospinning method to overcome bacterial infections and immune imbalances during wound healing. The bilayer fiber scaffold has a hydrophobic outer layer composed of polycaprolactone (PCL) and a hydrophilic inner layer composed of gelatin, poly (L-lactic acid) (PLLA), and magnesium oxide nanoparticles (MgO NPs), constituting the PCL/PLLA-Gelatin-MgO (PPGM) electrospun scaffold. This electrospun scaffold blocked the colonization and growth of bacteria and remained stable on the wound for continuous anti-inflammatory properties to promote wound healing. Furthermore, PPGM electrospinning modulated collagen deposition and the inflammatory microenvironment in the full-thickness skin model, significantly accelerating vascularization and epithelialization progression. This personalized Janus electrospun scaffold has excellent potential as a new type of wound dressing for first aid and wound healthcare.

Bioactive microspheres coated with acellular matrix (aECM) have received extensive attention in bone tissue engineering. In this work, biomimetic microspheres with different aECM ratios, uniform size and controllable size were prepared easily by blending natural porcine dermal aECM and poly (lactic-co-glycolic acid) (PLGA) using electrohydrodynamic spraying and solidification actuated by solvent extraction (ESASE) method. In this work, the appropriate polymer concentration and preparation voltage were investigated, and the surface morphology of the microspheres were observed by scanning electron microscope. Sirius red was used to visualize aECM exposure on the surface of the microspheres. The in vitro and in vivo experiments was carried out to evaluate the bioactivity and osteogenic properties of the microspheres. The results showed that the morphology and size of PLGA microspheres had little influence on the aECM blending. In vitro experiments showed that the higher the content of aECM, the better the cell adhesion performance. In vivo, rat calvarial defect models were observed and characterized at 4 and 8 weeks postoperatively, and the values of BV/TV of 50aECM/PLGA were 47.57 ± 1.14% and 72.92 ± 2.19%, respectively. The results showed that the skull healing effect was better in aECM-containing microspheres. In conclusion, aECM/PLGA composite microspheres can increase cell adhesion performance through the addition of aECM. Moreover, in vivo experiments have proved that aECM/PLGA microspheres is beneficial to bone repair, which means the aECM/PLGA microspheres is a promising bone tissue engineering material.

Poly (l-lactic acid) (PLLA) is a biocompatible, biodegradable material with piezoelectric properties, making it a promising candidate for providing self-powered stimulation to accelerate tissue repair. Repairs to various tissues, such as bone, cartilage, and nerve, necessitate distinct piezoelectric characteristics even from the same material. However, the extensive utilization of PLLA piezoelectric scaffolds in various tissue is hindered by their low and single piezoelectric constants. In this study, PLLA nanofiber membranes with enhanced and adjustable piezoelectric constants (d33) were fabricated through oriented electrospinning. By carefully controlling the parameters of the spinning solution, a steady increase in d33 values from 0 to 30 pC/N was achieved. This advancement allows tailoring of PLLA nanofiber membranes to meet various piezoelectric requirements of different tissues. As an example of tailoring the optimal piezoelectric constants, we developed PLLA-2-0, PLLA-2-10, PLLA-2-15 and PLLA-2-20 nanofiber membranes with d33 values of 0, 5, 10 and 15 pC/N, respectively. The impact of varying piezoelectric constants of PLLA nanofiber membranes on cellular behavior and repair efficacy were validated through in vitro cellular experiments and in vivo mandibular critical defect repair. The results indicated that PLLA-2-20 demonstrated superior cell proliferation rate up to 130% and an osteogenic differentiation level approximately twice of the control. In addition, PLLA-2-20 significantly promoted cell adhesion and migration, and the cell aspect ratio was about 5 times higher than that of the control group. In vivo, PLLA-2-20 optimal restorative effects on rat mandibles via endogenous mechanical force-mediated piezoelectric stimulation, leading to complete histological restoration within 8 weeks. These findings highlight the potential of the PLLA membranes with high and adjustable d33 by a straightforward process. This study provides a novel approach for the development of highly electroactive membranes tailored to specific tissue repair needs.

The utilization of intracanal medicaments is an indispensable procedure in root canal treatment(RCT). However, the conventional intracanal medicaments still need improvement regarding antimicrobial efficacy and ease of clinical operation. To address the above issues, OCT/PECT@OCT+ALK composite hydrogel characterized by programming sequential release of dual antimicrobial agents has been proposed. Thanks to the self-assemble ability of amphiphilic copolymer poly(ε-caprolactone-co-1,4,8-trioxa [4.6]spiro -9-undecanone)-poly(ethylene glycol)-poly(ε-caprolactone-co-1,4,8-trioxa [4.6]spiro-9- undecanone) (PECT), dual hydrophilic and hydrophobic antimicrobial agents could be easily encapsulated in the hydrogel system, and tailored for sequential drug release for a better antibiofilm effect. The hydrophilic octenidine (Octenidine dihydrochloride, OCT-HCl) is encapsulated in the hydrophilic part of hydrogel for instantaneous elevating the drug concentration through bursting release, and the hydrophobic octenidine(Octenidine, OCT) is further loaded into the PECT nanoparticles to achieve a slower and sustained release profile. Additionally, calcium hydroxide (Ca(OH)2) was incorporated into the system and evenly dispersed among PECT nanoparticles to create an alkaline (ALK) environment, synergistically enhancing the antibiofilm effect with higher efficiency and prolonged duration. The antibiofilm effect has been demonstrated in root-canal models and apical periodontitis rats, exhibiting superior performance compared to clinically used Ca(OH)2 paste. This study demonstrates that OCT/PECT@OCT+ALK composite thermosensitive hydrogel is a potential intracanal medicament with excellent antibiofilm effect and clinical operability.

Infected bone defects present a significant clinical challenge, as persistent bacterial burden and exacerbated inflammatory response induce microenvironmental acidification, which severely compromises tissue regeneration. Therefore, this study aims to construct a smart platform to regulate the local microenvironment and achieve the therapeutic effect of bone regeneration. Here, pitaya‑inspired compartmentalized microspheres (IBPR‑CuTA) were developed to provide spatiotemporally controlled pH‑responsive release of Cu 2+ , antibiotic rifampicin (RF) and natural extract icariin (ICA), coupling infection control with tissue regeneration. Specifically, under infected inflammation, the microspheres rapidly release Cu 2+ and RF, which facilitates early anti-bacterial and anti-inflammatory effects and promotes pH normalization. This is succeeded by a delayed release of remaining Cu 2+ to foster angiogenesis, and a subsequent sustained release of encapsulated RF and ICA, collectively providing long-term inhibition of infection recurrence and promotion of bone regeneration. Mechanistically, IBPR‑CuTA was associated with suppressed glucose metabolism-related signaling and TNF/NF‑κB activation, and induced a shift from pro‑inflammatory M1 to reparative M2 polarization. In vivo , IBPR‑CuTA concurrently eradicated infection and accelerated vascularized bone regeneration in an infected defect model. Collectively, these results demonstrate an immunometabolically guided, spatiotemporally programmed microsphere platform that integrates early bactericidal/antioxidant activity with sustained pro‑regenerative cues, highlighting translational potential for managing complex infected bone defects.

Oxidative stress is a central pathological driver in diabetic nephropathy (DN), leading to excessive reactive oxygen species (ROS) generation, chronic inflammation and progressive renal fibrosis. Effective modulation of oxidative imbalance thus represents a vital therapeutic strategy for mitigating DN progression. Here, we developed a bioinspired polyphenolic nanoplatform by first constructing polymerized grape seed polyphenol nanoparticles via enzymatic polymerization, followed by supramolecular assembly with the bioactive polyphenol resveratrol. This design combined the intrinsic bioactivity of polyphenols with active resveratrol integration, yielding a stable and biocompatible strategy that alleviated DN and enabled superior restoration of renal structure and function. In diabetic models, it promoted coordinated recovery of renal morphology, key structural proteins and physiological function, suggesting potential advantages for long-term renal protection beyond short-term biochemical improvement. These findings demonstrated that enzymatically engineered polyphenolic nanotherapeutics provided a potent and biocompatible approach for oxidative regulation and functional renal restoration in DN therapy.

Oral aphthous ulcers are among the most prevalent mucosal disorders, with infectious oral ulcers severely compromising patients’ quality of life. This study introduces a dual-responsive (ultrasound- and pH-sensitive) hydrogel–microsphere scaffold for their precise, staged management. The scaffold, composed of a pH-sensitive chitosan (CS) hydrogel and ultrasound-responsive sodium alginate (SA) microspheres, demonstrated injectability, biocompatibility, and sequential drug release. Specifically, ultrasound enhanced drug release and tissue penetration, improving therapeutic efficacy. The pH-responsive mechanism enabled targeted release of lidocaine (LID) and curcumin (CUR) at inflamed sites, which is expected to provide local anesthesia while promoting epithelial regeneration. Mechanistically, ultrasound stimulation activated PI3K-Akt and ECM-receptor interaction pathways, which were associated with enhanced cell migration, proliferation, and drug responsiveness. These effects not only enhanced the therapeutic efficacy, but also established a microenvironment favorable for tissue repair. Our platform integrates multimodal functionalities, including rapid release of the analgesic drug LID, followed by the sustained effects of CUR. Crucially, ultrasound facilitate this release process and enhance the therapeutic efficacy. This comprehensive approach modulates the local wound microenvironment, thereby creating favorable conditions for accelerating tissue repair. This work expands new avenues for stimulus-responsive drug delivery systems in the treatment of oral diseases and holds potential for clinical translation.

Hepatocellular carcinoma (HCC) remains a major cause of cancer-related mortality worldwide, with partial hepatectomy (PHx) serving as the primary curative treatment. Schizandrol B (SCHB) has demonstrated significant efficacy in promoting liver regeneration and restoring hepatic function following PHx. However, the clinical application of SCHB faces two critical challenges: poor oral bioavailability and inadequate liver-specific targeting. Here, this study developed a nanoemulsion based on natural components named SCHB@SPC/Gal-BSA/DHA. It effectively overcame the limitations of SCHB by synergizing Gal-BSA-mediated liver-specific targeting and DHA-enhanced intestinal absorption, achieving prolonged gastrointestinal tract retention, 2.03-fold higher oral bioavailability and 7.64-fold greater liver accumulation compared to free SCHB. In both 70% PHx and in situ PHx in HCC models, the nanoemulsion robustly accelerated liver regeneration, evidenced by upregulated proliferation markers via STAT3/YAP activation and normalized bile acids metabolism, ultimately restoring liver mass faster than control. This study demonstrates that the dual-targeted nanoemulsion effectively overcomes the key limitations of SCHB by combining enhanced intestinal absorption with liver-specific targeting. The developed nanoemulsion system not only improves drug delivery efficiency but also significantly promotes liver regeneration after PHx, offering a promising therapeutic approach for postoperative recovery in HCC patients while establishing a platform for future liver-targeted oral drug delivery systems.

The healing of infected wounds is a formidable clinical challenge that demands advanced biomaterials capable of simultaneously eradicating pathogens and orchestrating the complex regenerative process. Herein, we engineered an immunomodulatory and angiogenic multifunctional hydrogel dressing by integrating choline phosphorylated chitosan (CS-MCP) with tetracycline-loaded polydopamine nanoparticles (PDA@TH NPs) for the programmed healing of infected wounds. This composite hydrogel demonstrates on-demand, spatiotemporally controlled therapeutic responses. Under near-infrared (NIR) irradiation, it enables synergistic bactericidal activity through photothermal effects and triggered antibiotic release. Moreover, the system exhibits pH-responsive drug release behavior, specifically targeting the acidic microenvironment of infected tissues. Beyond its potent antibacterial function, the hydrogel actively promotes regenerative processes. In vitro, CS-MCP markedly enhanced the adhesion, proliferation, and tube formation of human umbilical vein endothelial cells, demonstrating potent pro-angiogenic effects. Furthermore, the polydopamine nanoparticles effectively scavenged reactive oxygen species (ROS), attenuating oxidative stress and inducing M2 macrophage polarization to foster an immunoregulatory microenvironment conducive to tissue repair. In a rat model of Staphylococcus aureus-infected full-thickness skin defects, the hydrogel significantly accelerated wound healing by comprehensively modulating the entire regeneration cascade: eliminating infection, mitigating inflammation, promoting angiogenesis, and enhancing collagen deposition. This study presents a novel immune-engaging and pro-regenerative strategy, representing a highly promising platform for the treatment of refractory infected wounds.

Bioadhesives are being increasingly used in clinical practice, but pose significant health risks due to their uncontrollable intraoperative leakage. Nevertheless, there is currently a critical lack of effective solutions. Herein, we propose a second near-infrared window (NIR-II) fluorescence-visible indocyanine green (ICG)-BioGlue adhesive for precision surgical adhesion with intraoperative leakage prevention. The fabricated ICG-BioGlue adhesive based on the non-covalent interaction exhibits nearly 100% fluorescence labeling efficiency and maintains long-term stable NIR-II fluorescence for up to 3 months. During surgery, ICG-BioGlue adhesive enables real-time visualization of wound adhesion in various fields such as aortic dissection adhesion and liver or kidney wound hemostasis, preventing leakage into surrounding tissues. Moreover, the stable fluorescence of ICG-BioGlue adhesive allows effective imaging-guided removal of long-term adhesive fragments in the body when they cause compression symptoms after surgery. Considering it consists solely of Food and Drug Administration-approved drugs ICG and BioGlue, with a straightforward synthesis method and versatile applicability, ICG-BioGlue adhesive holds excellent potential for clinical translation. Our study provides a leakage-preventing strategy to enhance biosafety and promote the widespread application of bioadhesives in clinical settings.
