Postoperative atrial fibrillation (POAF) is the most prevalent form of secondary atrial fibrillation and increase the risk of adverse cardiovascular outcomes, such as stroke, heart failure, and increased mortality. Herein, we designed an andrographolide (Andr)-loaded degradable polymer patch to deliver the drug directly to the atrial tissue for prevention of POAF. The sterile pericarditis (SP) rat model was adopted for highly relationship to clinical practice. The patch-released Andr effectively reduced the incidence of atrial fibrillation from 90% to 20%, and alleviated local atrial inflammation and oxidative stress in vivo, by using electrophysiological detection and histological analysis such as immunofluorescence, western blot, PCR and so on. In HL-1 cells, we found the use of Andr-loaded patch could strongly inhibited the cell death, reactive oxygen species (ROS) generation and mitochondrial injury caused by LPS. Meanwhile, the use of Andr-loaded patch could effectively inhibited macrophages polarize towards M1. Mechanistically, we verified that the regulation of the cytoplasm and mitochondria Ca2+ and ROS dynamic balance was quite important both in vivo and in vitro. Our strategy proved by regulating the inflammatory microenvironment, ROS balance and Ca2+ homeostasis, and the Andr-loaded atrial patch was effective for POAF in the SP rat model. The electrical signal of atrial stromal reentry in the case of this model was successfully mined, and the results of calcium channel were basically consistent with that of electrical signal channel. In addition, we have reported the infiltration and polarization of local inflammatory cells in the atrial of POAF at the tissue section level. Our study served as a new inspiration for the treatment of arrhythmic diseases and other ROS- and Ca2+- associated local illnesses.

Injectable facial fillers such as Sculptra® stimulate collagen regeneration to fill wrinkles; however, the collagen regeneration is not satisfactory due to the slow emergence of filling effect. In this study, we designed a regenerative dermal filler to provide both immediate and long-lasting filling effects. A hydrogel matrix composed of crosslinked hyaluronic acid (HA) and collagen was engineered to encapsulate porous poly(L-lactide) (PLLA) microspheres and tranexamic acid (TXA). The hydrogel matrix was administered via intradermal injection to achieve wrinkle filling. TXA is released to exert skin-whitening effects, while the porous PLLA microspheres and their degradation product, lactic acid, continuously stimulate collagen regeneration over an extended period. Facial volume increased immediately following hydrogel injection. Large amounts of new Type I and Type III collagens are generated. The porous structure of PLLA microspheres facilitated the “penetrating growth” of collagen fibers, which effectively filled facial depressions and smoothed wrinkles. Overall, the HA/collagen composite hydrogel filler exhibited excellent aesthetic effects.

Polypropylene (PP) mesh is commonly used in abdominal wall repair due to its ability to reduce the risk of organ damage, infections and other complications. However, the PP mesh often leads to adhesion formation and does not promote functional tissue repair. In this study, we synthesized one kind of aldehyde Bletilla striata polysaccharide (BSPA) modified chitosan (CS) hydrogel based on Schiff base reaction. The hydrogel exhibited a porous network structure, a highly hydrophilic surface and good biocompatibility. We wrapped the PP mesh inside the hydrogel and evaluated the performance of the resulting composites in a bilateral 1 × 1.5 cm abdominal wall defect model in rats. The results of gross observation, histological staining and immunohistochemical staining demonstrated the positive impact of the CS hydrogel on anti-adhesion and wound healing effects. Notably, the addition of BSPA to the CS hydrogel further improved the performance of the composites in vivo, promoting wound healing by enhancing collagen deposition and capillary rearrangement. This study suggested that the BSPA-modified CS hydrogel significantly promoted the anti-adhesion, anti-inflammatory and pro-angiogenesis properties of PP meshes during the healing process. Overall, this work offers a novel approach to the design of abdominal wall repair patches.

Bladder tissue engineering holds promise for addressing bladder defects resulting from congenital or acquired bladder diseases. However, inadequate vascularization significantly impacts the survival and function of engineered tissues after transplantation. Herein, a novel bilayer silk fibroin (BSF) scaffold was fabricated with the capability of vascular endothelial growth factor (VEGF) and platelet derived growth factor-BB (PDGF-BB) sequential release. The outer layer of the scaffold was composed of compact SF film with waterproofness to mimic the serosa of the bladder. The inner layer was constructed of porous SF matrix incorporated with SF microspheres (MS) loaded with VEGF and PDGF-BB. We found that the 5% (w/v) MS-incorporated scaffold exhibited a rapid release of VEGF, whereas the 0.2% (w/v) MS-incorporated scaffold demonstrated a slow and sustained release of PDGF-BB. The BSF scaffold exhibited good biocompatibility and promoted endothelial cell migration, tube formation and enhanced endothelial differentiation of adipose derived stem cells (ADSCs) in vitro. The BSF patch was constructed by seeding ADSCs on the BSF scaffold. After in vivo transplantation, not only could the BSF patch facilitate the regeneration of urothelium and smooth muscle, but more importantly, stimulate the regeneration of blood vessels. This study demonstrated that the BSF patch exhibited excellent vascularization capability in bladder reconstruction and offered a viable functional bioengineered patch for future clinical studies.

Postmenopausal osteoporosis (PMOP) is a predominant form of clinical osteoporosis. It has led to significant health and social burdens for older patients. Reestablishing the balance between osteogenic and osteoclastic is a crucial strategy for treating PMOP. Curcumin (Cur), a naturally derived polyphenolic substance, has gained recognition as a viable option for treating osteoporosis. Despite its potential, the clinical use of Cur is hindered by its limited bioavailability and the presence of side effects. Nanoparticles modified with aspartic acid octapeptide (ASP8) exhibit a strong affinity for bone tissue, facilitating targeted delivery. This study presents novel acid-responsive zeolite imidazolate framework-8 (ZIF) nanoparticles modified with ASP8 and loaded with Cur (Cur@ZIF@ASP8, CZA). Upon delivery by this nanoparticle drug delivery system, Cur can effectively regulate bone homeostasis, offering a potential therapeutic strategy for osteoporosis. This study demonstrated that CZA nanoparticles could successfully transport Cur to bone tissue without significant toxicity. Furthermore, nanoparticles promote bone formation and inhibit osteoclast activity. They also modify the expression of related genes and proteins, such as OCN, ALP, CTSK, and MMP9. Significant evaluations utilizing microcomputed tomography, Masson’s staining, hematoxylin and eosin staining, and immunofluorescence staining demonstrated that intravenous CZA administration in ovariectomized mice resulted in bone destruction while simultaneously reducing overall bone loss. In conclusion, CZA nanoparticles hold promise as a therapeutic option for osteoporosis.

Hydrogel-based wound dressings, which facilitate rapid wound closure and healing, are essential for effective wound management. However, the development of an ideal hydrogel that possesses excellent mechanical properties, effective self-healing capabilities, tissue adherence and antimicrobial characteristics for wound dressing presents a significant challenge in clinical settings. Inspired by lotus-fiber and mussel, we synthesized a novel multifunctional hydrogel composed of bacterial cellulose-reinforced dopamine-grafted oxidized hyaluronic acid/polyacrylamide (OHA-DA/PAM/BC). This was achieved through a one-pot reaction that employed free radical polymerization of acrylamide, dynamic Schiff bonding and intermolecular hydrogen bonding. Compared with the pure PAM hydrogels, which exhibited an elongation at break of 4022% and a maximum tensile strength of 26.42 kPa, the OHA-DA/PAM hydrogel demonstrated significantly enhanced stretchability at 9949% and an increased tensile strength of 34.73 kPa when 0.3% OHA-DA was incorporated during hydrogel formulation. Notably, the addition of 0.8% BC significantly enhanced the tensile strength to 57.04 kPa and super-stretchability to 10679%. The OHA-DA/PAM/BC hydrogel also exhibited remarkable self-healing capabilities, achieving a mechanical recovery of 84.74% within 12 hours. Additionally, its adhesive and injectable properties are advantageous for dynamic wound repair. Furthermore, the OHA-DA/PAM/BC hydrogel exhibited minimal hemolytic activity and potent intrinsic antibacterial properties against both E. coli and S. aureus. In a mouse model of wound healing, this hydrogel reduced the healing duration to 14 days while enhancing the regeneration of both skin structure and function. Histological analyses further revealed that the hydrogel significantly promoted the development of well-organized granulation tissue, angiogenic tissue and collagen accumulation in the wound region. This study successfully developed an OHA-DA/PAM/BC multifunctional hydrogel characterized by exceptional stretchability, self-healing, adhesiveness, injectability and antibacterial activity, demonstrating a significant impact on wound healing in vivo. These findings indicated that the OHA-DA/PAM/BC hydrogel holds substantial potential as wound dressings for future clinical applications.

High myopia severely threatens the visual health of adolescents, with pathological features of decreased collagen aggregation and scleral thinning, ultimately leading to axial elongation on preretinal imaging. Fibroblasts play crucial roles in scleral remodelling and myopia prevention. In this work, we developed a fibroblast-loaded carboxymethyl chitosan-aldehyde hyaluronic acid (CMCS-HA-CHO) injectable hydrogel for anti-scleral remodelling. The CMCS-HA-CHO hydrogel is formed through simple mixing under mild conditions via a Schiff base reaction between CMCS and HA-CHO. The CMCS-HA-CHO hydrogel can be injected into a posterior sclera with a low modulus, and the increasing modulus over time provides good mechanical support to the sclera. The hydrogel demonstrated excellent cytocompatibility and haemocompatibility, and the encapsulated fibroblasts maintained good activity. Both the hydrogel and fibroblast-loaded hydrogel effectively shortened the axial length of myopic eyes in guinea pigs in a deprivation model. In particular, the fibroblast-loaded hydrogel had the best therapeutic effect because of the synergy of cell therapy and mechanical support, which always shortened the eye axis within 4 weeks. Furthermore, increased collagen secretion promoted by fibroblasts can increase the thickness of the sclera and improve its biomechanical properties, ultimately repairing the physiological structure of the sclera. This fibroblast-loaded injectable hydrogel may represent a promising clinical approach for controlling myopia progression.

Tendinopathy is a common disorder that causes local dysfunction and reduces quality of life. Recent research has indicated that alterations in the inflammatory microenvironment play a vital role in the pathogenesis of tendinopathy. Herein, injectable methacrylate gelatin (GelMA) microspheres (GM) were fabricated and loaded with heparin-dopamine conjugate (HDC) and hepatocyte growth factor (HGF). GM@HDC@HGF were designed to balance the inflammatory microenvironment by inhibiting oxidative stress and inflammation, thereby regulating extracellular matrix (ECM) metabolism and halting tendon degeneration. Combining growth factors with heparin was expected to improve the encapsulation rate and maintain the long-term efficacy of HGF. In addition, the catechol groups on dopamine have adhesion and antioxidant properties, allowing potential attachment at the injured site, and better function synergized with HGF. GM@HDC@HGF injected in situ in rat Achilles tendinopathy (AT) models significantly down-regulated oxidative stress and inflammation, and ameliorated ECM degradation. In conclusion, the multifunctional platform developed presents a promising alternative for the treatment of tendinopathy.

Bioinert poly (methyl methacrylate) (PMMA) is widely employed as a bone cement material in orthopedic and trauma surgery applications; however, its susceptibility to bacterial infection and bioinert nature limits its clinical applications. In this study, we developed a PMMA-based bone cement incorporating a silver nanoparticle-carbon dots (AgNP@CDs) nanocomposite (∼70 nm) at concentrations (2 wt %) with a Young’s modulus (324.74 ± 7.08 MPa)to simultaneously combat bacterial infections, minimize cytotoxicity, and support tissue regeneration. The CDs stabilize and functionalize AgNPs, improving their dispersion and bioavailability while enabling the controlled and sustained release of antimicrobial ions through incorporation with bone cement. The antibacterial efficacy of the composite was thoroughly evaluated, revealing its ability to disrupt bacterial cell membranes, generate reactive oxygen species and inhibit bacterial growth. These mechanisms collectively contribute to a significant reduction in bacterial growth of up to ∼ 90% in both in vitro and in vivo studies. The incorporation of AgNP@CDs ensures sustained antimicrobial activity, preventing bacterial colonization by controlling the leaching of Ag ions. Biocompatibility assessments showed that the PMMA composite (PMMA@2Ag-CDs) significantly improved cell proliferation, adhesion, and migration compared with pure PMMA bone cement. Additionally, histological analysis revealed that the PMMA group showed a fibrous layer thickness of 699 ± 35.32 µm, indicative of inflammation, while the PMMA@2Ag-CDs group reduced this thickness from 301.18 ± 22.42 µm on day 7 to 198.07 ± 15.21 µm on day 14, significantly decreasing inflammation. The PMMA@2Ag-CDs composite demonstrated better tissue integration, with organized collagen deposition and enhanced angiogenesis, indicating more efficient tissue regeneration. The reduced inflammation and improved tissue remodeling suggest that this composite promotes a more favorable tissue regeneration environment and minimizes complications. This study demonstrates that the PMMA@2Ag-CDs composite offers a promising solution for the prevention of infections and mitigation of inflammatory responses. Functionalization of bone cement through the incorporation of Ag nanoparticle-carbon dot nanocomposites is a promising strategy with potential practical applications in orthopedic and trauma surgery.

Conventional hemodialysis and hemodiafiltration prove less effective at removing protein-bound uremic toxins (PBUTs) from the bloodstream of end-stage renal disease patients, primarily because PBUTs cannot pass through the small pores in the polymeric membranes. Hemoperfusion is an extracorporeal blood purification technique that employs an adsorption mechanism to remove multiple uremic toxins from such patients. Yet, the efficacy of hemoperfusion is constrained by some limitations of contemporary adsorbents, such as suboptimal capacity to adsorb PBUTs and poor hemocompatibility, presenting significant barriers for their clinical application. To address these challenges, we engineered a tailored hemoperfusion adsorbent by compounding sulfonated polysulfone (SPSf) and polyethyleneimine (PEI) into polyethersulfone (PES) microspheres to effectively capture and remove PBUTs through electrostatic interactions. Specially, we introduced sulfuric acid into the coagulation bath to increase the adsorption amount of the developed adsorbent (H-PES/SPSf@PEI microspheres), to neutralize strong positive charge of PEI, and to improve hemocompatibility. The tailored H-PES/SPSf@PEI microspheres neither damage blood cells nor activate the complement pathway when they contact with human blood. Moreover, H-PES/SPSf@PEI microspheres have a high adsorption amount towards major PBUTs, including hippuric acid (HA, 34.24 mg/g), 3-indoleacetic acid (IAA, 49.19 mg/g), p-cresol sulfate (PCS, 40.31 mg/g) and indoxyl sulfate (IS, 128.67 mg/g) by fitting adsorption isotherms. In a simulated hemoperfusion setting, the removal ratio of IS, IAA, PCS and HA by H-PES/SPSf@PEI microspheres reach nearly 75.33%, 41.73%, 44.36%, and 21.11%, respectively, with 47.89% of IS, 40.64% of IAA, 44.42% of PCS and 37.35% of HA being removed from BSA solution. In conclusion, H-PES/SPSf@PEI microspheres hold a potential to eliminate PBUTs from patients with end-stage renal disease.

Intraocular lens (IOL) is a crucial implant for cataract therapy. Posterior capsule opacification (PCO) is the most common postoperative complication after IOL implantation, which is the abnormal hyperplasia of the residual lens epithelial cells (LECS) after IOL implantation in cataract surgery. It is reported that the cellular microenvironment in the lens capsule changes after surgery, such as the elevated secretion of Matrix metalloproteinases (MMPs) and a decrease in pH due to undesired cell proliferation. In this study, MMP-2 and pH-triggered drug delivery polysaccharide multilayer coating was designed and introduced onto the IOL surface for obtaining the cellular microenvironment-sensitive drug-eluting intraocular implant. The methacrylated heparin (HEP-MA) was synthesized and used to layer-by-layer self-assemble with the doxorubicin-loaded chitosan nanoparticles on the IOL surface. The Matrix metalloproteinase-2 (MMP-2) sensitive peptide with Cysteine contained in both ends (GCRD-GPQGIWGQ-DRCG) was then used to crosslink the polysaccharide multilayer via the Michael addition reaction between sulfhydryl group in Cysteines and double bonds in methacrylate groups. The multilayer construction and subsequent cross-linking were validated through Ultraviolet-visible spectrophotometer (UV-Vis) and Fourier-transform infrared spectroscopy (FTIR). After modification, the IOL material surface becomes more hydrophilic while the optical properties were well maintained. The MMP-2 and pH-sensitive drug sustained release coating were successfully obtained on the IOL surface via such design. The enzyme-triggered cell proliferation inhibition was realized in the in vitro experiments. In an animal model, significant upregulation of MMP-2 was observed in the aqueous humor after cataract surgery. The multi-functionalized polysaccharide-coated IOL implanted in the animal eye via cataract surgery effectively inhibits PCO formation while it keeps good in-vivo biosafety.

Reconstructing bone defects remains a significant challenge in clinical practice, driving the urgent need for advanced artificial grafts that simultaneously promote vascularization and osteogenesis. Addressing the critical trade-off between achieving high porosity/strength and effective bioactivity at safe ion doses, we incorporated strontium (Sr) into β-tricalcium phosphate (β-TCP) scaffolds with a triply periodic minimal surface (TPMS) structure using digital light processing (DLP)-based three-dimensional (3D) printing. Systematically screening Sr concentrations (0-10 mol%), we identified 10 mol% as optimal, leveraging the synergy between the biomimetic TPMS architecture, providing exceptional mechanical strength (up to 1.44 MPa at 80% porosity) and facilitating cell recruitment, and precision Sr-dosing to enhance bioactivity. In vitro assays revealed that the Sr-TCP scaffold dose-dependently stimulated osteogenic differentiation and mineralization in mouse osteoblastic cell line (MC3T3-E1) cells, while also significantly enhancing the angiogenic capacity in human umbilical vein endothelial cells (HUVECs). In vivo studies indicated that the scaffold demonstrated synergistic osteogenic and angiogenic effects in rat femoral condylar defects, leading to marked improvements in bone healing. Collectively, this study establishes a novel design paradigm combining biomimetic topology with optimized ionic doping, resolving key limitations of conventional grafts and advancing the development of safe, highly effective biomaterials for vascularized bone regeneration.
