Portable disease monitoring is made possible by point-of-care testing (POCT), which can help prevent clinical deterioration. The widespread adoption of POCT, however, is limited by a shortage of efficient labeling probes. To address this, we developed monodisperse selenium nanoparticles (SeNPs) modified with polyethylene glycol (PEG-SeNPs) to improve their performance as labels. The nanoparticles transform from an aggregated to a monodisperse state after PEG regulation, with an average particle size of 50.23 ± 4.04 nm. This improvement increased the number of antibody binding sites on the surface of each nanoparticle from 34.2 to 91.6. Meanwhile, this study method’s sensitivity, stability and accuracy have all greatly increased with the use of artificial intelligence analysis. Relative to conventional lateral flow immunoassay (LFIA) systems, the LOD of this research achieved 0.3 ng/mL for NT-proBNP detection without compromising stability and specificity. Clinical sample validation showed 97.85% agreement with a chemiluminescence immunoassay. As a result, the LFIA platform based on PEG-SeNPs has outstanding analytical performance and is a practical POCT approach for early disease detection, prognostic monitoring and evaluation in illness self-management.

Purpose Rapid and accurate detection of acute heart failure (AHF) enables effective treatment of HF. This study aimed to to establish a timely test using colloidal selenium for initial screening and risk assessment of HF in primary care settings and at home. Methods Colloidal selenium was synthesized by reduction of sodium selenite with vitamin C under ambient conditions. In order to improve the stability of colloidal selenium and the coupling efficiency of the antibody, a novel synthetic method to coat polyethylene glycol 20000 (PEG20000) and sodium dodecyl sulfate (SDS) on colloidal selenium was developed. In order to improve the detection performance of colloidal selenium test strips, the labeling conditions and construction processes were optimized by the controlled variable method, and finally the test strips were successfully prepared. Results PEG20000 and SDS modified colloidal selenium had a very low detection limit of 250pg/mL, which met the sensitivity criteria for the diagnosis of acute heart failure in the Chinese Guidelines for the Diagnosis and Treatment of Heart failure (2024) that NT-proBNP ≤300 pg/mL can exclude acute heart failure, and ≤125 pg/mL can exclude chronic heart failure. Conclusion The developed single-step immunochromatographic method utilizing PEG20000 and SDS-modified colloidal selenium demonstrates meaningful potential for the rapid and reliable detection of NT-proBNP in serum samples from clinical patients. The colloidal selenium immunochromatographic technique developed in this study was assessed from the aspect of material synthesis, and the conditions for the application of this test paper were screened to verify the high specificity of the test paper, which met the criteria of the Chinese Heart Failure Diagnosis and Treatment Guidelines 2024 for acute heart failure.

Catalytic nanoparticle@metal-organic framework (MOF) composites have attracted significant interest in point-of-care testing (POCT) owing to their prominent catalytic activity. However, the trade-off between high loading efficiency and high catalytic activity remains challenging because high concentrations of nanoparticles tend to cause the misjoining and collapse of the MOFs. Herein, a facile strategy is reported to encapsulate high concentrations of platinum (Pt) nanoparticles into zeolitic imidazolate framework-8 (ZIF-8) using polydopamine (PDA) as a support for Pt@ZIF-8 and as a flexible scaffold for further immobilization of Pt nanoparticles. The resulting composite (Pt@ZIF-8@PDA@Pt) exhibits ultrahigh Pt nanoparticle loading efficiency, exceptional catalytic activity, stability, and a bright colorimetric signal. Following integration with lateral flow immunoassay (LFIA), the detection limits for pre- and post-catalysis detection of B-type natriuretic peptide (NT-proBNP) are 0.18 and 0.015 ng mL −1 , respectively, representing a 6-fold and 70-fold improvement compared to gold nanoparticle-based LFIA. Moreover, Pt@ZIF-8@PDA@Pt-based LFIA achieves 100% diagnostic sensitivity for NT-proBNP in a cohort of 184 clinical samples.

Apoptosis is a major pathophysiological change following myocardial ischemia/reperfusion (I/R) injury. Glucagon-like peptide 1 (GLP-1) and its receptor GLP-1R are widely expressed in the cardiovascular system and GLP-1/GLP-1R activates the protein kinase G (PKG)-related signaling pathway. Therefore, this study tested whether semaglutide, a new GLP-1 analog, inhibits I/R injury-induced cardiomyocyte apoptosis by activating the PKG/PKCε/ERK1/2 pathway. We induced myocardial I/R injury in rats and hypoxia/reoxygenation (H/R) injury in H9C2 cells and detected the effects of semaglutide, a PKG analog (8-Br-cGMP), and a PKG inhibitor (KT-5823) on the PKG/PKCε/ERK1/2 pathway and cardiomyocyte apoptosis. We found that semaglutide upregulated GLP-1R levels, and both semaglutide and 8-Br-cGMP activated the PKG/PKCε/ERK1/2 pathway, inhibited myocardial infarction (MI), decreased hs-cTNT levels, increased NT-proBNP levels, and suppressed cardiomyocyte apoptosis in I/R rats and H/R H9C2 cells. However, KT-5823 exerted contrasting effects with semaglutide and 8-Br-cGMP, and KT-5823 weakened the cardioprotective effects of semaglutide. In conclusion, semaglutide inhibits I/R injury-induced cardiomyocyte apoptosis by activating the PKG/PKCε/ERK1/2 pathway. The beneficial effect of GLP-1/GLP-1R, involved in the activation of the PKG/PKCε/ERK1/2 pathway, may provide a novel treatment method for myocardial I/R injury.
