Metabolic Division of Labor and Functional Differences Among IDH Isoenzymes
Metabolic Division of Labor and Functional Differences Among IDH Isoenzymes
Isocitrate dehydrogenase (IDH) is a key enzyme family involved in the tricarboxylic acid cycle, NADPH supply, α-ketoglutarate generation, and cellular redox homeostasis. In mammalian cells, the major isoenzymes include IDH1, IDH2, and IDH3. IDH1 and IDH2 are NADP⁺-dependent enzymes and are more involved in reducing power supply and metabolic regulation, while IDH3 is NAD⁺-dependent and is one of the core rate-limiting enzymes in the mitochondrial tricarboxylic acid cycle. Although all three use isocitrate as a substrate, they differ significantly in cellular localization, cofactor dependence, reaction direction, metabolic function, and disease association.
Keywords: IDH1; IDH2; IDH3; isocitrate dehydrogenase; tricarboxylic acid cycle; NADPH; α-ketoglutarate; 2-hydroxyglutarate; mitochondrial metabolism
1 Basic Functions of the IDH Family
1.1 Catalytic Reaction
The IDH family catalyzes the oxidative decarboxylation of isocitrate to generate α-ketoglutarate (α-KG), accompanied by reduction of a cofactor. According to cofactor type, IDH enzymes can be divided into NADP⁺-dependent IDH1/IDH2 and NAD⁺-dependent IDH3.
IDH1 and IDH2 mainly generate NADPH, and their reactions are closely associated with cellular reducing power, antioxidant defense, and anabolic metabolism. IDH3 mainly generates NADH and directly supports mitochondrial oxidative phosphorylation and energy metabolism.

2 Functional Characteristics of IDH1
2.1 Cellular Localization
IDH1 is mainly located in the cytosol and can also be present in peroxisomes. Because it is not located in the mitochondrial matrix, IDH1 more directly affects cytosolic NADPH levels, lipid synthesis, redox buffering, and cytosolic α-KG supply.
Cytosolic NADPH can be used for glutathione reduction, maintenance of the thioredoxin system, fatty acid synthesis, and cholesterol synthesis. Therefore, IDH1 is not only a metabolic enzyme but also an important node connecting redox homeostasis and anabolic metabolism.
2.2 Metabolic Functions
(1) NADPH supply
IDH1 generates NADPH by reducing NADP⁺, supporting maintenance of the reduced state of the GSH/GSSG and thioredoxin systems. When cells face ROS, drug stimulation, or metabolic stress, IDH1 activity can affect cytosolic antioxidant capacity.
(2) Lipid and biosynthesis
IDH1 contributes to cytosolic reducing power supply and can support fatty acid and cholesterol synthesis. In highly proliferative cells and tumor cells, IDH1 is closely related to anabolic metabolic reprogramming.
(3) α-KG-related regulation
α-KG generated by IDH1 can participate in amino acid metabolism and α-KG-dependent dioxygenase reactions, including DNA and histone demethylation-related processes. Changes in IDH1 activity may therefore influence the connection between metabolism and epigenetics.
2.3 Disease Association
IDH1 mutations are commonly found in certain gliomas, acute myeloid leukemia, cholangiocarcinoma, chondrosarcoma, and other tumors. Mutant IDH1 gains an abnormal catalytic function and reduces α-KG to D-2-hydroxyglutarate (2-HG). 2-HG can competitively affect α-KG-dependent enzymes, thereby altering DNA methylation, histone modification, and cellular differentiation status.
3 Functional Characteristics of IDH2
3.1 Cellular Localization
IDH2 is located in the mitochondrial matrix and is an NADP⁺-dependent IDH. Compared with IDH1, IDH2 is closer to the mitochondrial respiratory chain, exchange of tricarboxylic acid cycle intermediates, and the mitochondrial ROS-generating environment.
Mitochondria are an important source of cellular ROS. NADPH generated by IDH2 can support the mitochondrial glutathione and thioredoxin systems, helping clear hydrogen peroxide and lipid peroxides.
3.2 Metabolic Functions
(1) Mitochondrial antioxidant defense
IDH2 provides NADPH for mitochondria and is an important enzyme for maintaining the mitochondrial reducing environment. When IDH2 activity is insufficient, mitochondrial ROS can increase more easily, and membrane potential, ATP generation, and respiratory chain function may be impaired.
(2) Regulation of TCA intermediates
IDH2 can affect the balance among isocitrate, α-KG, and reducing power in mitochondria. Although IDH3 is the more direct NADH-generating enzyme in the TCA cycle, IDH2 is also important for mitochondrial metabolic buffering and oxidative stress adaptation.
(3) Relationship with reductive carboxylation
Under certain conditions such as hypoxia, limited mitochondrial function, or altered reducing pressure, NADP⁺-dependent IDH can participate in metabolic conversion toward reductive carboxylation, supporting carbon source supply for lipid synthesis and metabolic adaptation.
3.3 Disease Association
IDH2 mutations can also lead to 2-HG generation and are commonly found in acute myeloid leukemia, some gliomas, and other tumors. Unlike IDH1, IDH2 mutations occur in the mitochondrial environment, so their effects are more directly related to mitochondrial metabolism, reducing power balance, and regulation of cellular differentiation.
4 Functional Characteristics of IDH3
4.1 Cellular Localization and Structure
IDH3 is located in the mitochondrial matrix and is an NAD⁺-dependent isocitrate dehydrogenase. Mammalian IDH3 is composed of IDH3A, IDH3B, and IDH3G subunits and has a complex-like structural feature. Compared with IDH1/IDH2, IDH3 is more typically a tricarboxylic acid cycle enzyme.
4.2 Role in the TCA Cycle
IDH3 catalyzes the oxidative decarboxylation of isocitrate to generate α-KG while producing NADH. NADH then enters the electron transport chain to drive oxidative phosphorylation and ATP generation. Therefore, IDH3 directly affects mitochondrial energy metabolism flux.
Under physiological conditions, the IDH3 reaction tends to be irreversible and is an important control step in the TCA cycle. Its activity is regulated by cellular energy status. ADP and Ca²⁺ can enhance its activity, whereas elevated ATP and NADH levels can inhibit the reaction, thereby matching TCA flux with cellular energy demand.
4.3 Differences from IDH1/IDH2
The main function of IDH3 is not to provide NADPH, but to support energy generation through NADH. It is not mainly characterized by typical neomorphic 2-HG-producing mutations in tumors, nor does it serve as the primary buffer for cytosolic or mitochondrial reducing power. IDH3 abnormalities are more likely to manifest as reduced TCA cycle flux, insufficient NADH generation, and mitochondrial energy metabolism impairment.
5 Differences Among the Three Isoenzymes in Redox Homeostasis
5.1 Functional Division Between NADPH and NADH
IDH1 and IDH2 generate NADPH, which is mainly used for reductive biosynthesis and maintenance of antioxidant systems. IDH3 generates NADH, which mainly enters the respiratory chain for ATP production. Although NADPH and NADH are both reduced cofactors, they have different functional directions and cannot simply replace each other.
5.2 Differences in Antioxidant Defense
IDH1 mainly maintains the cytosolic reducing environment and affects cytosolic GSH, thioredoxin, and lipid synthesis-related reduction reactions. IDH2 mainly maintains the mitochondrial reducing environment and affects mitochondrial ROS, membrane potential, and respiratory chain stability. IDH3 indirectly affects cellular stress resistance by maintaining the TCA cycle and energy metabolism.
5.3 Relationship with ROS
When IDH1 or IDH2 activity decreases, insufficient NADPH supply can weaken antioxidant defense and reduce ROS clearance capacity. When IDH3 activity decreases, restricted TCA cycle activity, reduced NADH supply, and decreased respiratory chain efficiency may indirectly aggravate oxidative stress through energy impairment and abnormal electron transfer.
6 Differences Among the Three Isoenzymes in Tumor Metabolism
6.1 IDH1/IDH2 Mutations and 2-HG
IDH1 and IDH2 mutations can produce a neomorphic function that reduces α-KG to 2-HG. 2-HG accumulation affects α-KG-dependent dioxygenases, resulting in abnormal epigenetic regulation and blocked cellular differentiation. This is an important metabolic feature of IDH1/IDH2-mutant tumors.
6.2 IDH3 and Tumor Energy Metabolism
IDH3 is not mainly characterized by 2-HG-producing mutations. Its significance in tumors is more often reflected in TCA cycle flux, mitochondrial respiration, α-KG supply, and energy metabolic adaptation. Changes in IDH3 function may affect cell proliferation, hypoxic adaptation, and mitochondrial metabolic status.
6.3 Differences in Detection Strategies
IDH1/IDH2 studies often focus on mutation sites, 2-HG levels, NADPH changes, and epigenetic effects. IDH3 studies are more suitable for focusing on TCA cycle flux, NADH/NAD⁺ ratio, oxygen consumption rate, ATP generation, and mitochondrial function.
7 Cooperative Relationships Among the Three Isoenzymes in Metabolic Networks
7.1 Isocitrate and α-KG Nodes
IDH1, IDH2, and IDH3 all act on the metabolic node between isocitrate and α-KG, but their different cellular localization gives them different roles. IDH1 is more oriented toward cytosolic metabolic regulation, IDH2 toward maintenance of mitochondrial reducing power, and IDH3 toward energy generation.
7.2 Multiple Roles of α-KG
α-KG is not only a TCA cycle intermediate but also a key metabolite in amino acid metabolism, collagen synthesis, hypoxia signaling, and epigenetic regulation. IDH1/2/3 affect α-KG pools differently. In research, cytosolic α-KG, mitochondrial α-KG, and overall metabolic flux should be distinguished.
7.3 Metabolic Compensation
When the activity of one IDH isoform changes, cells may compensate through other metabolic pathways, such as glutaminolysis, malic enzyme, the pentose phosphate pathway, or mitochondrial metabolic reprogramming. Detecting one IDH protein alone is insufficient to explain overall metabolic status; cofactors, metabolites, and functional indicators should be analyzed together.
8 Detection Strategies for IDH1, IDH2, and IDH3
8.1 Protein and Gene Expression Detection
Western blot, ELISA, qPCR, and immunofluorescence can be used to detect expression levels of IDH1, IDH2, and IDH3A/IDH3B/IDH3G. Protein expression can reflect enzyme abundance, but it cannot fully represent enzyme activity or metabolic flux.
8.2 Enzyme Activity Detection
IDH1/IDH2 activity detection should focus on NADP⁺/NADPH conversion, while IDH3 activity detection should focus on NAD⁺/NADH conversion. If the reagent system does not distinguish cofactor type, NADP⁺-dependent and NAD⁺-dependent IDH activities can easily be confused.
8.3 Metabolite Detection
α-KG, isocitrate, citrate, 2-HG, NADPH/NADP⁺, NADH/NAD⁺, and related indicators can be used to evaluate IDH functional status. In IDH1/2 mutation studies, 2-HG is an important metabolic marker. In IDH3 functional studies, changes in NADH, ATP, and TCA intermediates are more informative.
8.4 Mitochondrial Function Detection
Both IDH2 and IDH3 are located in mitochondria, but their detection priorities differ. IDH2 is more suitable for analysis together with mitochondrial ROS, GSH/GSSG, NADPH, and membrane potential. IDH3 is more suitable for analysis together with oxygen consumption rate, ATP, NADH/NAD⁺, and TCA flux.
9 Selection in Common Research Scenarios
9.1 Tumor Mutations and Metabolic Reprogramming
When studying IDH-mutant tumors such as glioma, AML, or cholangiocarcinoma, priority should be given to IDH1/IDH2 mutations, 2-HG accumulation, DNA/histone methylation status, and cellular differentiation phenotype. IDH3 may serve as a background indicator of energy metabolism, but it is not the core focus of typical mutant IDH-driven 2-HG research.
9.2 Mitochondrial Oxidative Stress
When studying mitochondrial ROS, decreased membrane potential, and antioxidant defense, IDH2 is usually more direct than IDH1. If energy supply and respiratory chain function are also observed, IDH3, ATP, oxygen consumption rate, and NADH/NAD⁺ indicators should be included.
9.3 Cytosolic Reducing Power and Lipid Synthesis
When studying cytosolic NADPH, lipid synthesis, and reductive biosynthesis, IDH1 is more representative. Such experiments can combine NADPH/NADP⁺, fatty acid synthesis indicators, GSH/GSSG, and ROS detection.
9.4 TCA Cycle Flux and Energy Metabolism
When studying the tricarboxylic acid cycle, oxidative phosphorylation, and ATP generation, IDH3 is the more direct detection target. Changes in IDH3 activity should be analyzed together with citrate, isocitrate, α-KG, NADH/NAD⁺, oxygen consumption rate, and ATP.
