Oxygen Self-Supplied Nanoplatform for Enhanced Photodynamic Therapy Against Enterococcus Faecalis Within Root Canals

Jiawen Chen,Hong Zhang,Tiancong Zhao,Yiyan Yu,Jiazhuo Song,Yuanhang Zhao,Hamed Alshawwa,Xinying Zou,Zhimin Zhang

The successful treatment of persistent and recurrent endodontic infections hinges upon the eradication of residual microorganisms within the root canal system, which urgently needs novel drugs to deliver potent yet gentle antimicrobial effects. Antibacterial photodynamic therapy (aPDT) is a promising tool for root canal infection management. Nevertheless, the hypoxic microenvironment within the root canal system significantly limits the efficacy of this treatment. Herein, a nanohybrid drug, Ce6/CaO 2 /ZIF-8@PEI, is developed using a bottom-up strategy to self-supply oxygen for enhanced aPDT. Polyethylenimine (PEI) provides a positively charged surface, which enables precise targeting of bacteria. CaO 2 reacts with H 2 O to generate O 2 , which alleviates the hypoxia in the root canal and serves as a substrate for Ce6 under 660 nm laser irradiation, leading to the successful eradication of planktonic Enterococcus faecalis ( E. faecalis) and biofilm in vitro and, moreover, the effective elimination of mature E. faecalis biofilm in situ within the root canal system. This smart design offers a viable alternative for mitigating hypoxia within the root canal system to overcome the restricted efficacy of photosensitizers, providing an exciting prospect for the clinical management of persistent endodontic infection. Ag-loaded mesoporous bioactive glasses against Enterococcus faecalis biofilm in root canal of human teeth

Wei FAN,Daming WU,Tengjiao MA,Bing FAN

Ag-loaded mesoporous bioactive glass (Ag-MBG) powders were synthesized and characterized. The ions release of Ag-MBGs in Tris-HCl and the pH stability of simulated body fluids after immersing Ag-MBGs were tested. Root canals were inoculated with Enterococcus faecalis for 4 weeks, and the antibacterial activity of MBGs, Ag-MBGs and calcium hydroxide against E. faecalis biofilm were evaluated. Results showed that Ag-MBGs possessed highly ordered mesoporous structure with silver nanoparticles deposited in the mesopores, which enabled a sustained Ag ionsreleased. The biofilms treated with Ag-MBGs showed a significant structural disruption compared with MBGs. These results indicated that Ag-MBGs possess a potent antibacterial effect against E.faecalis biofilm in root canal, and the antibacterial activity was induced by the release of Ag ions from Ag-MBGs.

Omadacycline Efficacy against Enterococcus faecalis Isolated in China: In Vitro Activity, Heteroresistance, and Resistance Mechanisms

Zhiwei Lin,Zhangya Pu,Guangjian Xu,Bing Bai,Zhong Chen,Xiang Sun,Jinxin Zheng,Peiyu Li,Di Qu,Qiwen Deng,Zhijian Yu

This study aimed to evaluate the in vitro antimicrobial activity, heteroresistance emergence, and resistance mechanism of omadacycline (OMC) in clinical Enterococcus faecalis isolates from China. A total of 276 isolates were collected retrospectively in China from 2011 to 2015. The MICs of OMC, doxycycline (DOX), and minocycline (MIN) against E. faecalis were determined by broth microdilution. Tetracycline (TET)-specific resistance genes and multilocus sequence typing (MLST) of the isolates were investigated using PCR. The detection frequency of OMC heteroresistance in E. faecalis was evaluated with population analysis profiling (PAP). The mechanism of OMC heteroresistance and resistance in E. faecalis was examined by amplifying 30S ribosomal subunit genes, RNA sequencing (RNA-Seq), and in vitro recombination experiments. The OMC MICs of clinical E. faecalis isolates ranged from ≤0.06 to 1.0 mg/liter, and 42% of the E. faecalis isolates with an OMC MIC of 1.0 mg/liter were found to be sequence type 16 (ST16). Six OMC-heteroresistant isolates with MIC values of ≤0.5 mg/liter were detected among 238 E. faecalis isolates. The resistant subpopulations of heteroresistant isolates showed OMC MICs in the range of 2 to 4 mg/liter and were found without 30S ribosomal subunit gene mutations. Moreover, RNA sequencing and in vitro recombination experiments demonstrated that overexpression of a bone morphogenetic protein (BMP) family ATP-binding cassette (ABC) transporter substrate-binding protein, OG1RF_RS00630, facilitated OMC heteroresistance in E. faecalis. In conclusion, OMC exhibited better activity against clinical E. faecalis isolates from China than that of DOX or MIN, and overexpression of OG1RF_RS00630 in E. faecalis facilitated the development of OMC heteroresistance.

Calcium-silicate mesoporous nanoparticles loaded with chlorhexidine for both anti- Enterococcus faecalis and mineralization properties

Fan Wei,Li Yanyun,Sun Qing,Ma Tengjiao,Fan Bing

Background In infected periapical tissues, Enterococcus faecalis is one of the most common dominant bacteria. Chlorhexidine has been proved to show strong antibacterial ability against E. faecalis but is ineffective in promoting mineralization for tissues around root apex. Mesoporous calcium-silicate nanoparticles are newly synthesized biomaterials with excellent ability to promote mineralization and carry-release bioactive molecules in a controlled manner. In this study, mesoporous calcium-silicate nanoparticles were functionalized with chlorhexidine and their releasing profile, antibacterial ability, effect on cell proliferation and in vitro mineralization property were evaluated. Results The chlorhexidine was successfully incorporated into mesoporous calcium-silicate nanoparticles by a mixing-coupling method. The new material could release chlorhexidine as well as Ca2+ and SiO3 2− in a sustained manner with an alkaline pH value under different conditions. The antimicrobial ability against planktonic E. faecalis was dramatically improved after chlorhexidine incorporation. The nanoparticles with chlorhexidine showed no negative effect on cell proliferation with low concentrations. On dentin slices, the new synthesized material demonstrated a similar inhibitory effect on E. faecalis as the chlorhexidine. After being immersed in SBF for 9 days, numerous apatite crystals could be observed on surfaces of the material tablets. Conclusion sMesoporous calcium-silicate nanoparticles loaded with chlorhexidine exhibited release of ions and chlorhexidine, low cytotoxicity, excellent antibacterial ability and in vitro mineralization. This material could be developed into a new effective intra-canal medication in dentistry or a new bone defect filling material for infected bone defects.

Piezo-Fenton tandem catalysis of single-phase Bi2Fe4O9 nanosheets against Enterococcus faecalis biofilms in dentin canals

Lixiang Jian,Zhenyue Yu,Xiao Wu,Chunlin Zhao,Cong Lin,Min Gao,Xinan Chen,Yi Lin

Enterococcus faecalis ( E. faecalis ) is one of the primary bacteria that causes endodontic failure. The formation of biofilms in root canals makes it resistant to common antimicrobial irrigants. Here, single-phase Bi 2 Fe 4 O 9 (BFO) nanosheets are used to effectively eradicate E. faecalis biofilms via piezo-Fenton tandem catalysis. The nanosheet morphology endows BFO with excellent piezocatalytic ability, which boosts the production of H 2 O 2 under ultrasonic (US) stimuli. The in-situ generated H 2 O 2 further facilitates the iron-based Fenton reaction in BFO, thereby accelerating the destruction of membrane structures of E. faecalis biofilms. Moreover, the piezo-Fenton tandem catalysis only requires an US power of 60 W, which can be readily obtained via US irrigation during routine dental treatments. Accordingly, single-phase BFO nanosheets provide a practical and highly efficient approach for eradicating E. faecalis biofilms in dentin canals.

Comprehensive assessment of Enterococcus faecalis SN21-3: Probiotic features and safety evaluation for potential animal use

Haibo Lou,Jian Wang,Yaping Wang,Yongdong Gao,Wei Wang

In this study, the probiotic characteristics of Enterococcus faecalis SN21-3 were explored by genotype and phenotype, and the safety of this strain was evaluated in vitro and vivo . The complete genome sequence of E. faecalis SN21-3 revealed basic genetic information and corresponding genes underlying its probiotic characteristics. In vitro experiments were performed with E. faecalis SN21-3, which showed prominent antimicrobial activity and excellent abilities of resistance to simulated gastrointestinal fluid, cholesterol assimilation, adhesion, and antioxidant. In addition, E. faecalis SN21-3 was susceptible to the most common antimicrobial agents, did not contain transferable resistance gene and pathogenic gene, and did not perform haemolysis reactions and gelatinase activity in vitro . Furthermore, in vivo experiments demonstrated that feeding E. faecalis SN21-3 to broilers for 28 days did not significantly affect their normal growth or blood biochemistry parameters. Moreover, H&E staining showed no evidence of lesions in the heart, liver, spleen, or kidney of the broilers. No migration of E. faecalis SN21-3 was observed in the heart, liver, spleen, kidney, or blood samples during the feeding trial, indicating that the bacteria did not cause infections. In summary, E. faecalis SN21-3 exhibits numerous probiotic characteristics and is sufficiently safe for use as a novel animal probiotic.

Substantivity of Ag–Ca–Si mesoporous nanoparticles on dentin and its ability to inhibit Enterococcus faecalis

Fan Wei,Wu Yujie,Ma Tengjiao,Li Yanyun,Fan Bing

The main purpose of this study was to investigate the substantivity of Ag–Ca–Si mesoporous nanoparticles (Ag-MCSNs) on dentin and its residual antibacterial effects against Enterococcus faecalis . Ag-MCSNs were fabricated and characterized, ion release profile and pH were tested, and the ability to inhibit planktonic E. faecalis as well as the cytotoxicity was evaluated. Dentin slices were medicated with Ca(OH) 2 paste, 2 % chlorhexidine gel and Ag-MCSNs paste for 7 days and then irrigated. Dentin slices were then immersed in E. faecalis suspension for 6 days and then transferred to fresh brain heart infusion solution. The optical density value within 10 h after immersing and transferring were measured and compared among groups. Results indicated that Ag-MCSNs showed high pH, sustained Ag + –Ca 2+ –SiO 3 2− ion release, and high substantivity on dentin. The Ag-MCSNs exhibited strong antibacterial effects against planktonic E. faecalis and much better residual inhibition effects against E. faecalis growth on dentin than Ca(OH) 2 paste ( P  < 0.05). The Ag-MCSNs showed excellent antibacterial ability against E. faecalis and high substantivity on dentin, which might be developed to a new effective intra-canal medicament for human teeth.

Synergistic effects of silver ions and metformin against enterococcus faecalis under high-glucose conditions in vitro

Wu Xuying,Fan Wei,Fan Bing

Background This study aimed to evaluate the synergistic antibacterial activities of silver ions (Ag+) and metformin hydrochloride (Met) against Enterococcus faecalis (E. faecalis) under normal or high-glucose conditions. Results The minimum inhibitory concentration, minimum bactericidal concentration, growth curves, and colony-forming units were used to evaluate the antibacterial effects of Ag+ and Met on planktonic E. faecalis in Brain Heart Infusion broth with or without additional glucose. The influences of Ag+ and Met on four weeks E. faecalis biofilm on human dentin slices was also tested. Cytotoxicity was tested on MC3T3-E1 osteoblastic cells using CCK-8 assays. The results indicated that E. faecalis showed higher resistance to drug treatment under high-glucose conditions. Ag+ (40 μg/mL) plus Met (3.2% or 6.4%) showed enhanced antibacterial activities against both planktonic E. faecalis and biofilm on dentin slices, with low cytotoxicity. Conclusion sMet enhanced the bactericidal effects of Ag+ against both planktonic and biofilm E. faecalis under normal or high-glucose conditions with low cytotoxicity. Further molecular studies are needed to be conducted to understand the mechanisms underlying the synergistic activity between Met and Ag+.

Docosahexaenoic acid inhibits pheromone-responsive-plasmid-mediated conjugative transfer of antibiotic resistance genes in Enterococcus faecalis

Hongrui Zhou,Xiaobo Yang,Yutong Yang,Yuanyuan Niu,Jing Li,Xinyue Fu,Shang Wang,Bin Xue,Chenyu Li,Chen Zhao,Xi Zhang,Zhiqiang Shen,Jingfeng Wang,Zhigang Qiu

The rapid spread of antibiotic-resistance genes (ARGs) in Enterococcus faecalis ( E. faecalis ) poses a great challenge to human health and ecological and environmental safety. Therefore, it is important to control the spread of ARGs. In this study, we observed that the addition of 5 μg/mL docosahexaenoic acid (DHA) reduced the conjugative transfer of pCF10 plasmid by more than 95% in E. faecalis . DHA disturbed the pheromone transport by inhibiting the mRNA levels of the prgZ gene, causing the iCF10 pheromone to accumulate in the donor bacteria and bond to the PrgX receptor to form an inhibitory phase, which resulted in the down-regulation of the expression of genes related to conjugative transfer, inhibiting biofilm formation, reducing bacterial adhesion and thus inhibiting conjugative transfer. Collectively, DHA exhibited an admirable inhibitory effect on the transfer of ARGs in E. faecalis . This study provided a technical option to control the transfer of ARGs.

Phenylboronic Acid-Modified Gold Nanoclusters as a Nanoantibiotic to Treat Vancomycin-Resistant Enterococcus faecalis-Caused Infections

Le Wang,Wenfu Zheng,Leni Zhong,Yingkun Yang,Yao Chen,Qinghong Hou,Peiyuan Yu,Xingyu Jiang

Vancomycin is one of the last lines of defense against certain drug-resistant bacteria-caused infections. However, the high susceptibility to drug resistance and high toxicity seriously limit the application of vancomycin. Nanoantibiotics provide opportunities to solve these problems. Herein, we present mercaptophenylboronic acid (MBA)-modified gold nanoclusters with well-defined molecular formulas and structure (Au44(MBA)18) and excellent antibacterial activities against various drug-resistant bacteria such as vancomycin-resistant Enterococcus faecalis (VRE). Au44(MBA)18 interacts with bacteria by first attaching to teichoic-acid and destroying the cell wall and subsequently binding to the bacterial DNA. Au44(MBA)18 could be administered via multiple routes and has a high biosafety (500 mg/kg, no ototoxicity), overcoming the two major shortcomings of vancomycin (sole administration route and high ototoxicity). Our study is insightful for curing infections caused by multidrug-resistant bacteria using nanoantibiotics with high biosafety.

PB/CeO2 Nanoparticles Regulating Reactive Oxygen Species for the Control of Enterococcus faecalis Infection in Root Canals

Huiwen Wang,Yuting Wu,Mingrui Dai,Tingting Zhu,Daming Wu,Diya Leng

To investigate the antibacterial effect, mechanism, and cytotoxicity of Prussian blue/Cerium dioxide (PB/CeO 2 ) nanoparticles against Enterococcus faecalis (E. faecalis) and biofilm. PB/CeO 2 nanoparticles were synthesized and characterized. The antibacterial mechanism of nanoparticles was explored through peroxidase (POD) activity assay, hydroxyl radicals (·OH) detection, and measurement of bacterial reactive oxygen species (ROS) and glutathione (GSH)/glutathione disulfide (GSSG) levels. The biocompatibility of PB/CeO 2 was evaluated by Cell Counting Kit-8 (CCK-8) assay and histological examination of the major visceral organs of rats. The antibacterial effect of PB/CeO 2 was assessed using the colony-forming unit (CFU) method. The impact of PB/CeO 2 on E. faecalis biofilm on dentin slices was further observed with CLSM and SEM. ANOVA and t -test were applied for statistical analysis ( p < 0.05). PB/CeO 2 demonstrated significant antibacterial activity against E. faecalis , mainly when used with H 2 O 2 , significantly enhancing its antibacterial effect and effectively disrupting E. faecalis biofilms on dentin slices. PB/CeO 2 nanoparticles catalyzed ROS production, disrupting the antioxidant defense system of E. faecalis cells, damaging bacterial cell membranes, and ultimately causing bacterial death. PB/CeO 2 nanoparticles exhibit good biocompatibility at appropriate concentrations in vivo and in vitro. The novel multifunctional nanocomposite shows great antibacterial effects against E. faecalis and its biofilm, with low cytotoxicity and good biocompatibility, offering a novel disinfection strategy for root canal treatment.

Multifunctional nano-delivery system based on DNase I and photodynamic therapy for combatting enterococcus faecalis biofilm infections

Xia Meng,Cao Shisheng,Liu Zeqi,Zhao Jiakang,Yuan Yibo,Ni Jianhui,Guo Zhengjian,Wang Weihan,Yin Huijuan,Zou Zhaohui

Persistent or refractory apical periodontitis is primarily caused by microbial retention, as conventional root canal treatment often fails to eliminate infections completely, and systemic antibiotic therapy is insufficient to achieve effective concentrations for eradicating bacterial biofilms within root canals. This highlights the urgent need for novel therapeutics offering safe and effective antimicrobial strategies. Antimicrobial photodynamic therapy (aPDT) is a promising approach for root canal disinfection. However, commonly used photosensitizers such as Ce6 suffer from poor water solubility and strong aggregation tendencies, resulting in limited penetration into infected sites. In this study, we developed a DNase I-Lip@Ce6 nanodelivery system by combining deoxyribonuclease I (DNase I) with liposome-encapsulated Ce6. The liposomal carrier facilitated efficient delivery of Ce6 into target bacterial cells, while DNase I degraded extracellular DNA in the biofilm matrix, weakening its protective barrier. This synergistically enhanced Ce6 penetration and therapeutic efficacy, leading to the successful eradication of planktonic Enterococcus faecalis and in vitro biofilms. This strategy offers a novel approach for the precision treatment of persistent oral infections and holds strong potential for clinical translation.
