Islets transplantation is a promising method to treat type 1 diabetes. Unfortunately, current islet encapsulation and transplantation strategies are difficult to simultaneously achieve the minimally invasive transplantation, rapid mass transportation, accurate positioning, long-term stability and easy retrieval. Herein, we present a novel strategy for islet encapsulation and transplantation that integrates the advantages of microencapsulation and macroencapsulation. Specifically, islets are first encapsulated in methacrylated carboxymethyl chitosan (CMCSMA) microgels by droplet microfluidic technology, and then oxidized sodium alginate-succinimide ester (OSA-NHS) as the assembling agent is added to time-sequentially assemble the islets-laden microgels. The as-formed microgel assembly (MA-CHO/NHS), or namely bioartificial pancreas, is injectable before transplantation due to the quick formation of dynamic Schiff base bonds, and becomes quite stable after transplantation with the slow formation of stable amide bonds. In addition, the interconnected micropores inside the microgel assembly is beneficial for mass transportation, and the abundant aldehyde and succinimide ester groups on assembling agent ensures good tissue adhesion. This injectable, tissue-adhesive, microporous, positionable and retrievable bioartificial pancreas can reverse the blood glucose levels of diabetic mice from hyperglycemia to normoglycemia for at least 90 days, and thus shows a broad application prospect in the field of diabetes treatment.

Diabetic foot ulcers (DFUs), a serious and increasingly common chronic issue among diabetics, often do not respond well to generalized treatment strategies. Hypoxia and the overexpression of reactive oxygen species (ROS), resulting in inflammatory dysregulation and subsequent imbalance in macrophage phenotypes, are critical factors contributing to the prolonged non-healing of DFU wounds. These two issues often interact in a continuous, problematic cycle. Presently, there is a lack of comprehensive strategies aimed at addressing both of these factors simultaneously to interrupt this detrimental cycle. Herein, an immunomodulatory hydrogel (PHG2) is developed for reshaping the hostile DFU microenvironment. The engineered PHG2 not only removes excess internally-produced ROS but also generates O 2 , with its efficiency further boosted by local hyperthermia (42.5 °C) activated by near-infrared light. Through both in vitro and in vivo studies, along with transcriptomic assessment, it is confirmed that PHG2 disrupts the detrimental feedback loop between ROS and inflammation while also lowering the M1/M2 macrophage ratio. Such discoveries contribute to a significant enhancement in the healing process of injuries that undergo a gradual increase in movement, covering wounds from the back, mouth, to the foot. Ultimately, this method provides an easy, safe, and highly effective solution for treating DFUs.

Wound repair is a complex and dynamic process, involving stages of hemostasis, inflammation, proliferation, and remodeling. Standard methods currently used for clinical treatment of skin defects remain unsatisfying and alternative treatments have emerged including acupuncture mostly relying on the conventional needle method. It is applied sustained acupuncture therapy with bioengineered stem cell-loaded hydrogel microspheres (A-SGC), where the data gathered through transcriptomics, metabolomics, and single-cell omics analyses are used to construct a comprehensive multi-omics map underlying the wound regeneration process with such a therapy. Under the treatment of A-SGC, the interactions and Yin-Yang balance among fibroblasts, macrophages, endothelial cells, and keratinocytes facilitated skin regeneration instead of scarred healing. The genes related to the promotion of matrix synthesis and re-epithelialization played important roles in the transition from the inflammatory granulation phase to the stromal remodeling phase. This convergence use of hydrogel microspheres, stem cells, and sustained acupuncture therapy could have previously unattainable therapeutic implications for improved wound treatment.

The decellularized extracellular matrix (ECM) of cartilage is a widely used natural bioscaffold for constructing tissue-engineered cartilage due to its good biocompatibility and regeneration properties. However, current decellularization methods for accessing decellularized cartilaginous tissues require multiple steps and a relatively long duration to produce decellularized cartilage. In addition, most decellularization strategies lead to damage of the microstructure and loss of functional components of the cartilaginous matrix. In this study, a novel decellularization strategy based on a hydrostatic pressure (HP) bioreactor was introduced, which aimed to improve the efficiency of producing integral decellularized cartilage pieces by combining physical and chemical decellularization methods in a perfusing manner. Two types of cartilaginous tissues, auricular cartilage (AC) and nucleus pulposus (NP) fibrocartilage, were selected for comparison of the effects of ordinary, positive, and negative HP-based decellularization according to the cell clearance ratio, microstructural changes, ECM components, and mechanical properties. The results indicated that applying positive HP improved the efficiency of producing decellularized AC, but no significant differences in decellularization efficiency were found between the ordinary and negative HP-treated groups. However, compared with the ordinary HP treatment, the application of the positive or negative HP did not affect the efficiency of decellularized NP productions. Moreover, neither positive nor negative HP influenced the preservation of the microstructure and components of the AC matrix. However, applying negative HP disarranged the fibril distribution of the NP matrix and reduced glycosaminoglycans and collagen type II contents, two essential ECM components. In addition, the positive HP was beneficial for maintaining the mechanical properties of decellularized cartilage. The recellularization experiments also verified the good biocompatibility of the decellularized cartilage produced by the present bioreactor-based decellularization method under positive HP. Overall, applying positive HP-based decellularization resulted in a superior effect on the production of close-to-natural scaffolds for cartilage tissue engineering.Impact statementIn this study, we successfully constructed a novel hydrostatic pressure (HP) bioreactor and used this equipment to produce decellularized cartilage by combining physical and chemical decellularization methods in a perfusing manner. We found that positive HP-based decellularization could improve the production efficiency of integral decellularized cartilage pieces and promote the maintenance of matrix components and mechanical properties. This new decellularization strategy exhibited a superior effect in the production of close-to-natural scaffolds and positively impacts cartilage tissue engineering.

Hemostatic powder is widely employed for emergency bleeding control due to its ability to conform to irregularly shaped wounds, ease of use, and stable storage. However, current powders exhibit limited tissue adhesion and insufficient support for thrombus formation, making them easily washed away by blood. In this study, a hybrid powder (QAL) was produced by mixing quaternized chitosan (QCS) powder, catechol-modified alginate (Cat-SA) powder, and laponite (Lap) powder. Upon addition of QAL, the blood quickly transformed to a robust and adhesive blood gel. The adhesion strength of the blood gel was up to 31.33 ± 1.56 kPa. When compared with Celox, QAL showed superior performance in promoting hemostasis. Additionally, QAL exhibited effectiveness in eliminating bacteria while also demonstrating outstanding biocompatibility with cells and blood. These favorable properties, including strong coagulation, adhesion to wet tissue, antibacterial activity, biosafety, ease of use, and stable storage, make QAL a promising emergency hemostatic agent.

Inflammatory bowel disease (IBD) is characterized by the upregulation of reactive oxygen species (ROS) and dysfunction of gut immune system, and microbiota. The conventional treatments mainly focus on symptom control with medication by overuse of drugs. There is an urgent need to develop a closed-loop strategy that combines in situ monitoring and precise treatment. Herein, we innovatively designed the ‘cluster munition structure’ theranostic microgels to realize the monitoring and therapy for ulcerative colitis (a subtype of IBD). The superoxide anion specific probe (tetraphenylethylene-coelenterazine, TPC) and ROS-responsive nanogels consisting of postbiotics urolithin A (UA) were loaded into alginate and ion-crosslinked to obtain the theranostic microgels. The theranostic microgels could be delivered to the inflammatory site, where the environment-triggered breakup of the microgels and release of the nanogels were achieved in sequence. The TPC-UA group had optimal results in reducing inflammation, repairing colonic epithelial tissue, and remodeling microbiota, leading to inflammation amelioration and recovery of tight junction between the colonic epithelium, and maintenance of gut microbiota. During the recovery process, the local chemiluminescence intensity, which is proportional to the degree of inflammation, was gradually inhibited. The cluster munition of theranostic microgels displayed promising outcomes in monitoring inflammation and precise therapy, and demonstrated the potential for inflammatory disease management.

After a nuclear accident, radioactive nuclides are rapidly absorbed through wounds into the body, causing serious internal radiation injury. In this study, we synthesized chitosan methacryloyl (CSMA) and graft-modified it with the broad-spectrum chelating agent diethylenetriamine pentaacetate (DTPA) to prepare a DTPA-modified chitosan hydrogel dressing (CSMA-g-DTPA). The compounds and hydrogels were examined for their physical and chemical properties and their biological safety was verified. The effectiveness of CSMA-g-DTPA dressings for early decontamination and promotion of wound healing was evaluated in vivo. The results indicated that CSMA-g-DTPA exhibits good mechanical properties and swelling behavior as well as excellent biocompatibility. In vitro experiments revealed that CSMA-g-DTPA is capable of removing uranium and strontium. In vivo experiments demonstrated that the CSMA-g-DTPA hydrogel can decontaminate radioactive Sr-89 and U-238 from rat skin wounds, prevent the absorption of Sr-89 and U-238 through the wounds and alleviate oxidative stress and inflammatory responses in the wounds, thereby promoting wound healing. Overall, the DTPA-modified chitosan hydrogel dressing has good mechanical performance, biosafety and decontamination effects on strontium and uranium. It has potential applications for emergency decontamination of wounds during the early stages of a nuclear disaster.

Wound bacterial infections not only impede the healing process but can also give rise to a range of serious complications, thereby posing a substantial risk to human health. Developing effective wound dressings incorporating phototherapy functionalities, specifically photothermal therapy (PTT) and photodynamic therapy (PDT), remains a critical area of research in modern wound care. Existing PTT-PDT systems often suffer from challenges such as nanoparticle aggregation and inefficient reactive oxygen species (ROS) generation, which are essential for therapeutic efficacy. This study presents a pioneering approach by combining zeolitic imidazolate framework derivatives (ZIFs) and Au NPs in a silk fibroin (SF) hydrogel for the first time. This combination not only prevents particle aggregation but also significantly enhances photothermal conversion efficiency and ROS generation capacity. The digital light processing (DLP) printability of our hydrogel allows for customized wound dressings tailored to individual patient needs, improving therapeutic efficacy. The hydrogel's effectiveness was evaluated through rigorous in vivo experiments, demonstrating enhanced antibacterial properties and accelerated wound healing. The biocompatibility of our hydrogel ensures its suitability for clinical applications, minimizing adverse reactions while promoting healing. A wound healing rate of 99.06 % represents a substantial improvement over the control groups, indicating markedly enhanced therapeutic efficacy. These findings underscore its multifunctionality in addressing infected wounds, presenting a promising strategy for facilitating the rapid healing of acute complex wounds in clinical applications.

Due to the limited ability to self-repair, the regeneration of bone critical-sized defects (CSD) is a significant challenge. Bone tissue engineering scaffolds are considered promising candidates for CSD repair, but low cell infiltration efficiency and a lack of nutrients greatly restrict bone regeneration abilities. Herein, we developed a dynamic culturing of large biomimetic bone scaffolds, PCL/GelMA@cells that combining 3D printed polycaprolactone (PCL) multi-channel cylinder with gelatin methacryloyl (GelMA) encapsulated with bone marrow mesenchymal stem cells (BMSCs) and rat aortic endothelial cells (RAECs). A cell dynamic culture system was fabricated to simulate the dynamic microenvironment. Compared to static culturing, dynamic culturing proved to enhance the nutrient exchange within the large scaffold to promote the cells infiltration, growth, proliferation and induce osteogenic and angiogenic differentiation. Furthermore, a rat cranial CSD (D = 10 mm) repair model verified the accelerated vascular ingrowth and new bone formation with the implantation of dynamic culturing of PCL/GelMA@cells scaffold (∼10 times higher than Blank group), indicating the great potential of dynamical culturing of scaffolds for bone repair. In summary, the results highlight the significant advantages of the dynamical culturing of cell-loaded scaffolds for bone regeneration, offering a promising strategy for addressing critical size bone defects.

Three-dimensional (3D) bioprinting embedded within a microgel bath has emerged as a promising strategy for creating intricate biomimetic scaffolds. However, it remains a great challenge to construct tissue-scale structures with high resolution by using embedded 3D bioprinting due to the large particle size and polydispersity of the microgel medium, as well as its limited cytocompatibility. To address these issues, novel uniform sub-microgels of cell-friendly cationic-crosslinked kappa-carrageenan (κ-Car) are developed through an easy-to-operate mechanical grinding strategy. These κ-Car sub-microgels maintain a uniform submicron size of around 642 nm and display a rapid jamming-unjamming transition within 5s, along with excellent shear-thinning and self-healing properties, which are critical for the high resolution and fidelity in the construction of tissue architecture via embedded 3D bioprinting. Utilizing this new sub-microgel medium, various intricate 3D tissue and organ structures, including the heart, lungs, trachea, branched vasculature, kidney, auricle, nose, and liver, are successfully fabricated with delicate fine structures and high shape fidelity. Moreover, the bone marrow mesenchymal stem cells encapsulated within the printed constructs exhibit remarkable viability exceeding 92.1% and robust growth. This κ-Car sub-microgel medium offers an innovative avenue for achieving high-quality embedded bioprinting, facilitating the fabrication of functional biological constructs with biomimetic structural organizations.

Articular cartilage defects caused by trauma or osteoarthritis remain a significant challenge in clinical practice due to their poor self-healing capacity. Current clinical approaches are generally limited by their complex procedures or certain postoperation complications. Therefore, it is crucial to develop an effective strategy for one-step cartilage repair with long-term functional cartilage regeneration. This study reports the creation of an injectable hydrogel composed of cartilage decellularized extracellular matrix (CdECM) and hyaluronic acid methacrylate (HAMA). In the presence of photoinitiator ruthenium (Ru)/sodium persulfate (SPS), visible light (450 nm) not only triggered the polymerization of CdECM and HAMA to form stable hydrogel in 1 min but also facilitated the tyrosine cross-links between biotissue and CdECM without any further moiety grafting, which cannot be achieved by routinely used photoinitiators and provides the hybrid hydrogel with bioadhesive properties. This dual-network hybrid hydrogel demonstrated robust adhesion strength to cartilage and significantly enhanced mechanical performance in comparison to that of CdECM or HAMA alone. Further in vitro culture demonstrated the promotive effect of hybrid hydrogel on encapsulated porcine bone marrow mesenchymal stem cells toward chondrogenic differentiation after 21 days of incubation compared with HAMA. Through in vivo subcutaneous implantation in a mouse model for two and 4 weeks, we further illustrated that the hybrid hydrogel has improved biocompatibility in comparison to HAMA alone. Thus, the presented hydrogel is a promising biomaterial for cartilage regeneration through minimally invasive arthroscopic injection in clinical practice.

In articular cartilage defect, particularly in arthroscopy, regenerative hydrogels are urgently needed. It should be able to firmly adhere to the cartilage tissue and maintain sufficient mechanical strength to withstand approximately 10 kPa of arthroscopic hydraulic flushing. In this study, we report a carbene-mediated ultra adhesive hybrid hydrogel paints for arthroscopic cartilage repair, which combined the photo initiation of double crosslinking system with the addition of diatomite , as a further reinforcing agent and biological inorganic substances. The double network consisting of ultraviolet initiated polymerization of hyaluronic acid methacrylate (HAMA) and carbene insertion chemistry of diazirine-grafted gelatin (GelDA) formed an ultra-strong adhesive hydrogel paint (H2G5DE). Diatomite helped the H2G5DE hydrogel paint firmly adhere to the cartilage defect, withstanding nearly 100 kPa of hydraulic pressure, almost 10 times that in clinical arthroscopy. Furthermore, the H2G5DE hydrogel supported cell growth, proliferation, and migration, thus successfully repairing cartilage defects. Overall, this study demonstrates a proof-of-concept of ultra-adhesive polysaccharide hydrogel paints, which can firmly adhere to the articular cartilage defects, can resist continuous hydraulic pressure, can promote effective cartilage regeneration, and is very suitable for minimally invasive arthroscopy.
