Type 2 diabetes mellitus (T2DM) has evolved into a global health crisis, driving a silent yet progressive hepatic pathological cascade that advances from nonalcoholic fatty liver disease to fibrosis and ultimately to cholangiocarcinoma with a high probability. However, the covert nature and gradual progression of this pathological cascade pose significant challenges for early detection and continuous monitoring. Herein, we design a multiparameter magnetic resonance imaging (MRI) strategy for visually monitoring T2DM-associated hepatic dysfunction across disease stages in vivo, based on dual-modality NaGdF4 nanoprobes. Owing to the long tumbling time (τR), NaGdF4 nanoprobes can enhance longitudinal relaxivity (r1) to brighten T1-weighted signals, while the regular arrangement of Gd3+ in the crystal induces magnetic anisotropy, creating local static magnetic field heterogeneity that generates negative signals in susceptibility-weighted imaging (SWI) sequences. The multiparametric MRI strategy combined with pathological analysis comprehensively characterized hepatic disease progression in a T2DM mouse model and confirmed the association between T2DM-induced liver injury and precancerous biliary transformation. With the NaGdF4 nanoprobes, key imaging features were successfully identified, including impaired hepatic metabolic capacity with prolonged contrast retention, hypoxia-induced neovascularization, and biliary tract lesions progressing from intermediate reactive hyperplasia to advanced cholangiocarcinoma. This multiparameter MRI approach provides noninvasive, high-resolution insights into hepatic metabolic dysfunction, vascular remodeling, and biliary pathology, offering a powerful tool for early diagnosis and prognostic assessment of T2DM-associated liver complications.

Multidrug-resistant Gram-negative bacteria present a critical challenge for both diagnosis and therapy because diagnostic and therapeutic molecules have difficulty entering Gram-negative bacteria, and these molecules can also be expelled by bacteria using efflux pumps. Here, we report a supramolecular intrabacterial assembly peptide (IAP) that enables selective bacterial internalization via siderophore-like uptake. After internalization, IAP undergoes intra-bacterial RNA-triggered transformation from monodisperse molecules to nanofibrils, which bypass efflux pumps and enhance intra-bacterial retention. The transformation from monodisperse molecules to nanofibrils enhances r 1 relaxivity of Gd magnetic resonance imaging (MRI) motif of IAP from 9.9 ± 0.2 m M − 1 ·s − 1 (monodisperse state) to 24.5 ± 1.1 m M − 1 ·s − 1 (nanofibril state), enabling activated T 1 -weighted MRI and fluorescence imaging for Gram-negative bacteria in vivo. Beyond imaging, the IAP-RNA nanofibrils inside bacteria inhibit protein synthesis, induce reactive oxygen species accumulation, and disrupt the bacterial envelope. The multiple bacterial death pathways give IAP excellent efficacy against drug-resistant Gram-negative in both pneumonia and diabetic wound infection models, and no detectable resistance after prolonged exposure. This work presents a chemically encoded supramolecular strategy for activatable imaging and therapy based on intra-bacterial supramolecular assembly.

Hollow manganese-dioxide-based nanoplatforms, which combine
hollow
structures with the intrinsic theranostic properties of Mn 2+ , have shown great potential for precise tumor diagnosis and treatment.
However, single imaging or therapeutic modalities are still insufficient
for high requirements of precise cancer theranostics. Herein, a hollow
manganese-based theranostic nanoplatform, MnO 2 –IR-820-Q/DOX@TMTP1
nanocomposite (Mn-NCs), was designed and synthesized. With the assistance
of the Mn-NCs, near-infrared-II (NIR-II, 900–1700 nm) fluorescence
imaging with high spatial resolution and low autofluorescence was
combined with magnetic resonance imaging (MRI) for highly efficient
and precise multimodal imaging of cervical tumors. The organic cyanine
dye IR-820 and PbS@CdS quantum dots (QDs) were integrated to form
a hybrid nanocomposite (IR-820-Q) with strong NIR-II fluorescence
emission, which was subsequently assembled onto a MnO 2 nanocarrier
to enable MRI and protect IR-820-Q from degradation. The hollow MnO 2 nanoshells served as a nanoscale structural framework for
IR-820-Q/DOX loading, tumor microenvironment-responsive degradation,
and Mn 2+ -mediated imaging/therapeutic functions. The cervical
tumor-targeting peptide TMTP1 was introduced to improve the tumor-targeting
potential of the nanocomposite, enabling NIR-II fluorescence visualization
of primary tumors and lymph node metastatic lesions. Therefore, Mn-NCs
are well suited for tumor-targeted imaging-guided intervention of
cervical cancer and lymphatic metastasis. By constructing the theranostic
nanoplatform, the photothermal therapy (PTT) as well as photodynamic
therapy (PDT) effect from the IR-820 was enhanced, and with the hydroxyl
radicals (via Fenton-like reaction) of Mn 2+ in the tumor
microenvironment, effective chemodynamic therapy (CDT) was also achieved.
The loading of doxorubicin (DOX) further endowed the nanoplatform
with a chemotherapeutic capability. Thus, the developed theranostic
nanoplatform showed strong NIR-II fluorescence/MRI-guided synergistic
PTT/PDT/CDT and chemotherapy, which significantly improved the therapeutic
outcomes. This study demonstrates a promising strategy for precise
cervical cancer diagnosis and treatment, highlighting the potential
of organic–inorganic hybrid nanomaterials for clinical translation.

Gadolinium (Gd)-based magnetic resonance imaging (MRI)
contrast
agents (CAs) suffer from limitations including rapid renal clearance
and potential biological risks, highlighting the urgency to develop
safer and more efficient alternatives. Herein, we report a novel Mn(II)-based
MRI CA (Mn@nPFC-1) constructed by loading Mn 2+ ions into
a nanoscale hydrogen-bonded organic framework (nPFC-1), which is self-assembled
from 1,3,6,8-tetra( p -benzoic acid)pyrene (H 4 TBAPy). The Mn@nPFC-1 exhibits high stability benefiting from its
integration of hydrogen bonding and π–π stacking
interactions between the building blocks. In-vitro MRI tests under
3.0 T scanner show that Mn@nPFC-1 exhibits a high longitudinal relaxation
rate ( r 1 = 7.40 mM –1 ·s –1 ), which is significantly superior to
the clinically approved MnDPDP and clinical used Gd-based CAs. Fast
field cycling nuclear magnetic resonance (FFC-NMR) reveals that the
enhanced relaxation performance arises from the restricted rotational
motion of Mn 2+ and the prolonged rotational correlation
time. In-vivo studies across multiple tumor models demonstrate that
Mn@nPFC-1 enables effective T 1 -weighted
MRI contrast enhancement; it can further detect ultrasmall tumors
(3.29 mm 3 ) and assist in accurate MRI-guided tumor resection.
This work provides a promising strategy for designing biosafe, high-performance
Mn-based MRI CAs using hydrogen-bonded organic frameworks as carriers,
holding great potential for clinical tumor diagnosis and image-guided
therapy.

While targeted theranostics for cancer remains a pivotal research frontier, conventional ligand conjugation strategies exhibit persistent limitations in off-target accumulation and suboptimal tumor specificity, ultimately failing to achieve reliable detection of early-stage lesions or metastatic nodule while demonstrating insufficient therapeutic payload delivery. In this study, the manganese sulfide (MnS) nanoplatform was synthesized using transferrin (Tf) with tumor-targeting properties as carrier by a simple fabrication method. Notably, compared to clinically prevalent Gd-based contrast agents, Tf-MnS exhibited superior T1-weighted magnetic resonance imaging (MRI) contrast performance, with the longitudinal relaxation (r1) reaching 7.5253 mM-1 •s-1 which was significantly higher than 3.2915 mM-1 •s-1 of Gd-DTPA, and in the MRI of subcutaneous tumors and lung metastatic lesion in mice, the maximum relative signal-to-noise ratios reached 46.33% and 40.33% respectively. Remarkably, upon reaching the acidic tumor microenvironment, Tf-MnS disintegrated to release Mn2+ ions and hydrogen sulfide (H2S). The Mn2+ ions participated in Fenton-like reactions to produce cytotoxic hydroxyl radicals, while H2S concurrently inhibited catalase enzyme activity, thereby alleviating the insufficiency of hydrogen peroxide substrate and amplifying the therapeutic outcome. This synergistic mechanism endowed Tf-MnS with self-enhanced anti-tumor effect, inhibiting both lung metastatic lesion and subcutaneous tumors in mice of the Tf-MnS group, with a tumor inhibition rate of 54.26%. Collectively, this work proposes an innovative strategy for integrating accurate diagnosis and self-augmented therapy of tumors and lung metastatic lesion into a unified nanoplatform, offering a promising methodology for precision oncology.

Introduction: Magnetic Resonance Imaging (MRI)-guided photothermal therapy (PTT) holds significant promise for the treatment of solid tumors, however, its diagnostic and therapeutic efficacy in malignant pleural mesothelioma (MPM) remains underexplored. To address the limitations of traditional gadolinium (Gd)-based MRI contrast agents, such as intolerance in patients with renal insufficiency and T1 signal attenuation at high field strengths, as well as oxidative stress damage caused by manganese ion leakage from manganese-based materials, this study aims to develop highly chelated and stable manganese polydopamine (Mn-PDA) multifunctional nanoprobes for MRI monitoring and PTT treatment of MPM.Methods: Mn2+ and dopamine were chelated into Mn-PDA NP nanospheres in an ethanol-water system. The physicochemical properties were characterized using Transmission Electron Microscopy (TEM), Dynamic Light Scattering (DLS), zeta potential, Fourier Transform Infrared Spectroscopy (FT-IR), X-ray Photoelectron Spectroscopy (XPS), and Electron Paramagnetic Resonance (EPR). The photothermal performance under 808 nm laser irradiation was also evaluated. The MRI imaging performance (longitudinal/transverse relaxation rates, r1/r2) and manganese ion deposition quantification of Mn-PDA NPs were evaluated in vitro and in vivo at 3.0 T MR scanner. Additionally, the PTT efficacy of Mn-PDA NPs was evaluated in MSTO-211H cells and xenograft tumor models. Biocompatibility was assessed through H&E staining of major organs and blood biochemical analyses. Results: Mn-PDA NPs exhibited a spherical morphology with uniform size (∼200 nm), along with high photothermal conversion efficiency, favorable T1-weighted MRI contrast enhancement and low cytotoxicity. At 8 hours post-injection in vivo, Mn-PDA NPs resulted in a 42.9% enhancement in T1-weighted MRI signals within MPM tumors, accompanied by specific tumor accumulation. Under 808 nm laser irradiation, Mn-PDA NPs inhibited MPM tumor growth, as evidenced by reduced tumor volume, expanded areas of tumor necrosis (H&E staining), and downregulated Ki-67 expression. Moreover, stable body weight, normal histological features of major organs, and unaltered blood biochemical parameters were observed in the animals.Discussion: These findings indicate that Mn-PDA NPs are effective dual-functional agents, serving as both MRI contrast agents and photothermal therapeutics. They exhibit good tumor targeting ability, excellent imaging performance, potent therapeutic efficacy, and high biosafety, thereby offering a promising therapeutic strategy for MPM management.

In this study, fluorescence-encoded magnetic biocompatible nanoparticles (NPs) were constructed from CdSe@ZnS quantum dots (QDs) and Fe3O4 nanoparticles with a one-step reprecipitation-encapsulation method. The resultant hybrid NPs exhibit small size (∼130 nm in diameter), highly bright QDs, two-color emissions (green and red) under single-wavelength excitation, easy separation with a magnet and efficient cellular internalization. Energy transfer between the incorporated QDs was studied to better tailor the encoded fluorescence, and 11 barcodes were obtained by adjusting the ratio of green and red QDs. We used four sets of the barcodes to tag specific cancer cells (HepG2) as a proof-of-concept, and distinguished each set according to respective overlayed fluorescence images using laser confocal microscopy. Moreover, the incorporated Fe3O4 NPs endowed as-constructed optical barcode superparamagnetic property by T2-enhanced magnetic resonance effect with an r2 value of 145.25 s−1 mM−1 at 3 T. These results suggest that the multifunctional NPs are very promising for discriminating different cells and dual-modality imaging.

Myeloperoxidase (MPO)-mediated oxidative stress drives inflammatory tissue injury, yet converting this enzyme activity into a selective and sustained imaging readout remains chemically challenging. To address this limitation, we report Mn-TyrCDTA, a manganese chelate designed to couple kinetic inertness with MPO-triggered activation and retention mechanism. Replacement of a flexible EDTA backbone with a rigidified CDTA scaffold improved the kinetic inertness 3-fold under a Zn2+ challenge (dissociation t1/2 = 61.7 min). Incorporation of a tyramine-derived phenolic moiety enabled MPO/H2O2-mediated, one-electron oxidation and covalent protein anchoring, resulting in a 3.6-fold relaxivity enhancement and prolonged inflamed tissue retention. In rat models of acute pancreatitis, contrast enhancement correlated with tissue MPO activity (R2 = 0.83), enabling quantitative disease severity stratification. Complementary 68Ga-TyrCDTA PET studies demonstrated enzyme-dependent tracer accumulation, and MPO inhibition reduced the imaging signal by 85% (R2 = 0.98). These findings establish a rational design framework for the quantitative imaging of neutrophil-driven oxidative tissue injury.

“Always-on” MRI nanoprobe easily generates artifact signal due to non-specific interaction with tumor, resulting in inaccurate diagnosis. Stimulus-responsive nanoswitch can specifically amplify tumor signal and decrease background signal, improving diagnostic efficacy of the tumor. However, single stimulus for tumor specificity is often insufficient, which is prone to interference and leads to off-target effects. Herein, Cu and GSH “AND” logic-gated responsive nanoswitch (cRGD-dsESIONP) was developed by the conjugation of disulfide bonds between cRGD cyclopeptide and ESIONP. For tumors within only high GSH expression, the disulfide bonds on the surface of cRGD-dsESIONP were cleaved by GSH, but cRGD-dsESIONP cannot assembly into aggregates and still exhibited T 1 contrast imaging. However, for the tumors within high GSH and Cu expression, the cRGD-dsESIONP was cleaved into ESIONP-SH by GSH, subsequent ESIONP-SH captured intracellular Cu and assembled into aggregates, which resulted in switching from T 1 contrast imaging to T 2 contrast imaging, then enlarged tissue signal-to-noise ratio, and accurately diagnosed tumors. In vivo experimental results demonstrated that cRGD-dsESIONP had excellent biocompatibility and enabled precise imaging for microtumors within high Cu and GSH expressions. The design of logic-gated nanoswitch in response to GSH and Cu provides a novel approach for accurate MRI diagnosis of microtumors.

Developing of multifunctional nanoplateforms for simultaneous accurate cancer detection and target therapy is essential in cancer treatment. Herein, we present a facile synthesis of DOX/MNPs-FA nanocomposites for high-performance MRI and tumor-targeted drug delivery. In such core–shell structured DOX/MNPs-FA nanocomposite, the high magnetism of iron oxide core can be tuned and achieved by controlling the Zn2+ dopant amount, making them suitable as excellent contrasts in T2-weighted MR imaging. The outer porous silica shell with large pores about 5.4 nm in diameter possesses high surface area and pore volume. Anticancer drug (DOX molecules) can be stored in the large pore channels and is triggered to be released under acidic condition (pH 4–6). Importantly, the presence of folic acid on the surface of DOX/MNPs-FA allows the targeted delivery of the DOX to tumor cells, and therefore improves tumor chemotherapy efficiency. Our in vitro studies also demonstrated that DOX/MNPs-FA could be efficiently internalized into HeLa cells via the folate receptor-mediated endocytosis, and generate a greater cytotoxicity toward cancer cells compared to free DOX and DOX/MNPs. Therefore, these DOX/MNPs-FA multifunctional nanocomposites have great potential applications for simultaneous tumor diagnosis and targeted chemotherapy.

Nanocomposites combining magnetic and plasmonic components have received widespread attention in recent years due to their potential applications in biomedical research. Herein, we describe a facile method for growing small iron oxide nanoparticles on various plasmonic core materials with different shapes and surfaces by utilizing a polypyrrole interlayer. As a promising new form of non-apoptotic regulated cell death, ferroptosis has potential as an effective supplement to apoptosis-based cancer treatments. However, high intracellular glutathione (GSH) levels and insufficient hydrogen peroxide (H 2 O 2 ) in the tumor limit the efficacy of ferroptosis. Here, we designed a theranostic nanoplatform, named FCS/GCS, by incorporating amphiphilic polymer skeletal (P-SS-D), cinnamaldehyde prodrug (CA-OH) and iron ions (Fe 3+ )/gadolinium ions (Gd 3+ ) via chelation reactions between Fe 3+ /Gd 3+ and polyphenols. When delivered in the tumor microenvironment with high GSH level, the nanoparticles are depolymerized by the poly(disulfide) backbone of P-SS-D. The activated CA consumes the GSH and elevates intracellular H 2 O 2 , followed by a high level of Fenton reaction to generate abundant •OH levels. The generation of reactive oxygen species (ROS) further accelerates CA activation. The GSH consumption by disulfide , CA and Fe 3+ , downregulates GPX4 and generates •OH, which accelerate lipid peroxides (LPO) accumulation and consequently enhances ferroptosis. Additionally, the released Gd 3+ may serve as a contrast agent for tumor-specific T 1 -weighted magnetic resonance imaging (MRI). Thus, the rationally designed FCS/GCS system is a promising strategy for effective MRI-based visual ferroptosis therapy. Statement of significance Ferroptosis is a new form of non-apoptotic regulated cell death and has potential as an effective supplement to apoptosis-based cancer treatment. However, the efficiency of ferroptosis is limited by excessive glutathione level and insufficient hydrogen peroxide level in tumor site. In this study, we fabricate a theranostic nanoplatform (FCS/GCS) to amplify oxidation stress in tumor site for effective ferroptosis-based cancer treatment, and tumor specific magnetic resonance imaging is introduced for supervision. Our nanoplatform may provide a promising strategy for MRI-based visual ferroptosis therapy with high specificity and efficiency.
