Rationale : Ferritin with unique hollow cavity is an emerging protein-based nanoplatform for anticancer-drug delivery, but the in vivo chemotherapeutic effectiveness is still unsatisfactory with such a monotherapy modality, which is urgently in need of improvement. Methods : Here a novel ferritin nanotheranostic with anticancer-drug doxorubicin encapsulated into its hollow interior and nanoradiosensitizer bismuth sulfide nanocrystals inlayed onto its polypeptide shell was synthesized for combinational therapeutic benefits. The formation mechanism of bismuth sulfide nanocrystals based on ferritin has been analyzed. The in vitro and in vivo treatment effects were carried out on HeLa cancer cells and tumor-bearing mice, respectively. The biocompatibility and excretion of the ferritin nanotheranostic have also been evaluated to guarantee their biosafety. Results : The polypeptide shell of ferritin provides nucleation sites for the bismuth sulfide nanocrystals through coordination interaction, and simultaneously inhibits the further growth of bismuth sulfide nanocrystals, rendering the bismuth sulfide nanocrystals like rivets inlaying onto the polypeptide firmly, which can not only strengthen the architectural stability of ferritin to prevent drug burst leakage during systemic circulation, but also act as excellent computed tomography contrast agents and nanoradiosensitizers for in vivo imaging-guided cancer combinational treatments. Conclusions : The design concept of inlaying bismuth sulfide nanocrystals onto the polypeptide shell of doxorubicin-encapsulated ferritin significantly inhibits the tumor growth and simultaneously further broadens the application of ferritin in nanomedicine.

Pancreatic ductal adenocarcinoma (PDAC) has a low survival rate and limited treatment options. Concurrent chemoradiotherapy is considered beneficial to improve tumor control, but the low drug bioavailability at tumor site and the low radiation tolerance of surrounding healthy organs greatly limits its effectiveness. Lipiodol, a natural drug carrier used in clinical transarterial chemoembolization, has shown potential as a radiosensitizer due to its high Z element iodine composition. Thus, this study aims to repurpose lipiodol as a sensitizer to simultaneously enhance chemo- and radiotherapy for PDAC. To this end, a stable lipiodol emulsion (IOE) loaded with gemcitabine is designed using clinically approved surfactants. At in vivo level, IOE demonstrates better radiotherapeutic effect than existing nanoradiosensitizers and enhanced drug bioavailability over free drug, leading to significant tumor inhibition and improved survival rates under concurrent chemo-radiotherapy. This may due to the sustained drug release, homogenous spatial distribution, and long-term retention ability of IOE in solid PDAC tumor. Furthermore, to better understand the functioning mechanism of drug-loaded IOE, in vitro study is conducted to reveal the ROS- and DNA damage-related therapeutic pathways. Lastly, a comprehensive toxicity assessment also proves the good biocompatibility and safety of as-prepared IOE. This study offers a clinically feasible sensitizer for simultaneous chemoradiotherapy and holds potential for other types of cancer treatment in clinics.

Radiotherapy (RT) is a common modality in the clinical management of breast cancer. However, the radiation damage to healthy tissues caused by the high-dose radiation and the limited therapeutic efficacy with DNA damage repair pose significant challenges in RT. In this work, an intelligent responsive nanoplatform with dual effect radiosensitization (mZGSSM-AMD) has been developed to improve the efficacy of RT. The doping of Lu promotes the generation of reactive oxygen species (ROS) under X-ray irradiation, thereby facilitating DNA breakage. To hinder the repair of damaged DNA, NO is loaded into mZGSSM-AMD, enhancing damage of tumor during radiotherapy. Furthermore, persistent luminescence (PersL) recovers with the degradation of MnO 2 in tumor microenvironment (TME), which guides precision radiosensitization of tumors. Experimental results indicate that growth of tumors is significantly suppressed by effective radiosensitization of mZGSSM-AMD. The mZGSSM-AMD with dual effect radiosensitization exhibits exceptional antitumor effectiveness, providing promising application prospects for the imaging-guided precise radiosensitization therapy.

Radiation therapy is one of the core means of tumor treatment, especially in local control and radical treatment has an irreplaceable role. However, radiotherapy resistance is one of the major challenges in current clinical practice. Tumor cells have a strong ability to repair DNA damage, which can effectively resist DNA double-strand breaks caused by X-ray, thus weakening the killing effect of radiotherapy. In addition, the complexity of the tumor microenvironment (TME) will further reduce the sensitivity of radiotherapy, leading to poor treatment results. With the rapid development of nanotechnology, multi-modal combined therapy nanoplatform has gradually become a new strategy to overcome radiotherapy resistance. These nanoplatforms achieve synergies by integrating multiple therapeutic approaches, such as radiation sensitization, photothermal therapy and chemotherapy. In this study, we utilized ZIF-8, a member of the metal-organic frameworks, to simultaneously load ICG and Rapamycin for X-ray sensitization and combined photothermal therapy. In this formulation, Rapamycin enhances tumor cells' sensitivity to radiotherapy by inhibiting the mTOR signaling pathway, increasing DNA damage, regulating the cell cycle, and stimulating the STING pathway, which amplifies the tumor immune response. Meanwhile, ICG, as a photosensitizer, effectively converts light energy into heat, achieving tumor photothermal ablation. The modified drug-delivery system becomes a tumor-microenvironment-responsive smart carrier, increasing tumor cell uptake, prolonging retention at the tumor site, and achieving targeted drug delivery. It releases drugs in the specific tumor microenvironment, enhancing photothermal and radiotherapy sensitization effects. The results show that the smart dual-loaded nanoplatform effectively combines photothermal therapy and external-beam sensitization, reshapes the immunosuppressive tumor microenvironment and significantly inhibits tumor proliferation in the HepG2 subcutaneous xenograft model, demonstrating marked antitumor activity and good biosafety.

Chemoradiotherapy is a well-established treatment paradigm in oncology to consistently improve local tumor control compared to the sole administration of chemotherapy or radiotherapy. Despite its importance, few agents have been identified to further improve the therapeutic ratio and reduce the incidence of complications. Advances in nanomedicine have offered innovative strategies to improve chemoradiotherapy. In the present study, we evaluated the efficacy and safety of FePt nanoparticles (NPs) in chemoradiotherapy using HeLa cells and HEK293T cells. FexPt100−x NPs at different compositions (x = 26, 53 and 77) were synthesized and characterized by means of X-ray diffraction, transmission electron microscopy, and Fourier transform infrared spectroscopy. After ligand exchange, the cytotoxicity of FexPt100−x NPs was evaluated by MTT assay in HEK293T cells while the composition effects of FePt NPs on the cytotoxicity and the potential application of FePt NPs in combination with X-ray radiation at clinically relevant MV energies were investigated in HeLa cells in vitro. Besides, the cellular uptake of NPs was measured indirectly by atomic absorption spectroscopy and transmission electron microscopy. The results indicated that FexPt100−x NPs inhibited the growth of HeLa cells in a concentration- and composition-dependent manner after 24 h incubation, with low cytotoxicity to HEK293T cells at the given concentrations (0–20 μg mL−1). Furthermore, the combination of FePt NPs and radiotherapy resulted in a marked inhibition of HeLa cells, in contrast with that of the individual FePt NPs treated group or the radiation alone group. Moreover, Fe53Pt47 NPs, exhibiting significant cytotoxicity and enhanced radiosensitization effects on HeLa cells without damage to HEK293T cells, might theoretically satisfy the ultimate goal of personalized chemoradiotherapy. Our present work exhibited high therapeutic efficacy of FePt NPs in combination with radiotherapy without apparent cytotoxicity, suggesting the potential of FePt NPs as a promising nanoprobe in improving the outcome of tumor chemoradiotherapy.

ABSTRACT Developing a multi-functional radiosensitizer with high efficiency and low toxicity remains challenging. Herein, we report a mesoporous heterostructure radiosensitizer ( [email protected] ) containing Lu-based upconversion nanophosphor (UCNP) and Bi-based nanomaterial loaded with iron phthalocyanine for X-ray and NIR light dual-triggered tri-modal tumor therapy. NaLuF 4 :Yb,Tm, a Lu-based UCNP, offers radiosensitization and upconversion luminescence for optical bio-imaging. However, Bi has a higher X-ray mass attenuation coefficient than Lu . Thus, after stepwise fabrication, Na 0.2 Bi 0.8 O 0.35 F 1.91 :Yb (NBOF) was assembled with the UCNP to form a mesoporous heterostructure composite. This enhanced the radiosensitization effect and drug load to realize multi-modal tumor therapy. After coating it with folate-conjugated amphiphilic PEG (PFA), [email protected] realized tumor photothermal/photodynamic/radio-therapy. The structure of [email protected] was well characterized. Different properties triggered by X-ray and NIR light were evaluated. Finally, a highly efficient tumor ablation effect was demonstrated in vitro and in vivo . Consequently, this kind of nanocomposite provides a unique strategy for designing a theranostic platform for oncotherapy. Statement of Significance The synergy of enhanced radiotherapy and photothermal/photodynamic therapy is found to improve tumor therapeutic efficacy. On that basis, a heterostructure nanohybrid containing Lu-based UCNP and Bi-based mesoporous material is synthesized. The heterostructure nanohybrid can be loaded with FePc and decorated with folate-modified amphiphilic PEG to form a multi-functional theranostic nano-platform. The platform exhibits upconversion luminescence capacity, X-ray attenuation property, photothermal effect, and X-ray and NIR dual-light triggered ROS generation capability. These features can not only enable upconversion luminescence/CT bioimaging of living beings but also be applied to the photothermal/photodynamic/radio- synergistic tumor ablation. To sum up, the nanomaterial offers a novel method for the construction of a new theranostic platform.

Radiotherapy is recommended as a modality for cancer treatment in clinic. However, cancerous cells were resistant to therapeutic irradiation due to its DNA repair. In this work, single-walled carbon nanotubes with unique physical properties of hollow structures and high specific surface area were introduced as carrier for iron-palladium (FePd) to obtain iron-palladium decorated carbon nanotubes (FePd@CNTs). On one hand, FePd nanoparticles possess significant ability in radiosensitization as previously reported. On the other hand, carbon nanotubes offer higher efficiency in crossing biological barriers, inducing the accumulation and retention of FePd nanoparticles within tumor tissue. In order to verify the radiosensitization effect of FePd@CNTs, both in vitro and in vivo experiments were conducted. These experiments showed that the FePd@CNTs exhibited remarkably better radiosensitization effect and more obvious accumulation than FePd NPs, suggesting a potential of FePd@CNTs in radiosensitization.

Anticancer treatment is largely affected by the hypoxic tumor microenvironment (TME), which causes the resistance of the tumor to radiotherapy. Combining radiosensitizer compounds and O2self-enriched moieties is an emerging strategy in hypoxic-tumor treatments. Herein, we engineered GdW10@PDA-CAT (K3Na4H2GdW10O36·2H2O, GdW10, polydopamine, PDA, catalase, CAT) composites as a radiosensitizer for the TME-manipulated enhancement of radiotherapy. In the composites, Gd (Z = 64) and W (Z = 74), as the high Z elements, make X-ray gather in tumor cells, thereby enhancing DNA damage induced by radiation. CAT can convert H2O2to O2and H2O to enhance the X-ray effect under hypoxic TME. CAT and PDA modification enhances the biocompatibility of the composites. Our results showed that GdW10@PDA-CAT composites increased the efficiency of radiotherapy in HT29 cells in culture. This polyoxometalates and O2self-supplement composites provide a promising radiosensitizer for the radiotherapy field.Keywords:radiosensitizer;polyoxometalates;catalase;O2self-supplement;radiotherapy

The integration of local radiation therapy (RT) with transcatheter arterial chemoembolization (TACE) can effectively manage unresectable hepatocellular carcinoma (HCC). Nonetheless, achieving effective synergistic treatment efficacy by controlled on-dem, radiosensitization following TACE remains a significant issue. A nanoradiosensitizer-Lipiodol formulation (DSeSeP-API(Lip)) is developed using Touch-, -Go following X-ray irradiation (X-tag) for the theranostics of HCC. The nanoradiosensitizer (DSeSeP-API) is fabricated by conjugating ultrasmall AuPtICG (API) nanoparticles featuring an amphiphilic polymer with X-ray responsive diselenide motif (DSPE-Se-Se-PEG-SH), satisfying the ‘3 s ’ properties: 1) s table in Lipiodol; 2) s ensitive to X-ray; 3) phase- s hift from Lipiodol to the aqueous physiology. The homogeneous, stable lipiodol formulation is subsequently prepared via our super-stable homogeneous intermixed formulation technology (SHIFT). Following the successful transcatheter arterial embolization (TAE), the subsequent RT program commences. The initial X-ray irradiation is administered concurrently with the cleavage of the diselenide bond to facilitate the phase transition of hydrophilic SeP-API from Lipiodol ( 1 X-Tag). Secondary X-ray irradiation is applied at the peak cellular uptake of sensitizers for enhanced X-ray dynamic therapy (XDT) ( 2 X-ray). The research illustrates the capability of the nanoradiosensitizer-Lipiodol formulation with the “X-tag” feature to amplify the synergistic TAE−XDT therapeutic effects while effectively minimizing adverse effects.

Preoperative radiotherapy is a cornerstone of treatment for locally advanced rectal cancer, but intrinsic tumor cell radio resistance remains a primary obstacle limiting therapeutic efficacy. Herein, for the first time, we successfully constructed Hf 0.7 Ti 0.3 O 2 @PEG (HT) nanoparticles by introducing titanium (Ti) into hafnium dioxide (HfO 2 ). These HT nanoparticles synergistically combined the superior X-ray energy deposition capability of Hf and the efficient radiation-induced ROS generation performance of Ti, leading to a 1.4-fold higher ROS yield compared to HfO 2 . Moreover, HT nanoparticles significantly enhanced radiosensitivity by inducing extensive DNA damage and oxidative stress in tumor cells. This resulted in a tumor inhibition rate of 87.7% ± 2.1% in mouse models. Notably, HT nanoparticles enabled a marked reduction in radiation dose while maintaining therapeutic efficacy and safety, overcoming the major limitation of conventional HfO 2 nanoradiosensitizers in ROS generation. This study presents a promising new approach for rectal cancer radiotherapy and holds significant potential for improving outcomes in radioresistant rectal cancer.

Radiotherapy is an effective anti-cancer therapy for patients with non-small cell lung cancer (NSCLC), however, the prognosis is unsatisfactory owing to radio-resistance and toxicity. It is crucial to improve radiotherapy efficacy. Ag nanoparticles (NPs) and ginsenoside Rg3 (Rg3) exerted
antitumor and radio-sensitization effects. Therefore, we investigated whether poly-lactide-co-glycolide-polyethylene glycol (PLGA-PEG)-Rg3-Ag will function as a noninvasive, tracing, radiotherapy sensitizer. The morphology of NPs was visualized with transmission electron microscopy (TEM).
The drug loading content, encapsulation efficiency, and cumulative drug release of Rg3 was determined by HPLC. Cellular uptake of NPs in A549 and SPCA-1 was measured by immunostaining. The radio-sensitization effect of PLGA-PEG-Rg3-Ag in vitro was determined in A549 by detecting proliferation,
colony formation, and apoptosis with CCK-8, clonogenic survival assay, and flow cytometry, while in vivo was determined in nude mice by testing the body weight and tumor volume. PLGA-PEG-Rg3-Ag exerted radio-sensitization effect by reducing cell proliferation and colony formation while
enhancing cell apoptosis in A549; reduced tumor volume in nude mice. PLGA-PEG-Rg3-Ag exhibits radio-sensitization effects in NSCLC.

The small difference between tumor and normal tissues in their responses to ionizing radiation has been a significant issue for radiotherapy of tumors. Herein, we report that dumbbell-shaped heterogeneous copper selenide-gold nanocrystals can serve as an efficient radiosensitizer for enhanced radiotherapy. The mean lethal dose of X-rays to 4T1 tumor cells can be drastically decreased about 40%, that is, decreasing from 1.81 to 1.10 Gy after culture with heterostructures. Due to the synergetic effect of heterostructures, the dose of X-rays is also much lower than those obtained from mixture of Cu2–xSe + Au nanoparticles (1.78 Gy), Cu2–xSe nanoparticles (1.72 Gy) and Au nanoparticles (1.50 Gy), respectively. We demonstrate that the sensitivity enhancement ratio of Cu2–xSe nanoparticles was significantly improved 45% (i.e., from 1.1 to 1.6) after the formation of heterostructures with gold. We also show that the heteronanocrystals exhibit an enhanced photothermal conversion efficiency, due to the synergetic interactions of localized surface plasmon resonance. These properties highly feature them as a multimodal imaging contrast agent (particularly for photoacoustic imaging, computed tomography imaging, and single photon emission computed tomography after labeled with radioisotopes) and as a radiosensitizer for imaging guided synergetic radiophotothermal treatment of cancer. The research provides insights for engineering low-Z nanomaterials with high-Z elements to form heteronanostructures with enhanced synergetic performance for tumor theranostics.
