Membranous glomerulopathy (MGN) is an autoimmune kidney disease that is the primary cause of nephrotic syndrome (NS) in adults. Tetrandrine, a bisbenzylisoquinoline alkaloid, is known to have numerous pharmacological effects. In this study, network pharmacology analysis and experimental validation were conducted to analyze the mechanisms by which tetrandrine functions as a therapeutic intervention for MGN. A systematic network pharmacology method was applied to identify potential targets and determine the therapeutic mechanism of tetrandrine in MGN treatment. A Heymann nephritis (HN) rat model was developed to assess the therapeutic effects of tetrandrine on NS and validate the predicted molecular mechanisms. We obtained 86 potential targets of tetrandrine for the treatment of NS. In vivo experiments showed that tetrandrine could reduce the 24-h urine protein content, decrease glomerular basement membrane proliferation, and significantly decrease thylakoid stroma and cell proliferation in the HN rat kidney tissue. Moreover, tetrandrine suppressed kidney cell apoptosis and upregulated the expression of nephrin and podocin in HN model rats. qRT-PCR results revealed that tetrandrine inhibited IL-1β, TNFα, and MCP-1 levels in HN model rats. Western blot results indicated that tetrandrine can protect against MGN via the PI3K/Akt signaling pathway. Thus, by using a combination of network and experimental pharmacology methods, we demonstrate that tetrandrine can treat MGN via the PI3K/Akt signaling pathway and provide novel insights into the mechanisms underlying tetrandrine-mediated management of MGN.

A simple and selective method was developed for the simultaneous determination of tetrandrine and fangchinoline in herbal medicine by HPLC with electrochemical detection (ECD) on a bare glassy carbon electrode. The HPLC separation and ECD conditions have been optimized. The separation was carried out on a WondaSil ® C18-WR column (4.6 mm × 250 mm, 5 μm), with the mobile phase of acetonitrile–ammonium acetate buffer (pH 6.5; 40 mM) (32:68, v/v) using an isocratic elution at the flow rate of 0.5 mL/min. The electrochemical detection potential was set at +0.9 V. The obtained LODs for tetrandrine and fangchinoline were 0.26 and 0.27 μmol/L, respectively. The method was successfully applied to the analysis of tetrandrine and fangchinoline contents in Stephania tetrandra S. Moore. It has been demonstrated that the LC-ECD method is an excellent technique for analysis of the herbal medicine. The mean recoveries were in the range of 95–105%, while the precision expressed as repetition of peak area was lower than 2.7%.

Ischemic stroke is the most common type of stroke and brings about a big disease burden because of high mortality and disability in China. Tetrandrine, a bisbenzylisoquinoline alkaloid isolated from the Chinese herb Radix Stephania tetrandra, has been demonstrated to possess anti-inflammatory and free radical scavenging effects and even regulate astrocyte activation, but the possible role of tetrandrine in ameliorating cerebral ischemia/reperfusion injury of ischemic stroke remains unknown. The aim of this study was to determine the effects of tetrandrine on neurological injury and differential proteomic changes induced by transient reversible middle cerebral artery occlusion (MCAO) in mice. Male Balb/c mice were divided into sham (n = 30), MCAO + saline as control (n = 30), and MCAO + Tet as tetrandrine-treated (n = 30) groups. Mice in the control and tetrandrine-treated groups underwent 120 min of MCAO following reperfusion. Immediately and 2 h after MCAO, the mice received either normal saline (sham operated and control groups) or tetrandrine (tetrandrine-treated group) intraperitoneally. Neurological defects, brain water content, and infarct volume at 24 h after stoke were used to evaluate neurological injury extent. Treatment with tetrandrine not only mitigated cerebral neurological deficits ( P < 0.05) and infarct size ( P < 0.01), but also decreased brian edema in the ischemic brain ( P < 0.05). Then, fluorescence two-dimensional difference in gel electrophoresis was used to detect our systematic differential profiling of proteomic changes responding to tetrandrine administration. We validated that the expression of GRP78, DJ-1 and HYOU1 was associated with neuroprotective effect of tetrandrine in MCAO model by Western blotting. These findings indicate a potential neuroprotective role of tetrandrine for ischemic stroke and yield insights into cellular and molecular mechanisms of tetrandrine taking place in ischemic stroke.

Idiopathic pulmonary fibrosis (IPF), a lethal respiratory disease with few treatment options, occurs due to an excessive process of epithelial-mesenchymal transition (EMT) and abnormal accumulation of extracellular matrix (ECM), eventually leading to fibrotic scarring and destruction of alveolar structures. Here, a bioinspired inhalable nanocomplex (CCMφ@PTNPs) is reported for inhibiting the EMT progress and clearing the intrapulmonary ECM, thereby reversing established fibrotic foci in IPF. The nanocomplex consists of a PLGA core loaded with Chinese herbal tetrandrine (TET) and pirfenidone (PFD), and a macrophage cell membrane (Mφ) shell modified with collagenase type I (CLN) and collagen-targeting peptide (CBP). Following inhalation, CCMφ@PTNPs achieves precise abilities of targeting and degrading collagen barrier with the aid of CBP and CLN, ultimately achieving efficient penetration and drug delivery of fibrotic lesions in alveoli. The co-delivery of PFD and TET not only inhibited the transforming growth factor-β1(TGF-β1) pathway to achieve collagen modulation but also restored TGF-β1-induced impaired autophagy, which ultimately reduced local collagen production and accelerated clearance, thus synergistically enhancing the lung function of the bleomycin-induced mouse model. In summary, this study proposes a novel inhalation drug delivery strategy for the treatment of IPF.

In this work, a sensitive glassy carbon electrode modified with palladium-doped electrochemically reduced graphene oxide (Pd@ErGO/GCE) was developed for the selective and simultaneous determination of tetrandrine (TET) and fangchinoline (FAN). The electrochemical behavior was investigated by cyclic voltammetry (CV) and a multimodal voltammetric platform was established: (1) differential pulse voltammetric (DPV) quantification of TET (LOD = 55 nM), (2) linear sweep voltametric (LSV) determination of total content (LOD = 45 nM), and (3) simultaneous determination of both analytes via DPV. The methods have great reproducibility and repeatability, great interference tolerance, and satisfactory accuracy in serum (recovery: 95.7% ~ 104%) and urine (97.4% ~ 103%). Comprehensive validation against HPLC-UV ( t = 0.32) confirmed method accuracy for pharmaceutical quality control in Radix Stephaniae tetrandrae. This methodology offers a reliable framework for alkaloid monitoring in herbal and biological matrices.

10-Hydroxycamptothecin is a drug to cure various cancers. However, the 10-hydroxycamptothecin cannot be widely applied in clinics due to fast elimination and resistance of various cancers to the drug. Nevertheless, co-treatment with tetrandine is known to reverse the resistance of multi-drug resistant cancers, and may present an effective strategy to improve the efficacy of 10-hydroxycamptothecin. In order to improve the bioavailability and prolong the treatment time of the 10-hydroxycamptothecin in vivo, we prepared 10-hydroxycamptothecin-tetrandrine liposome complexes with 10-hydroxycamptothecin as the basic anticancer drug, tetrandrine and liposomes as carriers. In this article, an ultra-high performance liquid chromatography tandem mass spectrometry method for the analysis of 10-hydroxycamptothecin and tetrandrine in plasma has been developed, validated, and utilized to compare the pharmacokinetics of both drugs in the original dosage form and administered as liposome complexes. According to the pharmacokinetic parameters of mean residence time, half-life period and clearance rate, the 10-hydroxycamptothecin-tetrandrine liposome complexes prolongs the retention and circulation time of 10-hydroxycamptothecin in vivo, achieving a good sustained release effect. To the best of our current knowledge, the pharmacokinetic properties of 10-hydroxycamptothecin-tetrandrine liposome complexes in rats have not been reported yet. Our study can provide a helpful reference for further related study.

Paclitaxel is a broad-spectrum anti-cancer drug by targeting microtubulin. However, multidrug resistant (MDR) makes its clinical application more difficult and results in failure of chemotherapy. Tetrandrine as a potential multidrug resistant modulator could be combined with other anti-cancer drugs. In this study, ultra-performance liquid chromatography (UHPLC) combined with quadrupole time-of-flight mass spectrometry (QTOF) was applied to simultaneously qualitative and quantitative analysis of paclitaxel for the pharmacokinetic studies while combined with tetrandrine. This method was developed based on non-target screening mode IDA (Information Dependent Acquisition). As a result, the validated range was 0.25–64 ng/ml (30 µl plasma) for paclitaxel. Totally 33 metabolites of paclitaxel and tetrandine were identified in vivo and in vitro. The main metabolites of PTX were dose-dependent decreased with different amounts of tetrandine co-administration no matter in vivo and in vitro, the exposure of PTX increased in pharmacokinetic study. The verified method is sensitive accurate and effective for the simultaneous determination of paclitaxel and its metabolites in blood, urine and live microsome incubation samples and it was successfully applied to evaluate the pharmacokinetics and drug–drug interaction between paclitaxel and tetrandine. Furthermore, a biosensor technology, surface plasmon resonance (SPR) analysis was applied to preliminary evaluate the competitive protein binding of multiple components. The SPR analysis indicated that the affinity between 6-hydroxy-paclitaxel and micotubulin is similar to that between paclitaxel and micotubulin, and tetrandrine also does not form a competitive combination with paclitaxel. For human, 6-hydroxy-paclitaxel is the one of main metabolites of paclitaxel, so the results suggested that tetrandine has an influence on the metabolite of paclitaxel, but tetrandine and the main metabolites of PTX probably do not affect PTX's biological targeting, the effect of its pharmacological action needs to be further studied.
